For recurrence of illness due to overexertion after a major disease has subsided, Zhishi Zhizi Chitang (Bitter Orange, Gardenia, and Fermented Soybean Decoction) is the primary treatment.
Zhishi (Immature Bitter Orange, four pieces, stir-fried), Zhizi (Gardenia Fruit, fourteen pieces), Chitang (Fermented Soybean, one tael).
Take the above three ingredients, add two cups of clear sour rice water, boil empty first until reduced to one cup, then add Zhishi and Zhizi, boil until eight-tenths remains, add Chitang, boil for five or six more boils, remove dregs, take warm, cover with blankets to induce slight sweating. If there is retained food, add Dahuang (Rhubarb Root), adjusting dosage according to deficiency or excess.
For fever recurring after the resolution of Cold Damage (Shanghan), Xiaochaihu Tang (Minor Bupleurum Decoction) is the primary treatment. If the pulse is floating, resolve it by inducing sweating; if deep and solid, resolve it by purging.
For edema/water qi accumulation below the waist after a major disease has subsided, Musi Zexie San (Oyster Shell and Alisma Powder) is the primary treatment.
Musi (Oyster Shell, calcined), Zexie (Alisma Rhizome), Shuqi (Prepared Aconite Lateral Root, washed to remove fishy smell), Shanglu (Phytolacca Root, calcined), Haizao (Sargassum Thallus, washed), Gualougen (Trichosanthes Root), Tinglizi (Descurainia Seed, each in equal parts).
Grind into powder, administer with water using a square-inch measure. Stop administration when urination becomes smooth, twice daily.
For lethargy and prolonged unresponsiveness after a major disease has subsided, caused by cold in the stomach, warm with pills, Lizhong Wan (Regulate the Middle Pill) is suitable.
For emaciation, shortness of breath, and nausea with desire to vomit after the resolution of Cold Damage, Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) is the primary treatment.
Danzhuye (Lophatherum Herb, two handfuls), Shigao (Gypsum Fibrosum, half a jin, crushed), Banxia (Pinellia Tuber, three taels), Renshen (Ginseng Root, three taels), Maimendong (Ophiopogon Tuber, four taels, heart removed), Gancao (Licorice Root, one tael, honey-fried). Add Shengjiang (Fresh Ginger Rhizome, one and a half taels) if vomiting occurs.
Chop the herbs, take five qian per dose, add one and a half cups of water, add over one hundred grains of Jingmi (Rice Grain), boil until eight-tenths remains and rice is cooked. Remove dregs, take warm.
For patients with wind-pathogen exterior syndrome who have resolved the exterior but remain unwell, recovery is expected in twelve days.
[Hai Zang]: Generally, "overexertion" refers to activity. Activity varies; there are differences between internal Qi/Blood exhaustion and external strain. If it is Qi overexertion, manifesting as weakness and mental fatigue, the method should be mild lifting/supporting. If it is Blood and sinew/bone overexertion, tonify with formulas like Siwu Tang (Four Substance Decoction). If the overexertion affects the Spleen internally, belonging to the Middle Jiao, regulating the middle can cure it. This is a tangible disease. Only observing external symptoms classifies it as recurrence, not true overexertion, such as re-contracting wind-cold.
[Ben Cao Reference]: Records a case where a patient suffered from Cold Damage, sweated, and after several days suddenly developed fever, spontaneous sweating, wiry-rapid pulse, and mental restlessness. This was true recurrence due to overexertion. I diagnosed: this resulted from excessive mental labor. The Spirit resides here; having not returned to its original state, further mental labor injured the Spirit, causing disharmony of Ying and Wei. One should tonify the Son organ, benefit the Spleen, and relieve the overexertion, hopefully leading to recovery. Prescribed Bupi Tang (Spleen-Tonifying Decoction), assisted by Xiaochaihu Tang to harmonize. Someone questioned: "Deficiency should tonify the Mother, why tonify the Son now?" I said: "Do you not know the difference between deficiency and overexertion? Nan Jing states: Deficiency tonifies the Mother, Excess drains the Son. Everyone knows deficiency requires tonifying the Mother. Qian Jin Fang states: For severe Heart overexertion, tonify Spleen Qi to benefit it; when Spleen prospers, it influences the Heart. For overexertion, one should tonify the Son, which is unheard of. The Mother generates me; the Son succeeds me and assists me. When treating deficiency, we tonify the one who generated us, consistent with Jin Nang's principle that if the root receives Qi, the branches receive shade. When treating overexertion, we tonify the one who assists us, consistent with Xunzi's saying that a son cannot be rich while the father is poor. This is why treating deficiency and overexertion differs."
