This substance is most excellent for inducing the eruption of smallpox, or it is particularly good to take it mixed with a decoction prepared from *Arnebiae Radix et Rhizoma* (Zicao, 紫草) and *Glycyrrhizae Radix et Rhizoma* (Gancao, 甘草).
Wanquan San (from Master Ruan): Treats smallpox eruptions that lack ruddy color.
Ingredients: *Saposhnikoviae Radix* (Fangfeng, 防风), *Panacis Ginseng Radix* (Renshen, 人参), *Bombyx Batryticatus* (Chantui, 蝉蜕) in equal parts.
Preparation and Administration: Finely cut the herbs. Take four qian per dose, add one sheng of water, three leaves of *Menthae Haplocalycis Herba* (Bohe, 薄荷), decoct until six fen remains, and take warm. For patterns with excess heat and fullness, add *Rhizoma Cimicifugae* (Shengma, 升麻).
Anban Tang (from Master Tang):
Ingredients: *Arnebiae Radix et Rhizoma* (Zicao, 紫草), *Akebiae Caulis* (Mutong, 木通) each five fen; *Bombyx Batryticatus* (Chantui, 蝉蜕), *Saposhnikoviae Radix* (Fangfeng, 防风) each three fen; *Glycyrrhizae Radix et Rhizoma* (Gancao, 甘草) two fen.
Preparation and Administration: Finely cut the herbs. Add one zhong of water, decoct until five fen remains, and take warm.
Another pattern: In individuals with exterior excess, where skin fat is thick and muscle pores are dense, toxic factors cannot easily dissipate, leading to slow eruption. It is appropriate to use Xiaodu Yin (Detoxification Decoction) or Touji San (Muscle-Penetrating Powder). If there is hard constipation, add *Radix Rheomi* (Dahuang, 大黄) and *Semen Ziziphi Spinosae* (Zhizi Ren, 栀子仁) to Xiaodu Yin and decoct. If eruptions are too dense, Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) or Zhangshi Jiedu Fangfeng Tang (Master Zhang’s Detoxification and Wind-Dispersing Decoction) is appropriate. For deficiency of blood and qi, Shiqi San (Ten Wonders Powder) is appropriate; for throat obstruction, Rusheng Tang (Like-Sage Decoction) added with *Menthae Haplocalycis Herba* (Bohe, 薄荷) and *Aurantii Fructus* (Zhike, 枳壳) is appropriate. For hot breath in the mouth, sore throat, and sores on the tongue and mouth, Ganlu Yin (Sweet Dew Decoction) is appropriate. For convulsions due to wind-stroke, Baolong Wan (Dragon-Holding Pill) is appropriate. For irritability and thirst, Dushen Tang (Single Ginseng Decoction) or Huang [Lian] Liuyi Tang ([Coptis] Six-One Decoction) is appropriate.
Touji San:
Ingredients: *Arnebiae Radix et Rhizoma* (Zicaorong, 紫草茸), *Rhizoma Cimicifugae* (Lvshengma, 绿升麻), *Glycyrrhizae Radix et Rhizoma* (Fengancao, 粉甘草) each one qian.
Preparation and Administration: Finely cut, decoct in water and take, or decoct together with Xiaodu Yin for superior effect.
Rusheng Tang:
Ingredients: *Platycodonis Radix* (Jiegeng, 桔梗) two qian, *Glycyrrhizae Radix et Rhizoma* (Gancao, 甘草) one qian.
Alternative Formula: Add *Bupleuri Radix* (Niubangzi, 牛蒡子) and *Ophiopogonis Tuber* (Maimendong, 麦门冬) each one qian.
Preparation and Administration: Finely cut, decoct in water, and take frequently.
Baolong Wan (See section on Convulsions/Wind-Stroke).
Differentiating Favorable and Unfavorable External Signs (13):
Warm body indicates favorable prognosis; cold body indicates unfavorable. Ability to eat and solid stool indicate favorable; inability to eat and loose stool indicate unfavorable.
Differentiating Severity of External Signs (14):
Mild cases: Erupt in three waves, varying in size, sparse on head and face, none in ears, roots are red and vibrant, plump and glossy.
