〔《本》〕治偏正头风，夹脑风，并一切头风，不问年深日近，克日取效，名透顶散。
细辛（表白者，三茎） 瓜蒂（七个） 丁香（三粒） 糯米（七粒） 脑子 麝香（各一，黑豆大。）
上将脑麝，乳钵内研极细，却将前四味研匀，另自治为末，然后入乳钵内，荡起脑麝令匀，用瓦罐子盛之，
谨闭罐口，患人随左右搐之，一大豆许，良久出涎一升许，则安。
东垣云∶湿热在头而头痛者，必以苦吐之。若用上项搐鼻药而涎少者，必兼下项吐法治之。
〔《圣》〕治痰厥头痛。以乌梅十个取肉，盐二钱，酒半盏，合水煎至七分去渣，非时温服，吐即住。
〔丹〕头痛难忍支，风痰所致。栀子末蜜和浓，涂舌上，吐即止。（《兵部集》）
头痛非冷非风，此膈有痰也。浓煎茶啜一二升，探吐之，吐而又吐，候苦汁出乃止，不损人，待渴自止妙。

〔子和〕茶调散
瓜蒂、好茶二味，等分为末，每服二钱，齑汁调下，空心用之。
丹霞僧，病头痛，常居暗室，不敢见明，其头热痛，以布围其巅，上置冰于其中，日易数次，热不能已。
诸医莫识其证，求见戴人。戴人曰∶此三阳蓄热故也。乃置炭火于暖室中，出汗涌吐，三法并行，七日方愈。
常仲明之子，自四岁得风痰疾，至十五岁转甚，每月发一二次，发必头痛，痛则击数百拳，出黄绿涎一二
盏方愈。此年发益频，目见黑花，昏不知人，三四日方省，诸医皆用南星、半夏化痰之药，终无一效。戴人
以双解散发汗，以苦剂吐痰，病去八九，续以分剂平调，自春至秋，如此数次，方获全瘥。
邪在胃而头痛者，必下之，其证必兼隔痞，或动作则痛甚，或右手脉滑盛者是也。治法并见脾胃条。
东风生于春，病在头，俞在颈项。（全文五脏。）春气者，病在头。（恶风为胃风。）
〔河〕新沐中风为首风，头面多汗恶风，当先风一日则病甚，至其风日则少愈，大川芎丸主之。
大川芎丸 治首风旋晕弦急，外合阳气，风寒相搏，胃膈痰饮，偏正头疼，身体拘蜷。
川芎（一斤） 天麻（四两，用郓州者）
上为末，炼蜜丸。每两作十丸，每服一丸，细嚼茶酒下，食后服。
风气循风府而上，则为脑风，项背怯寒，脑户极冷，神圣散主之。
神圣散 治脑风，邪气留饮不散，项背怯寒，头痛不可忍者。
麻黄（去节） 细辛（去苗） 干葛（生一半，炒一半） 藿香叶（各等分）
上为末，每服二钱，煮荆芥、薄荷，酒调下，茶亦得。并治血风证。
又方 治脑风邪气留饮，头疼不可忍者。用远志末，不以多少，于鼻中搐，于痛处揉之，相兼前药可用也。
〔丹〕头疼连眼痛，此风痰上攻，须用白芷开之。
雨前茶 川芎 白芷 防风 台乌 细辛 当归
为末，汤调服。
〔世〕偏正头风，川芎、细辛、防风、白芷、乌头尖、全蝎梢、南星姜制、麝香少许，为末，每一字，温酒调服。
〔丹〕头风方
羌活（三钱） 细辛（一钱） 川芎（二钱） 干葛（二钱） 甘草（一钱半） 防风（二钱） 半夏（一钱）
上除川芎、细辛二味外，研为末，用酒和炒，晒干，入川、细二味末和匀，每服一钱，茶清调下。
又方
荆芥 草乌尖（七片） 防风 甘草 台芎 蔓荆子 桔梗 麻黄
上为末，茶调服。
〔《本》〕治头疼，面赤，烦闷咽干，上膈风痰，头目晕昏，百节疼痛，背项拘急，芎辛丸。
川芎 防风 僵蚕 独活（各一两） 桔梗（三两） 麻黄 细辛 白附子 羌活 甘草（各四两） 薄
荷荆芥（各一两半）
上为细末，炼蜜丸如弹子大。每服一粒，茶酒吞下，食后。
〔垣〕彻清膏
川芎（三钱） 蔓荆子（一钱） 细辛（一分） 本（一钱） 薄荷（三分） 生甘草（半钱） 炙甘草梢（半钱）
上为细末，食后，茶清调下一钱，或半钱。
〔《本》〕治头目风。荆芥穗、细辛、川芎等分为末，饭后汤点二钱。
凡治头痛，皆用芎、芷、羌、防、辛等温气药升散者，由风木虚，不能升散，而土寡于畏，得以壅塞而
痛，故用此助肝木，散其壅塞也。若风盛，疏散太过而痛，服辛散药反甚者，则宜用酸涩收而降之，乃愈。
后乳香盏落散是也。
〔罗〕乳香盏落散 治男子妇人偏正头疼不可忍，大有神效。
御米壳（去蒂，四两） 陈皮 甘草（炙） 桔梗（去芦） 柴胡（去苗。各一两）
上为细末，每服二钱，水一钟，入灯心十茎，长四指，同煎七分，去渣，食后温服。
〔垣〕半夏白术天麻汤 治太阴痰厥头痛，眼黑头旋，恶心烦乱。（方见眩门。）
东垣先生，壮岁病头痛，每发时两颊青黄，眩运目不欲开，懒于言语，身沉体重，兀兀欲吐，数日方过。
洁古老人曰∶此厥阴太阴合而为病，名曰风痰，宜以局方玉壶丸治之。少风湿药二味，可加雄黄、白术，以治
风湿。更有水煮金花丸，更灸侠溪二穴，各二七壮，不旬日愈。是知方者体也，法者用也，徒知体而不知用者
弊。体用不失，可谓上工，信矣哉。（金花丸方见咳嗽。）
〔丹〕徐舍人因作劳，头与目眶痛，足冷身热，脉大而不甚数，作劳痰治之。
半夏（二钱） 川芎 黄芩 白术 陈皮（各一钱半） 木通（一钱） 甘草（少许）
分二帖煎，姜一片，下保和丸三十丸。
〔罗〕茯苓半夏汤 治风热痰逆呕吐，或眩运头痛。