Bupi Tang treats post-sweating recovery from Cold Damage where the Spleen/Stomach are injured by cold foods, causing chest/diaphragm discomfort and general Qi/Blood disharmony.
Renshen (Ginseng Root), Baizhu (Atractylodes Macrocephala Rhizome), Gancao (Licorice Root), Chuanpi (Tangerine Peel, white part removed), Qingpi (Green Tangerine Peel, white part removed), Ganjiang (Dried Ginger Rhizome, each in equal parts).
Grind into powder, take three qian per dose, add one cup of water, boil briefly, take hot. Adding salt is also acceptable.
[Hai Zang]: To treat recurrence due to overexertion, use Maimendong Tang (Ophiopogon Decoction). It is effective for those whose Qi is about to fail, capable of reviving the dying.
Maimendong (Ophiopogon Tuber, one tael), Gancao (Licorice Root, two taels, honey-fried), Jingmi (Rice Grain, ten he).
Remove the heart of Maimendong, grind into fine powder. Add two cups of water, cook Jingmi until soft, remove rice grains, retain about one and a half cups of decoction, add five qian of medicine, two dates (pitted), fifteen fresh Zhu Ye (Bamboo Leaves), simmer to one cup, remove dregs, take warm. For those unable to swallow, drip slowly into the mouth with cotton. Also suitable for infants who cannot be force-fed. This formula does not use Shigao because there is no heat in the Triple Burner, and for those near death with failing Yang, Shigao is contraindicated. Adding Renshen is highly beneficial.
[Wai Tai Mi Yao]: For insufficiency after major disease, classified as deficiency consumption, to tonify deficiency. Take one sheng of milk from a yellow cow aged five to seven years, add four sheng of water, boil down to one sheng. If hungry, drink slowly, do not drink too much. Ideally, continue taking it within ten days for best results.
[Zhou Hou Bei Ji Fang]: Treats critical illness newly recovered, early overexertion, excessive eating/drinking leading to recurrence, nearing death. Calcinate Biejia (Tartaric Turtle Carapace), take one square-inch measure.
[Huo Ren Shu]: Xiong Shoushi Tang (Male Rat Feces Decoction) treats recurrence due to overexertion.
Zhizi (Gardenia Fruit, fourteen pieces), Xiong Shoushi (Male Rat Feces, fourteen pieces, those pointed at both ends are authentic), Zhike (Bitter Orange Fruit, three pieces, stir-fried).
Grind into fine powder, take four qian per dose, add one and a half cups of water, add two cun of Congbai (Scallion White), thirty grains of Xiangchi (Fragrant Fermented Bean), simmer to one cup, divide into two doses. Do not let the patient know it contains rat feces.
Qiwei Congbai Tang (Seven-Ingredient Scallion White Decoction) treats Cold Damage recurrence due to movement/exertion or slightly excessive eating. If recurrence is severe, it resembles initial Cold Damage. For early signs, this formula is suitable.
Congbai (Scallion White, with roots), Gegen (Kudzu Root), Xinchi (New Fermented Bean, half he), Shengjiang (Fresh Ginger Rhizome, sliced, one he), Maimendong (Ophiopogon Tuber, heart removed), Shudi (Rehmannia Root, prepared, three qian).
Use flowing water, four sheng, agitate with a ladle.
Boil the above seven ingredients in clear water until reduced by two-thirds, remove dregs, divide into two doses, take gradually to induce sweating.
Regarding Cold Damage recurrence, it is most appropriate to follow Danxi's method of using Renshen, [missing character, likely Huangqi or Baizhu], Danggui (Angelica Sinensis), Baizhu, combined with diaphoretic agents to induce sweating.