Severe cases: Erupt all at once, densely distributed like silkworm eggs, grayish-white, accompanied by diarrhea and thirst, body warm but abdomen distended, head warm but feet cold.
Differentiating Early and Late Manifestations of Smallpox (15):
Mild cases: Pathogen is in the Fu organs, erupting as fine rashes resembling mosquito bites, dotted red, commonly known as Bran Pox. Severe cases: Pathogen is in the Zang organs, forming true smallpox. Resembling peas, with red roots and white heads, pustules exuding pus and water, commonly known as Smallpox. Visible after two to three days, faintly emerging like millet grains, or like mung beans, or like water droplets; those that are glossy and clear are good. On the fourth day, varying sizes with red roots and glossiness indicate mild cases; those that are dense, depressed at the apex, and accompanied by diarrhea indicate severe cases. On the sixth and seventh days, plump, red, and glossy lesions indicate mild cases; those with fever, panting, dry mouth, abdominal distension, and cold toes indicate severe cases. On the eighth and ninth days, fully grown, red, and full lesions with waxy color indicate mild cases; those with chills, confusion, abdominal distension, irritability, thirst, rapid breathing, and teeth grinding indicate extreme severity. On the tenth and eleventh days, when scabs form and fall off, recovery is imminent. On the twelfth and thirteenth days, failure to scab when expected is unfavorable; if the body is slightly hot, use purgatives to prevent residual toxicity; if not high fever, or with abdominal distension, diarrhea, or thirst, use Shi’erwei Yigong San (Twelve-Ingredient Extraordinary Merit Powder) to rescue.
Differentiating Self-Healing Without Medication (16):
Sparse footprints of pox, red and spreading roots, no diarrhea or thirst, normal milk/food intake, warm limbs, no high fever.
For the above six patterns, medication is not required; only careful nursing is needed. The room must be kept warm, avoiding all foul odors. Avoid contact with outsiders, sexual activity, menstruating women, and those with axillary odor. Burning *Radix Rheomi* (Dahuang, 大黄) and *Atractylodis Rhizoma* (Cangzhu, 苍术) is recommended to dispel evil qi. Burning Agarwood, Sandalwood, Aquilariae Lignum Resinatum (Xiangchen, 沉香), Dalbergiae Odoriferae Lignum (Tanxiang, 檀香), Liquidambaris Resina (Ruxiang, 乳香), and Borneolum Syntheticum (Nashexiang, 脑麝) should be avoided. Hanging Coriandrum Sativum Herba (Husui, 胡荽) inside curtains, spraying bed curtains with Husui infusion, and burning Costus Root (Muxiang, 木香) are beneficial. The toxicity of smallpox fears foul odors most; do not trust monks or Daoists to chant scriptures to cleanse impurities, as they have no power and may introduce more foul odors. Do not rely on them without prevention. Beware!
Differentiating Five Incurable Patterns (17):
Itching and collapse, chills with teeth grinding, unquenchable thirst. Purple-black pox, panting and restless. Grayish-white, depressed apex, abdominal distension. Warm head, cold feet, confusion with desire to drink. Rapid breathing, diarrhea, thirst.
Differentiating Yin and Yang Patterns of Rashes (18):
Red rashes belong to Yang, subsiding with cooling. White rashes belong to Yin, subsiding with warming.

(Ancestral Formula)
After smallpox, regardless of which meridian the abscess/toxin appears, when initially red and swollen, use Black, Green, and Red beans soaked in sour vinegar, ground into paste. Brush frequently with a goose feather; it recedes immediately with miraculous effect.
(Medical Case)
A child had not had a bowel movement for twenty days after smallpox. Stool was dry and painful, nearing death. Previously used *Radix Rheomi*, *Mirabilite*, *Aurantii Fructus*, *Crotonis Fructus*, honey suppositories, and drank about a bowl of sesame oil, all ineffective. I instructed someone to hold real sesame oil in their mouth, insert a small bamboo tube into the rectum, and blow the oil into the intestines. Bowel movement occurred shortly after. Truly an excellent formula.
