[Qian Yi] Convulsions caused by pox and rashes result from the mutual overcoming of wind and fire; only rashes can induce convulsions. Rashes are generated by the Spleen. When the Spleen is deficient, the exuberant Liver Qi overacts upon it (Wood overacting on Earth). The heat qi strikes each other, disturbing the Heart Spirit (Shen). Since the Heart governs joy, heat causes the spirit to become unsettled, leading to convulsions that evolve into epilepsy (Xian syndrome). [Note: The original text "Zi is born of the Heart" contains a theoretical inconsistency in standard Five Elements theory as Kidney/Water is born by Metal/Lung, not Fire/Heart directly in the generating cycle, or refers to a specific lineage. Translated literally as per instruction.] Zi is born of the Heart; when the Heart generates heat, heat generates wind. Wind belongs to the Liver. The two organs (Heart and Liver) contend with each other, wind and fire struggle, thus convulsions occur. Treatment should involve draining the Heart and Liver and tonifying their mothers (the sources), using Gualou Tang (Trichosanthes Decoction) as the primary formula. (Zhang Yuansu stated: All pain, itching, and sores belong to the Heart. Regardless of whether the fire pathogen is excess or deficiency, it should be discussed from the perspective of Heart Fire. If the Spleen is deficient, Liver Qi overacts; if Liver and Heart Fire combine, Gualou Tang is used. If the Spleen Earth is excess, and hyperactive Fire counter-overacts causing epilepsy, this is an excess pathogen. For constipation, use Xieqing Wan (Drain the Green Pill); for loose stools, use Xieqing Tang (Drain the Green Decoction). Pulse differentiation is also necessary.)
Gualou Tang (Formula see under Convulsive Disease of Infants section)
[Li Gao] In cases where convulsions occur before the eruption of pox/rashes, it is caused by external contraction of Wind-Cold pathogens and internal emergence of Heart Heat. One should administer Jiedu Wan (Detoxification Pill) with Tea Powder, or treat with Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction).
[Worldly Physicians] When treating pox/rashes about to erupt, if the body is hot and restless with sudden惊厥 (convulsions/fright), use Qufeng Gao (Wind-Expelling Ointment) or Xiao Rusheng Yin (Small Like-Sage Drink). If urination is obstructed, use Bazheng San (Eight-Positive Powder). If there is excessive phlegm-saliva, use Lijing Wan (Phlegm-Expelling Pill) or Baolong Wan (Embracing Dragon Pill), administered according to symptoms.
[Zhu Zhenheng] Before the eruption of pox/rashes, if there is initial body heat, fright, jumping, and convulsions, this is not Fright-Wind (Jing Feng), but should be treated with diaphoretic (sweat-inducing) herbs.
[Worldly Physicians] Formula for Wind-Induced Convulsions in Smallpox/Measles
Renshen (Ginseng Radix et Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), Jiangcan (Bombyx Batryticatus), Nanting (Arisaematis Tuber processed with Ginger), Baifu (Typhonii Rhizoma processed with Ginger), Gancao (Licorice Root processed with honey).
Equal parts of the above, add three slices of Shengjiang (Fresh Ginger), decoct in water and take orally. The convulsions will stop immediately.
[Qian Yi] After the rash disease, if epilepsy is prone to develop, it is difficult for residual sores to cause epilepsy because Wood overacts on Spleen, and Wood returns to Heart. If it is Cold-Fright, use Liangjing Wan (Cooling Fright Pill); if it is Warm-Fright, use Fenhong Wan (Pink Pill, i.e., Wenjing Wan/Warming Fright Pill).
[Huoyou Xinshu] To treat pain from smallpox sores, use Wenjing Wan (Warming Fright Pill), dissolved in water and taken orally. (Formula see previous Convulsions section).
[Zhang Yuansu] To treat restlessness and pain after sores erupt, use Wuwu Muxiang San (Five-Ingredient Costus Powder).
Qingmuxiang (Costi Radix, 2 liang), Dingxiang (Caryophylli Flos, 1 liang), Linglingxiang (Lingzhi Xiang/Agarwood? Note: Linglingxiang usually refers to Agastache or similar aromatic, here likely distinct local herb, translated as Linglingxiang), Shexiang (Moschus, 1 fen), Baijian (Alumen, 1 liang).
Take 4 qian per dose, decoct in 1.5 small cups of water. For severe heat, add Xijiao (Rhinoceros Horn, 1 liang). If Xijiao is unavailable, substitute with Shengma (Cimicifugae Rhizoma). For mild cases, one dose yields great effect.
Another method: Mix Mangxiao (Mirabilite) with Pig Gallbladder bile, apply to sores to loosen them. When scabs fall off without scars, apply Yellow Earth (Huangtu) for good results. If this condition presents with astringent urination and blood, the middle is damaged (critical prognosis). If the back of the sores is black and sunken without pus, death is inevitable and untreatable.
[Worldly Physicians] When smallpox is about to form scabs, if they are dry, hard, and painful, apply Su (Butter/Ghee) to moisten them. If scabs can be removed, remove them. If Su is unavailable, use Zhu You (Pig Fat) boiled in water as a substitute. This type of pain cannot be avoided merely by taking medicine.