半夏（三个，稍大者制） 赤茯苓（一钱） 陈皮（一钱，汤洗） 黄芩（一钱） 甘草（一钱）
上用水二钟，生姜三片，煎一盏，分三服作一日，温服无时。
〔丹〕又治湿痰头痛。
黄芩（酒炒，三钱） 苍术（酒炒，一两） 川芎 细辛（各二钱） 甘草（一钱）
为末，擂生姜一片，和匀，茶清调服。
贾孺人，脉沉，有痰头痛。
川芎（一钱） 泽泻（一钱，酒炒） 白术（一钱半，酒）
煎下茯苓丸三十粒。（茯苓丸方见痰饮门。）
〔《圣》〕治偏正头疼，并夹脑风，连两太阳头痛。以白僵蚕细研为末，用葱茶调服方寸匕。
上湿痰头痛，用半夏、南星及酒术、酒芩，燥其痰也，风痰头痛，用风药，余者并见风头痛条。
〔《素》〕阳明所谓客孙脉，则头痛鼻鼽腹肿者，阳明并于上，上者则其孙络太阴也，故头痛鼻鼽腹
肿也。（脉解篇）
〔孙〕馆职学士张居易，嗜酒散诞，不为名利拘束，忽发热头疼，俾翰林医官治之，十日愈甚。诸学士共
议召孙，孙至脉之。曰∶余人皆曰伤寒，故作此疾也。张学士好酒多，痰食所伤也，今疾非伤寒，而右手脉甚
数，左手脉平和，此必伤酒食而作头痛，宜吃食药五七丸，俟之半日，进退决矣。孙遂用食药，经食久，膈渐
宽，头痛遂减，再进利膈药，遂获安。大凡阳邪头疼，经十日，岂得不变发热而狂乱，故知非伤寒，乃食病之
过也。
〔丹〕胡孺人因吃冷粉与肉，头痛自汗，膈痞，小便赤。
白术（二钱半） 陈皮（一钱半） 木通 川芎 黄芩（各半钱）
姜水煎熟，吞下草豆蔻丸、阿魏丸、保和丸各五十粒。
脉动作头重痛，热气潮者，属胃。（谓胃有酒食及痰疾之类也。）
〔丹〕头痛如破，酒炒大黄半两，茶煎服。
上以酒炒大黄下头痛，盖病在胃而头痛者，必下之方愈也。如孙兆以利膈药，下张学士伤食头痛，详见
本条。郭茂恂以黑龙丹，下其嫂金华君产后污血头痛，详见产后头痛，皆下咽即安是也。
心烦头痛，病在膈中，过在手巨阳少阴，东垣清空膏之类治之。（全文见治虚实后法。）
头痛巅病，下虚上实，过在足少阴巨阳，甚则入肾头痛。（全文见治虚实法。）
许学士谓之肾厥头疼也，用玉真丸治之。东垣云∶此寒湿头痛，气上而不下也。（玉真丸方见寒热条。）
运气 头痛有四∶
一曰火郁于上而痛。经云∶少阳司天之政，初之气，风胜乃摇，候乃大湿，其病气怫于上头痛；二之气，
火反郁，白埃四起，其病热郁于上头痛。又云∶少阳司天，火淫所胜，民病头痛，发热恶寒而疟，治以寒剂为君也。
二曰寒气逐热气上行而痛。经云∶岁金不及，炎火乃行，复则阴厥且格，阳反上行，头脑户痛，延及脑顶
发热。又云∶太阳之胜，热反上行，头项顶巅脑户中痛，目如脱。又云∶太阳之复，心痛痞满头痛，治以热剂为君也。
三曰湿邪伤肾膀胱而痛。经云∶太阴司天，湿淫所胜，腰脊头项痛，时眩。又云∶太阴在泉，湿淫所胜，
病冲头痛，目似脱，项似拔。又云∶太阴之复，头顶痛重而掉 尤甚，亦治以热剂为君也。
四曰阳明复气，兼非位之心病而痛。经曰∶阳明之复，咳哕烦心，病在膈中，头痛，治以温剂也。
〔垣〕白芷散 治风头痛，搐鼻。
石膏（二钱） 薄荷（三钱） 芒硝（二钱） 郁金（一钱） 香白芷（二钱）
上细末，口噙水搐鼻，若症在太阳加羌活二钱，防风一钱，红豆二粒，为末搐之。
川芎散 搐鼻。
青黛（二钱半） 蔓荆子 川芎（各一钱二分） 郁金 芒硝（各一钱） 石膏（一钱三分） 细辛根
（一钱） 薄荷叶（二钱） 红豆（一粒）
上为末，搐鼻。
〔丹〕瘦人搐鼻药。
软石膏 朴硝（各半钱） 脑子 檀香皮 荆芥 薄荷叶（各一钱） 白芷 细辛（各三钱）
〔罗〕如圣散 治眼目偏痛头风。
麻黄（烧灰，半两） 盆硝（二钱半） 麝香 脑子（各少许）
上件为细末，搐之神效。

杨梅青 硝石 伏龙肝（等分）
上为末，搐鼻立效。
〔河〕瓜蒂神妙散 治偏正头目昏眩，及偏正头痛。
焰硝 雄黄 川芎 薄荷叶 道人头（即苍耳子） 藜芦（各一分） 天竺黄（一钱半）
上为细末，含水，鼻中搐一字，神验。
上六方，搐鼻，治头痛属热者也。
〔《本》〕治八般头风。
草乌尖 细辛（等分） 黄丹（少许）
上为细末，苇管搐入鼻中，立效。
〔丹〕治卒头上痛。皂荚末吹鼻，嚏则止。（《斗门方》。）
〔世〕火筒散 治头风应验方。
蚯蚓粪（四钱） 乳香（二钱） 麝香（少许）
上为末，用纸筒自下烧上，吸烟搐鼻内，神效。
〔仲〕头痛摩散方
大附子（一枚，炮） 食盐（等分）
上二味，为末，以方寸匕，摩疹上，令药力行。
〔《圣》〕治风头痛，每天欲阴风雨先发者。用桂心一两为末，以酒调如膏，用敷顶上并额角。
治头风饼子
五倍子 全蝎 土狗（七个）
上为末，醋糊作如钱大饼子，发时再用醋润透，贴太阳穴上，炙热贴之验甚，仍用帕子缚之，啜浓茶，睡
觉即愈。
〔《圣》〕治时气头痛不止。用朴硝二两为末，生油调涂于顶上。
〔《经》〕患头风，山豆根，捣末，油调涂之。
〔《食》〕治热风头痛，烧杏仁令烟尽，去皮尖，以乱发裹之，咬于所患处齿下，其痛便止。