Essential Methods of Ancients for Treating Smallpox (21)
Master Wang’s *Zhi Mi* states: Smallpox is a type of seasonal epidemic; one person contracts it and infects others. Also states: Rashes with heat erupt easily, covering the skin upon eruption.
Master Zhang Huan states: Uniform qi leads to fast eruption; blood follows qi, qi reversal causes blood stagnation.
Master Wang states: Rashes are generated by the Spleen. Spleen deficiency with Liver excess allows Wood to overcome Earth; heat agitates the Heart Spirit causing fright.
Master Qian states: Liver Wind and Heart Fire struggle causing convulsions. Also states: Convulsions before rash formation strongly contraindicate cooling the Heart. Rashes belong to the Heart; Heart governs Blood. Heart cold prevents Blood circulation, preventing rash eruption. Be very cautious. Generally, treating fright requires soothing the Liver, promoting urination, and regulating Qi.
Master Yang Renzhai states: For high heat, promote urination with Wuling San (Five-Ingredient Powder with Poria) or Dao Chi San (Guide Out Redness Powder). For mild heat, detoxify with Xiaodu Yin or Sisheng San (Four Saints Powder).
Depression, Inversion, and Black Depression:
1. Pattern transforming into Kidney-related black depression: Qian’s Baixiang Wan (Baixiang Pills) and Xuanfeng San (Wind-Releasing Powder) are appropriate.
2. Pattern caused by external Wind-Cold: In winter, Wuji San (Five Accumulations Powder) minus *Ephedrae Herba* (Mahuang, 麻黄), plus *Cinnamomi Cortex* (Guixin, 桂心) and *Arnebiae Radix et Rhizoma* (Zicao, 紫草). In spring, Buhuanjin Zhengqi San (Unchanging Metal Upright Qi Powder) plus *Chuanxiong Rhizome*, *Angelicae Dahuricae Radix*, *Saposhnikoviae Radix*; or if attacked by Wind Evil, Xiaofeng San (Wind-Dispersing Powder) plus *Arnebiae Radix et Rhizoma*, taken concurrently.
3. Pattern caused by damage from milk/food, internal Qi obstruction: Yang’s Jieji San (Regulating and Harmonizing Powder) or Sijunzi Tang (Four Gentlemen Decoction) added with *Amomi Fructus* (Suosha, 缩砂), *Aquilariae Lignum Resinatum* (Muxiang, 木香), *Chuanxiong Rhizome*, *Arnebiae Radix et Rhizoma*. For spontaneous diarrhea, Fuzi Lizhong Tang (Aconite Regulate the Middle Decoction) is appropriate.
4. Pattern caused by parents' impropriety regarding sexual activity, menstruation, or nurse's axillary foul odor: Burn Ruan’s Bihui Dan (Evil-Dispelling Pellet) to fumigate. Spray bed curtains with Coriander Wine, hang Coriander in bed curtains. For severe cases, dissolve Suhexiang Wan (White Musk Pill) in Coriander Decoction, apply and fumigate. Shengzhi Zaisu San (Reviving Powder) is miraculously effective.
5. Pattern of toxicity entering interior causing black depression: Zhuwei Gao (Pig Tail Ointment) is highly effective.

Postscript by Mr. Weng of Cangzhou: Infants and children should be treated differently. Infants require treating the nurse simultaneously to ensure her Qi and Blood are clear and harmonious, diet regulated, and administered Qi-regulating and milk-promoting teas to nourish her milk, so the infant drinks it, resulting in plump, glossy sores without risk of depression. Children require pulse diagnosis to determine exterior/interior and deficiency/excess for sweating or purging. If neither deficient nor excess, maintain harmony, ensure Spleen Qi flows smoothly, aiding Lung Metal via Mother element, facilitating pus formation and scabbing quickly, avoiding collapse. If robust with dense pores, pre-emptive sweating is needed. If constipated or difficult urination, purging and draining are essential to avoid trouble. For other patterns, double the dosage, rarely failing. Scholars should ponder this.
