[Verse] Scabies at the hairline occurs at the edge of the hairline, resembling millet grains or beans; it is itchy, painful, and firm. The top is white while the surrounding flesh is red; it is easy to treat in the early stage. However, in obese individuals with thick skin at the nape, the condition is most lingering and difficult to cure.
[Note] This condition arises at the hairline behind the neck, resembling millet grains or beans. The skin of the neck is white, while the surrounding flesh is red and hard. The pain feels like a needle prick, and the itching feels like burning fire. After rupture, pus flows out; sometimes it spreads into the hair. This is caused by internal damp-heat stagnation combined with external wind invasion, where internal and external pathogens interact. Initially, Gan Zhu Dan (Wan Ling Dan) should be used to induce sweating. Subsequently, Fangfeng Tongsheng San processed with wine should be used to clear heat and resolve the exterior. Applying Huanglian Gao externally yields good results. In obese individuals, the flesh at the hairline behind the neck is thick and wrinkled, causing hairs to pierce into the sores. If neglected for a long time without healing, and combined with cold-dampness coagulation, the shape resembles a lying melon. It breaks, weeps fluid, alternates between breaking and healing, and is commonly known as "Rou Gui" (Flesh Turtle). Although it may persist for years without serious harm, applying Hupo Gao can provide slight relief.
Formula: Hupo Gao (Amber Ointment)
Composition: Ding Fen (1 liang), Xueyu (8 qian), Qingfen (4 qian), Yinzhu (7 qian), Huajiao (14 grains), Huangla (4 liang), Hupo (powdered, 5 fen), Mayou (12 liang).
Method: Deep-fry Xueyu, Huajiao, and Mayou until charred, remove residues, add Huangla and melt completely. Filter through linen cloth, pour into a ceramic basin. Pre-grind Ding Fen, Yinzhu, Qingfen, and Hupo into fine powder, mix them together, and slowly add to the oil while stirring occasionally with a willow branch until cooled. Apply using cotton胭脂 (Yanzhi) or red cotton paper.
[Verse] Hupo Gao treats various sores, effectively activating blood, resolving stasis, detoxifying, and transforming rot. It contains Ding Fen, Xueyu, Qingfen, Yinzhu, Huajiao, Huangla, and Hupo, boiled in Mayou to make an ointment that also heals ulcers.
Alternative Formula: Gan Zhu Dan (i.e., Wan Ling Dan, see Swelling/Ulcer Section); Fangfeng Tongsheng San (see Alopecia Areata section); Huanglian Gao (see Nasal Ulcer section).
[Verse] Headwind (Tou Feng) causes pain in the eyes and brow bone. Wind, Fire, Cold, and Phlegm are the four causes. Some cases result from Yangmei Du (Syphilis toxin) attacking the crown, while others occur due to wind invasion postpartum.
[Note] This condition presents with aversion to cold and wind. Pain that wanders and decreases with warmth indicates Wind Pain. Pain accompanied by alternating chills and fever, bitter taste, extreme thirst, constipation/urinary difficulty, and insomnia indicates Fire Pain. Pain with cold extremities, blueish complexion, pale lips, rebellious Qi but no thirst, and clear urine indicates Cold Pain. Pain with heavy body, sore limbs, chest irritability, vomiting, and spitting of phlegm foam indicates Phlegm Pain. For these four conditions, the ancient formula Qianghuo Chonghe Tang, with doubled Chuanxiong and added Juhua, should be used. Based on the meridian and specific symptoms, add guiding herbs for treatment. If neglected, wind attacks the brow bone causing sour ache and eyelid twitching, gradually affecting the eyeball, leading to "blue clouds obscuring the pupil," often resulting in vision loss. If only brow bone sour ache exists, Bi Yun San inhaled nasally is very effective.
Formula: Qianghuo Chonghe Tang
Composition: Fangfeng, Baizhi (each 1 qian); Xixin, Gancao (raw, each 5 fen); Shengdi, Cangzhu, Huangqin (each 1 qian); Qianghuo (1.5 qian); Chuanxiong (2 qian).
Guiding Herbs: Add 3 scallion heads, 1 slice of fresh ginger, and 2 dates (pitted). Decoct in water and take after meals.
If pain starts from the back of the neck (Bladder Meridian), double Qianghuo and add Gaoben.
If pain starts from behind the ear (Gallbladder Meridian), add Chaihu.
If pain radiates from Taiyang to the forehead and eyes (Stomach Meridian), double Baizhi and add Gegen and Duan Shigao.
If headache is accompanied by abdominal pain and heaviness (Spleen Meridian), double Cangzhu.
If headache is accompanied by cold feet and rebellious Qi (Kidney Meridian), double Xixin; if severe, add Mahuang and Sheng Fuzi, reduce Huangqin.
If headache is accompanied by vomiting saliva and cold extremities (Liver Meridian), add Wuzhuyu.
If headache involves fire, heat, and thirst, double Jiuhuangqin and add raw Shigao. If constipated, add raw Dahuang.
If headache involves spitting phlegm but limbs are not cold, add Banxia.
[Verse] Chonghe Tang treats headwind damaging the eyes, arising from wind, fire, cold, and phlegm. Long-standing brow ache and twitching, obscuring the pupil and damaging vision, can be cleared by this formula. Ingredients include Fangfeng, Baizhi, Xixin, Gancao, Shengdi, Cangzhu, Huangqin, Qianghuo, Chuanxiong. Carefully add meridian-guiding herbs, decoct with scallion, ginger, and dates in water.
Alternative Formula: Bi Yun San
Composition: Chuanxiong, Ebusicao (each 1 liang); Xixin, Xinyi (each 2 qian); Qingdai (1 qian).
Method: Grind into fine powder. The patient holds cool water in their mouth. An assistant blows the powder into the nostrils using a reed tube until sneezing occurs. Use a small amount frequently for slower but steady effect.
[Verse] Bi Yun San eliminates headwind symptoms, especially effective for brow bone sour ache. Ingredients include Ebusicao, Xinyi, Qingdai, Chuanxiong, and Xixin ground together, inhaled periodically.
Alternative Method: Application to Both Taiyang Acupoints
Indication: Severe headache feeling like shattering.
Composition: Que Nao, Chuanxiong, Baifuzi (equal parts).
Method: Grind into powder, mix with scallion juice to form a paste, apply on paper to both Taiyang acupoints. Effective.
For postpartum wind-cold invading the brain causing headache, sweating methods are contraindicated. Siwu Tang with doubled Chuanxiong and added Jingjie Sui should be taken for a gradual effect.
Headache caused by Yangmei Du entering the brain marrow is treated in the Syphilis section.
Alternative Formula: Siwu Tang (see Ulcer Section)