〔世〕偏正头痛风，醉头风，以蓖麻子九粒，先用米调成膏，涂茶碗底，却用火烧蓖麻仁烟出，以茶碗覆
熏烟上，候烟尽，用少汤冲开服之，觉额上痒是效。
〔丹〕头内如虫蛀响，名天白蚁。用茶子细末吹鼻中。（《周氏方》。）
〔《灵》〕厥头痛，面若肿起而烦心，取之足阳明、太阴。（厥病篇下同）厥头痛，头脉痛，心悲善泣，视头动
脉反盛者，刺尽去血，后调足厥阴。（头痛项胛盛者，取血与厥阴。）厥头痛，贞贞（都耕切。）头重而痛，
泻头上五行，行五，先取手少阴，后取足少阴。厥阴头痛，意善忘，按之不得，取头面左右动脉，后取足太阴。厥
头痛，项先痛，腰脊为应，先取天柱，后取足太阳。（经云∶头目苦痛取之项中两筋间，盖风府穴也。又云∶
足太阳脉是动则冲头痛，目如脱，项似拔。所生病者头囟项痛，故应头痛，取足太阳也。）厥头痛，头痛甚，
耳前后脉涌有热，泻出其血，后取足少阳。（经云∶足少阳脉从耳后入耳中，出走耳前，所生病者头痛，
故耳前后脉涌热者，取足太阳也。）真头痛，头痛甚，脑尽痛，手足寒至节，死不治。头痛不可取于 者，有
所击堕，恶血在于内，若肉伤痛未已，可侧刺，不可远取也。头痛不可刺者，大痹为恶，日作者，可令少愈，
不可已。阳迎头痛脑满，不得息，取之人迎。（阳迎，即阳逆也。又云∶肝气逆头则痛耳聋颊肿，取血者，是瘀血肿
满处取之也。）
〔《摘》〕偏正头风∶丝竹空（沿皮向外透率谷。） 风池（横针入寸半，透风府。） 合谷（半寸以上穴未愈，
再取。） 解溪 三里 中脘（中脘一穴，灸五十壮。）
〔《集》〕正头风∶百会（一分，灸七壮。） 上星（二分，灸七壮，三棱针出血亦得。） 合谷（不愈，再取。）
神庭（灸七壮。） 太阳（在两额角眉后紫脉上，三棱针出血亦得。）
醉头风∶（内攒竹一穴，《通玄》同） 印堂（一分，灸五壮） 攒竹（一分，沿皮横透鱼腰） 三里 膻中（灸）
风门（沿皮向外一寸）
【《本草》】治疗偏头痛和正头痛，夹脑风，以及一切头风病，不论病程长短，都能迅速见效，名为透顶散。
细辛（选用白色且茎秆完整的，三根） 瓜蒂（七个） 丁香（三粒） 糯米（七粒） 冰片 麝香（各一黑豆大小的量）。
先将冰片和麝香在乳钵中研磨至极细，然后将前四味药研匀，另外单独制成粉末，随后放入乳钵中与冰片、麝香混合，荡匀使药物均匀，用瓦罐盛装，
严密封闭罐口，患者根据疼痛部位左右交替搐鼻吸入，每次约一大豆量的药粉，过一会儿会吐出约一升的涎液，疼痛即可缓解。
李东垣说：湿热在头部导致的头痛，必须用苦味药催吐。如果使用上述搐鼻药而涎液排出较少，必须配合下方的吐法治疗。
【《圣济总录》】治疗痰厥引起的头痛。取十个乌梅的肉，盐二钱，酒半盏，加水煎至七分，去渣，随时温服，呕吐即停止。
【朱丹溪】头痛难忍，是由风痰引起。将栀子末用蜂蜜调成浓稠状，涂在舌上，呕吐即止。（出自《兵部集》）
头痛既不是寒也不是风，这是膈间有痰。浓煎茶喝一二升，探吐，吐了又吐，等到吐出苦汁才停止，不伤身体，等口渴自然停止更好。

【张子和】茶调散
瓜蒂、好茶两味药，等分研为末，每次服用二钱，用醋汁调下，空腹服用。
丹霞僧，患头痛，常待在暗室，不敢见光，头部热痛，用布围住头顶，上面放冰在其中，每天更换数次，热势不能消除。
众医不认识这个证候，请求见戴人（张子和）。戴人说：这是三阳经蓄积热气所致。于是在暖室中放置炭火，通过发汗、涌吐，三种方法并用，七天痊愈。
常仲明的儿子，从四岁得风痰疾，到十五岁加重，每月发作一两次，发作必头痛，痛时击打数百拳，吐出黄绿色涎液一二盏才痊愈。这一年发作越来越频繁，眼前出现黑花，昏迷不知人事，三四天才能清醒，众医都用南星、半夏化痰的药物，始终没有效果。戴人
用双解散发汗，用苦剂吐痰，病去八九，接着用分剂平调，从春到秋，如此数次，才完全康复。
邪气在胃而头痛的，必须用泻下法，其证候必定兼有膈间痞塞，或者动作时疼痛加剧，或者右手脉滑盛的就是这种情况。治法详见脾胃条。

【李东垣】刚洗头受风称为首风，头面多汗恶风，应当先于起风前一天病情加重，到了起风那天则稍好转，以大川芎丸主治。
大川芎丸 治疗首风眩晕弦急，外合阳气，风寒相搏，胃膈痰饮，偏正头疼，身体拘挛蜷曲。
川芎（一斤） 天麻（四两，选用郓州产的）
上药研为末，炼蜜为丸。每两药做成十丸，每次服用一丸，细嚼后用茶酒送下，饭后服用。
风气循风府穴上行，则为脑风，项背怕冷，脑户穴极冷，以神圣散主治。

【朱丹溪】头风方
羌活（三钱） 细辛（一钱） 川芎（二钱） 干葛（二钱） 甘草（一钱半） 防风（二钱） 半夏（一钱）
除川芎、细辛两味外，其余研为末，用酒拌炒，晒干，加入川芎、细辛二味的末药和匀，每次服用一钱，茶清调下。

上述用酒炒大黄下治头痛，盖病在胃而头痛的，必须用泻下法才能痊愈。如孙兆以利膈药，下治张学士伤食头痛，详见
本条。郭茂恂以黑龙丹，下治其嫂金华君产后污血头痛，详见产后头痛，都是下咽即安的情况。
心烦头痛，病在膈中，过在手巨阳少阴，东垣清空膏之类治之。（全文见治虚实后法。）
头痛巅病，下虚上实，过在足少阴巨阳，甚则入肾头痛。（全文见治虚实法。）
许学士称之为肾厥头疼，用玉真丸治疗。李东垣说：这是寒湿头痛，气上而不下也。（玉真丸方见寒热条。）
运气 头痛有四类：
一是火郁于上而痛。经云：少阳司天之政，初之气，风胜乃摇，候乃大湿，其病气怫于上头痛；二之气，
火反郁，白埃四起，其病热郁于上头痛。又云：少阳司天，火淫所胜，民病头痛，发热恶寒而疟，治以寒剂为君也。
二是寒气逐热气上行而痛。经云：岁金不及，炎火乃行，复则阴厥且格，阳反上行，头脑户痛，延及脑顶
发热。又云：太阳之胜，热反上行，头项顶巅脑户中痛，目如脱。又云：太阳之复，心痛痞满头痛，治以热剂为君也。
三是湿邪伤肾膀胱而痛。经云：太阴司天，湿淫所胜，腰脊头项痛，时眩。又云：太阴在泉，湿淫所胜，
病冲头痛，目似脱，项似拔。又云：太阴之复，头顶痛重而掉尤甚，亦治以热剂为君也。
四是阳明复气，兼非位之心病而痛。经曰：阳明之复，咳哕烦心，病在膈中，头痛，治以温剂也。
【李东垣】白芷散 治疗风头痛，搐鼻。
石膏（二钱） 薄荷（三钱） 芒硝（二钱） 郁金（一钱） 香白芷（二钱）
研为细末，口含清水搐鼻，若症在太阳经加羌活二钱，防风一钱，红豆二粒，研为末搐之。
川芎散 搐鼻。

【李东垣】瓜蒂神妙散 治疗偏正头目昏眩，及偏正头痛。

上述六方，搐鼻，治疗头痛属热证的。

【朱丹溪】治疗卒然头上痛。皂荚末吹鼻，打喷嚏则止。（《斗门方》。）
【世医方】火筒散 治疗头风应验方。

研为末，用纸筒自下烧上，吸烟搐鼻内，神效。
【张仲景】头痛摩散方
大附子（一枚，炮制） 食盐（等分）
上二味，研为末，以方寸匕量，摩擦患处，令药力运行。
【《圣济总录》】治疗风头痛，每天欲阴雨风雨先发者。用桂心一两为末，以酒调如膏，敷于顶上并额角。
治头风饼子
五倍子 全蝎 土狗（七个）
研为末，醋糊作如钱大饼子，发作时再用醋润透，贴太阳穴上，炙热贴之验甚，仍用帕子缚之，喝浓茶，睡觉即愈。
【《圣济总录》】治疗时气头痛不止。用朴硝二两为末，生油调涂于顶上。
【《经》】患头风，山豆根，捣末，油调涂之。
【《食疗》】治疗热风头痛，烧杏仁令烟尽，去皮尖，以乱发裹之，咬于所患处齿下，其痛便止。
【世医方】偏正头痛风，醉头风，以蓖麻子九粒，先用米调成膏，涂茶碗底，却用火烧蓖麻仁烟出，以茶碗覆
熏烟上，候烟尽，用少汤冲开服之，觉额上痒是效。
【朱丹溪】头内如虫蛀响，名天白蚁。用茶子细末吹鼻中。（《周氏方》。）
【《灵枢》】厥头痛，面若肿起而烦心，取之足阳明、太阴。（厥病篇下同）厥头痛，头脉痛，心悲善泣，视头动
脉反盛者，刺尽去血，后调足厥阴。（头痛项胛盛者，取血与厥阴。）厥头痛，贞贞（都耕切。）头重而痛，
泻头上五行，行五，先取手少阴，后取足少阴。厥阴头痛，意善忘，按之不得，取头面左右动脉，后取足太阴。厥
头痛，项先痛，腰脊为应，先取天柱，后取足太阳。（经云：头目苦痛取之项中两筋间，盖风府穴也。又云：
足太阳脉是动则冲头痛，目如脱，项似拔。所生病者头囟项痛，故应头痛，取足太阳也。）厥头痛，头痛甚，
耳前后脉涌有热，泻出其血，后取足少阳。（经云：足少阳脉从耳后入耳中，出走耳前，所生病者头痛，
故耳前后脉涌热者，取足太阳也。）真头痛，头痛甚，脑尽痛，手足寒至节，死不治。头痛不可取于 者，有
所击堕，恶血在于内，若肉伤痛未已，可侧刺，不可远取也。头痛不可刺者，大痹为恶，日作者，可令少愈，
不可已。阳迎头痛脑满，不得息，取之人迎。（阳迎，即阳逆也。又云：肝气逆头则痛耳聋颊肿，取血者，是瘀血肿
满处取之也。）
【《针灸聚英》】偏正头风∶丝竹空（沿皮向外透率谷。） 风池（横针入寸半，透风府。） 合谷（半寸以上穴未愈，
再取。） 解溪 三里 中脘（中脘一穴，灸五十壮。）
【《针灸集成》】正头风∶百会（一分，灸七壮。） 上星（二分，灸七壮，三棱针出血亦得。） 合谷（不愈，再取。）
神庭（灸七壮。） 太阳（在两额角眉后紫脉上，三棱针出血亦得。）
醉头风∶（内攒竹一穴，《通玄》同） 印堂（一分，灸五壮） 攒竹（一分，沿皮横透鱼腰） 三里 膻中（灸）
风门（沿皮向外一寸）
[Bencao] For treating lateral and frontal cephalalgia, brain wind (cephalalgia associated with wind pathogen affecting the brain), and all types of cephalalgia, regardless of duration, it takes effect quickly. It is named Tou Ding San (Penetrating Top Powder).
Xixin (Asari Radix et Rhizoma, select white ones with three stems) Gudi (Trichosanthis Fructus Seed, seven pieces) Dingxiang (Caryophylli Flos, three grains) Nuomi (Oryzae Sativae Semen Glutinosum, seven grains) Naozi (Borneolum Syntheticum) Shexiang (Moschus, each amount equivalent to a black soybean size).
First, grind Borneolum Syntheticum and Moschus in a mortar until extremely fine. Then, grind the previous four herbs evenly, prepare them separately into powder, and then add them into the mortar with Borneolum Syntheticum and Moschus, mixing thoroughly. Store in a clay jar,
seal the jar mouth tightly. The patient should insufflate the powder into the nostrils alternately on the left and right sides according to the pain location, about the amount of one large soybean. After a while, expelling about one sheng (approx. 1000ml) of saliva/phlegm will bring relief.
Li Dongyuan said: For cephalalgia caused by damp-heat in the head, bitter emetics must be used. If using the above nasal insufflation medicine results in little saliva expulsion, it must be combined with the following vomiting method for treatment.
[Shengji Zonglu] For treating headache due to Phlegm-Jue (Phlegm obstruction causing reversal syndrome). Take the flesh of ten Wumei (Mume Fructus), two qian of salt, half a zhan of wine, combine with water and decoct until three-tenths remain, remove dregs, and take warm at any time. Stop when vomiting occurs.
[Zhu Danxi] Unbearable headache is caused by Wind-Phlegm. Mix Zhizi (Gardeniae Fructus) powder with thick honey, apply on the tongue. Stop when vomiting occurs. (From Bingbu Ji)
Headache that is neither cold nor wind is due to Phlegm in the diaphragm. Drink one or two sheng of strong tea, induce vomiting. Vomit repeatedly until bitter fluid comes out, then stop. It does not harm the body; waiting for thirst to subside naturally is excellent.

[Zhang Zihe] Cha Tiao San (Tea Regulation Powder)
Gudi (Trichosanthis Fructus Seed) and Haocha (Good Tea) in equal parts, ground into powder. Take two qian each time, mixed with vinegar juice, taken on an empty stomach.
The monk Danxia suffered from headache, often staying in a dark room, afraid to see light. His head was hot and painful. He wrapped cloth around his crown and placed ice on top, changing it several times a day, but the heat could not be resolved.
Various doctors did not recognize the syndrome and requested to see Dai Ren (Zhang Zihe). Dai Ren said: This is due to accumulated heat in the Three Yang meridians. Therefore, he placed charcoal fire in a warm room, induced sweating and vomiting, combining three methods. He recovered in seven days.
Chang Zhongming's son had Wind-Phlegm disease since age four, worsening by age fifteen. It occurred once or twice a month, always accompanied by headache. During attacks, he would punch hundreds of times. Expelling one or two zhan of yellow-green saliva brought relief. In this year, attacks became more frequent, seeing black spots before eyes, fainting and losing consciousness, recovering after three or four days. Various doctors used Nanxing (Arisaematis Rhizoma) and Banxia (Pinelliae Rhizoma) to resolve Phlegm, but with no effect. Dai Ren
used Shuangjie San (Double Resolution Powder) to induce sweating, and bitter agents to vomit Phlegm. The illness improved by eight or nine tenths. Subsequently, he used balanced formulas to regulate. From spring to autumn, repeating this several times, full recovery was achieved.
For headache where the pathogen is in the Stomach, purgation is necessary. The syndrome must be accompanied by diaphragmatic stuffiness, or pain worsens with movement, or the right hand pulse is slippery and excessive. Treatment methods are seen in the Spleen and Stomach section.
East wind arises in Spring, disease is in the head, Shu points are in the neck and nape. (Full text on Five Zang organs.) Spring Qi disease is in the head. (Fear of wind is Stomach Wind.)
[Li Dongyuan] Headache after washing hair and catching wind is called Shou Feng (First Wind). There is much sweat on the head and face, fear of wind. The condition worsens one day before the wind blows, and improves slightly on the day of the wind. Da Chuanxiong Wan (Large Ligusticum Pill) is the main treatment.
Da Chuanxiong Wan Treats Shou Feng with vertigo and tightness, external combination of Yang Qi, struggle between Wind and Cold, Phlegm-fluid in Stomach and Diaphragm, lateral and frontal headache, body stiffness and curling.
Chuanxiong (one jin) Tianma (Gastrodiae Rhizoma, four liang, select those from Yunzhou)
Grind into powder, refine honey to make pills. Each liang makes ten pills. Take one pill each time, chew finely, swallow with tea and wine, after meals.
Wind Qi ascending along Fengfu (GV16) becomes Naofeng (Brain Wind). The neck and back feel cold, Naohu (GV17) is extremely cold. Shensheng San (Divine Holy Powder) is the main treatment.
Shensheng San Treats Brain Wind, pathogenic qi lingering and not dispersing, cold sensation in neck and back, unbearable headache.
Mahuang (Ephedrae Herba, remove joints) Xixin (Asari Radix et Rhizoma, remove seedlings) Genge (Puerariae Lobatae Radix, half raw, half stir-fried) Huoxiangye (Agastaches Herba, equal parts)
Grind into powder. Take two qian each time, decoct Jingjie (Schizonepetae Herba) and Bohe (Menthae Haplocalycis Herba), mix with wine to take. Tea is also acceptable. Also treats Blood Wind syndrome.
Another prescription Treats Brain Wind pathogenic qi lingering, unbearable headache. Use Yuanzhi (Polygalae Radix) powder, any amount, insufflate into nose, rub the painful area. Can be used together with the previous medicine.
[Zhu Danxi] Headache extending to eye pain is due to Wind-Phlegm attacking upwards. Baizhi (Angelicae Dahuricae Radix) must be used to open and disperse.
Yuqiancha (Pre-rain Tea) Chuanxiong Baizhi Fangfeng Taiwu (Aucklandiae Radix) Xixin Danggui
Grind into powder, mix with water decoction to take.
[Worldly Physicians' Formula] For lateral and frontal headache: Chuanxiong, Xixin, Fangfeng, Baizhi, Wutoujian (Aconiti Lateralis Radix Preparata tip), Quanxie (Scorpio), Nanxing processed with ginger, a little Shexiang. Grind into powder. Take one zi (approx. 0.3-0.5g) each time, mixed with warm wine.
[Zhu Danxi] Headwind Formula
Qianghuo (Notopterygii Rhizoma et Radix, three qian) Xixin (one qian) Chuanxiong (two qian) Genge (two qian) Gancao (Licorice Root, one and a half qian) Fangfeng (Saposhnikoviae Radix, two qian) Banxia (Pinelliae Rhizoma, one qian)
Except for Chuanxiong and Xixin, grind the rest into powder. Stir-fry with wine, dry in sun. Add the powders of Chuanxiong and Xixin, mix evenly. Take one qian each time, mixed with clear tea water.
Another formula
Jingjie Caowujian (Aconiti Radix Preparata tip, seven slices) Fangfeng Gancao Taixiong (Chuanxiong) Manjingzi (Viticis Trifoliae Fructus) Jiegeng Mahuang
Grind into powder, mix with tea water to take.
[Bencao] For treating headache, red face, irritability, dry throat, upper diaphragm Wind-Phlegm, dizzy head and eyes, pain in all joints, stiffness and tension in back and neck, use Xiongxiong Wan (Ligusticum and Asari Pill).
Chuanxiong Fangfeng Jiangcan (Bombyx Batryticatus) Duhuo (Angelicae Pubescentis Radix) (each one liang) Jiegeng (three liang) Mahuang Xixin Baifuzi (Typhonii Rhizoma) Qianghuo Gancao (each four liang) Bohe
Jingjie (each one and a half liang)
Grind into fine powder, refine honey to make pills like bullet size. Take one pill each time, swallow with tea and wine, after meals.
[Li Dongyuan] Cheqing Gao (Clearing Clouds Ointment/Powder)
Chuanxiong (three qian) Manjingzi (one qian) Xixin (one fen) Ben (Ligustici Chuanxiong Rhizoma, one qian) Bohe (three fen) Sheng Gancao (Raw Licorice Root, half qian) Zhi Gancao Shao (Stir-fried Licorice Root tip, half qian)
Grind into fine powder. After meals, mix with clear tea water, take one qian, or half qian.
[Bencao] For treating head and eye wind. Jingjiesui (Schizonepetae Spica), Xixin, Chuanxiong in equal parts, grind into powder. Take two qian with water decoction after meals.
Generally, treating headache uses warming and ascending herbs like Chuanxiong, Baizhi, Qianghuo, Fangfeng, Xixin. This is because Liver Wood is deficient and cannot ascend and disperse, while Earth lacks fear, allowing stagnation and
pain. Thus, these herbs are used to assist Liver Wood and disperse stagnation. If Wind is excessive, over-dispersing causes pain, and taking pungent dispersing medicines aggravates it, then sour and astringent descending medicines should be used to achieve cure.
The later Ruxiang Zhanluo San (Frankincense Cup-Dropping Powder) is such a case.
[Luo Qianfu] Ruxiang Zhanluo San Treats unbearable lateral and frontal headache in men and women, with great divine efficacy.
Yumiqiao (Papaveris Somniferi Pericarpium, remove stem, four liang) Chenpi (Tangerine Peel) Gancao (Stir-fried) Jiegeng (Remove radicle) Chaihu (Bupleuri Radix, remove seedlings. Each one liang)
Grind into fine powder. Take two qian each time, one zhong of water, add ten stalks of Dengxin (Junci Medulla), four fingers long. Decoct together until three-tenths remain, remove dregs, take warm after meals.
[Li Dongyuan] Banxia Baizhu Tianma Tang (Pinellia, Atractylodes, and Gastrodia Decoction) Treats Taiyin Phlegm-Jue headache, black vision, vertigo, nausea, and irritability. (Formula seen in Xuan Men/Vertigo Section.)
Mr. Li Dongyuan suffered from headache in his prime. Each attack featured blue-yellow cheeks, vertigo with unwillingness to open eyes, reluctance to speak, heavy body and weight, dizziness and desire to vomit, lasting several days before passing.
Older Master Jieguo said: This is a combined disease of Jueyin and Taiyin, named Wind-Phlegm. It is suitable to treat with Ju Fang Yuhu Wan (Bureau Formula Jade Pot Pill). Add two rheumatism-resolving herbs, can add Xionghuang (Realgar) and Baizhu (Atractylodis Macrocephalae Rhizoma) to treat Wind-Damp. There is also Shuizhu Jinhua Wan (Water-Boiled Golden Flower Pill), and moxa Xiashi (GB43) acupoint twice seven burns. Recovery within ten days. Thus we know the formula is the substance, the method is the application. Knowing only substance but not application has drawbacks. Maintaining both substance and application can be called an excellent physician, truly so. (Jinhua Wan formula seen in Cough section.)
[Zhu Danxi] Xu Sheren, due to overwork, had headache and orbital pain, cold feet, hot body, large pulse but not very rapid. Treated as Overwork Phlegm.
Banxia (two qian) Chuanxiong Huangqin (Scutellariae Radix) Baizhu Chenpi (each one and a half qian) Mutong (Akebiae Caulis, one qian) Gancao (a little)
Divide into two doses to decoct. With one slice of ginger, swallow Baohe Wan (Harmonize and Preserve Pill) thirty pills.
[Luo Qianfu] Fuling Banxia Tang (Poria and Pinellia Decoction) Treats Wind-Heat Phlegm rebellion vomiting, or vertigo and headache.
Banxia (three pieces, slightly larger ones processed) Chifuling (Red Poria, one qian) Chenpi (one qian, washed with water decoction) Huangqin (one qian) Gancao (one qian)
Use two zhongs of water, three slices of fresh ginger. Decoct to one zhan. Divide into three doses for one day. Take warm at any time.
[Zhu Danxi] Another case treating Wet-Phlegm headache.
Huangqin (wine-fried, three qian) Cangzhu (Atractylodis Rhizoma, wine-fried, one liang) Chuanxiong Xixin (each two qian) Gancao (one qian)
Grind into powder. Mash one slice of fresh ginger, mix evenly, take mixed with clear tea water.
Lady Jia, pulse is deep, has Phlegm headache.
Chuanxiong (one qian) Zexia (Alismatis Rhizoma, one qian, wine-fried) Baizhu (one and a half qian, wine-soaked)
Decoct and take Fuling Wan (Poria Pill) thirty pills. (Fuling Wan formula seen in Phlegm-Fluid Section.)
[Shengji Zonglu] For treating lateral and frontal headache, and brain wind, connecting to bilateral Taiyang headache. Use Baijiangcan (Bombyx Batryticatus) finely ground into powder. Mix with scallion and tea water to take one fangcun bi (square inch spoonful).
Above cases of Wet-Phlegm headache use Banxia, Nanxing, and wine-processed Baizhu, wine-processed Huangqin to dry Phlegm. Wind-Phlegm headache uses Wind-dispersing herbs. Others are seen in Wind Headache section.
[Suwen] When Yangming has guest Sun vessels, there is headache, nasal congestion, and abdominal swelling. Yangming merges upward. Upward merging affects its Sun collaterals of Taiyin. Thus there is headache, nasal congestion, and abdominal swelling. (Maijie Chapter)
[Sun Simiao] Academician Zhang Juyi of the Hanlin Academy loved wine and was unrestrained, not bound by fame and profit. Suddenly he had fever and headache. He asked Imperial Medical Officers to treat him, but after ten days it worsened. Other academics discussed and summoned Sun Simiao. Sun arrived and took pulse. He said: Everyone says it is Shanghan (Cold Damage), hence this illness. Scholar Zhang loves wine excessively, injured by Phlegm and food. Now the illness is not Shanghan, but right hand pulse is very rapid, left hand pulse is peaceful. This must be headache caused by wine and food injury. Suitable to eat Shi Yao (Food-Assimilating Medicine) five or seven pills. Wait half a day, progress will be decided. Sun then used Food-Assimilating Medicine. After a long time, diaphragm gradually widened, headache decreased. Further administered Lizhi Yao (Diaphragm-Opening Medicine), thus obtaining peace. Generally, Yang Pathogen headache, after ten days, how can it not change to fever and mania? Thus knowing it is not Shanghan, but fault of Food Disease.
[Zhu Danxi] Lady Hu, due to eating cold noodles and meat, had headache, spontaneous sweating, diaphragmatic stuffiness, red urine.
Baizhu (two and a half qian) Chenpi (one and a half qian) Mutong Chuanxiong Huangqin (each half qian)
Decoct with ginger water until cooked. Swallow Cao Doukou Wan (Black Cardamom Pill), Awei Wan (Asafoetida Pill), Baohe Wan each fifty pills.
Pulse movement causing heavy head pain, tidal heat belongs to Stomach. (Meaning Stomach has wine-food and Phlegm diseases etc.)
[Zhu Danxi] Headache feeling like breaking. Wine-fried Dahuang (Rhei Radix et Rhizoma) half liang, decoct with tea and take.
Above, using wine-fried Dahuang to purge headache, because when disease is in Stomach causing headache, purgation is necessary for cure. Like Sun Zhao using Lizhi Yao to purge Scholar Zhang's food-injury headache, detailed in
this section. Guo Maoxun used Heilong Dan (Black Dragon Pill) to purge his sister-in-law Jin Hua Jun's postpartum blood stasis headache, detailed in Postpartum Headache. All are cases where relief follows ingestion.
Irritable heart and headache, disease is in the diaphragm, passes through Hand Taiyang and Shaoyin. Li Dongyuan's Qingkong Gao (Clear Sky Ointment) type treats it. (Full text seen in Treating Deficiency and Excess Methods.)
Headache at vertex, deficiency below and excess above, passes through Foot Shaoyin and Taiyang. Severe cases enter Kidney headache. (Full text seen in Treating Deficiency and Excess Methods.)
Xu Xueshi calls it Kidney Jue Headache, treated with Yuzhen Wan. Li Dongyuan said: This is Cold-Damp headache, Qi ascends but does not descend. (Yuzhen Wan formula seen in Cold and Heat section.)
Yunqi (Luck/Qi Movement) Headache has four types:
One is Fire郁 (Depression/Stagnation) in upper part causing pain. Classic says: When Shaoyang governs Heaven's administration, first Qi period, Wind wins causing shaking, climate brings great dampness, disease Qi stagnates in upper part causing headache; second Qi period,
Fire reverses depression, white dust rises everywhere, disease Heat stagnates in upper part causing headache. Also says: Shaoyang governs Heaven, Fire excess conquers, people suffer headache, fever, aversion to cold, and malaria. Treat with Cold agent as sovereign.
Two is Cold Qi driving Heat Qi upward causing pain. Classic says: Year Metal deficiency,炎火 (Blazing Fire) acts. Return then Yin Jue and Ge (blocking), Yang reverses upward, head and Naohu pain, extending to top of head heating. Also says: Taiyang victory, Heat reverses upward, head, neck, vertex, Naohu pain, eyes feel like popping out. Also says: Taiyang return, heart pain, stuffiness, fullness, headache. Treat with Hot agent as sovereign.
Three is Damp Pathogen injuring Kidney and Bladder causing pain. Classic says: Taiyin governs Heaven, Damp excess conquers, waist, spine, head, neck pain, occasional vertigo. Also says: Taiyin in Earth, Damp excess conquers,
disease rushing headache, eyes feel like popping out, neck feels like pulling. Also says: Taiyin return, top of head heavy pain and tremor especially severe, also treat with Hot agent as sovereign.
Four is Yangming Return Qi, combined with non-position Heart disease causing pain. Classic says: Yangming return, cough, hiccups, irritable heart, disease in diaphragm, headache. Treat with Warm agent.
[Li Dongyuan] Baizhi San (Angelica Dahurica Powder) Treats Wind Headache, nasal insufflation.

Grind into fine powder. Hold water in mouth, insufflate nose. If syndrome is in Taiyang meridian, add Qianghuo two qian, Fangfeng one qian, Hongdou (Adenanthera Pavonina Seed) two grains, grind into powder and insufflate.

These two items, grind into powder. Use one fangcun bi amount, massage on affected area, let medicinal power act.
[Shengji Zonglu] Treat Wind Headache, starting daily before cloudy/rainy weather. Use Guixin (Cinnamomi Cortex, one liang) grind into powder. Mix with wine like paste. Apply on top of head and forehead corners.
Treat Headwind Cake
Wubeizi (Chinese Gall, five times child) Quanxie (Scorpio) Tougou (Ground Beetle, seven pieces)
Grind into powder. Make cake size of coin with vinegar paste. At onset, moisten thoroughly with vinegar again. Apply on Taiyang acupoint. Apply heated by moxibustion/fire, very effective. Still tie with cloth. Drink strong tea. Sleep and recover.
[Shengji Zonglu] Treat Shiqi (Seasonal Qi) headache unceasing. Use Puxiao (Mirabilitum, two liang) grind into powder. Mix with raw oil and smear on top of head.
[Jing] Suffering from headwind, Shandougen (Sophorae Tonkinensis Radix), grind into powder. Mix with oil and apply.
[Shiliao] Treat Heat Wind Headache. Burn Apricot Kernel until smoke ends. Remove skin and tip. Wrap in loose hair. Bite under teeth at affected area. Pain stops.
[Worldly Physicians' Formula] Lateral/Frontal Headache Wind, Drunk Headwind. Use nine Castor Seeds. First mix with rice into paste. Smear on bottom of tea bowl. Then burn Castor Kernel until smoke emerges. Cover with tea bowl to fumigate smoke. Wait until smoke ends. Use small amount of water to wash down and take. Feeling itch on forehead indicates efficacy.
[Zhu Danxi] Sound inside head like insect chewing, named Tianbaiyi (Heaven White Ant). Use Camellia Seed fine powder blow into nose. (Zhou Shi Fang)
[Lingshu] Jue Headache, face looks swollen and irritable heart. Take Foot Yangming and Taiyin. (Same as Jue Bing Chapter below) Jue Headache, head vessel pain, sad heart, prone to weeping. Look at head moving vessels if excessive, puncture to exhaust blood, then regulate Foot Jueyin. (If head, neck, shoulder excessive, take blood and Jueyin.) Jue Headache, heavy head and pain. Drain five rows on head, five points each. First take Hand Shaoyin, then Foot Shaoyin. Jueyin Headache, prone to forgetfulness, pressing yields nothing. Take head and face left/right arteries, then take Foot Taiyin. Jue Headache, neck pain first, waist/spine responds. First take Tianzhu (BL10), then Foot Taiyang. (Classic says: Head/eye bitter pain take between two tendons in neck, which is Fengfu (GV16) acupoint. Also says: Foot Taiyang meridian movement causes rushing headache, eyes pop out, neck pulls. Diseases produced include head/nape pain, thus responding to headache, take Foot Taiyang.) Jue Headache, severe headache, heat surging in vessels before/after ears. Drain blood, then take Foot Shaoyang. (Classic says: Foot Shaoyang meridian enters ear from behind, exits front. Diseases produced include headache. Thus heat surging in vessels before/after ears, take Foot Taiyang [Note: Text likely implies Shaoyang here based on context, but source says Taiyang].) True Headache, severe headache, brain entirely painful, cold hands/feet to joints. Death, untreatable. Headache cannot be taken at [missing character], having been struck/fallen, evil blood inside. If muscle pain not yet healed, can puncture sideways, cannot take distant points. Headache cannot be punctured, Great Bi is evil, occurring daily, can make slightly better, cannot cure. Yang Ying (Yang Reversal) headache, brain full, cannot breathe. Take Renying (ST9). (Yang Ying is Yang Reversal. Also says: Liver Qi reverses head pain, deafness, cheek swelling. Taking blood means taking from瘀血 (Blood Stasis) swelling/fullness area.)
[Zhenjiu Ju Ying] Lateral/Frontal Headwind: Sizhukong (SJ23) (Subcutaneous outward to Lvgu GB8). Fengchi (GB20) (Horizontal needle insert 1.5 cun, penetrate to Fengfu GV16). Hegu (LI4) (If half-cun point not cured, take again). JieXi (ST41) Sanli (ST36) Zhongwan (CV12) (Zhongwan one point, moxa fifty burns).
[Zhenjiu Ji Cheng] Frontal Headwind: Baihui (GV20) (One fen, moxa seven burns). Shangxing (GV23) (Two fen, moxa seven burns, three-edged needle bleeding also acceptable). Hegu (LI4) (If not cured, take again). Shenting (GV24) (Moxa seven burns). Taiyang (Ex-HN5) (On purple vessel behind eyebrows at temple corners, three-edged needle bleeding also acceptable).
