If a woman suffers from abdominal pain, Xiao Jian Zhong Tang (Minor Construct the Middle Decoction) is the primary treatment.
[Annotation]
If the acute and urgent abdominal pain is caused by excessive Liver Wood leading to Spleen Earth deficiency, with middle Jiao deficiency and cold, this formula can be used to tonify deficiency, relieve tension, and stop pain.

Question: A woman is ill; her diet remains normal, but she experiences vexation and heat, cannot lie down, and instead must lean forward to breathe, unable to lie flat. What is the cause? The Master said: This condition is called "Zhuan Bao" (Displaced Bladder), characterized by urinary retention. It occurs because the ligaments of the bladder are twisted and obstructed, causing this disease. If urination is promoted, the disease will heal. Shen Qi Wan (Kidney Qi Pill) is indicated for treatment.
[Annotation]
The pathology is not in the Stomach, hence diet remains normal. The pathology is in the Bladder, hence urinary retention. Yang Qi fails to transform, resulting in vexation and heat. Water fluids fail to descend, causing the patient to lean forward and pant, unable to lie flat. This disease is named Zhuan Bao because the bladder ligaments are twisted and unsmooth, leading to this condition. Therefore, promoting urination leads to recovery. The primary treatment uses Shen Qi Wan (Kidney Qi Pill) to warm and move the Yang Qi of the Lower Jiao. Once Yang Qi transforms, urine is excreted, and all symptoms resolve naturally. Here, "Bao" (Bladder/Uterus) refers to the urinary bladder, not the blood uterus (womb).
[Collected Annotations]
Zhao Liang said: This formula appears in the Deficiency and Emaciation section to treat lower back pain, difficult urination, and urgency of urination. Why is it used here as well? It is used due to Kidney deficiency. Tonifying the Kidney restores Qi transformation; once Qi transforms, water flows, and the disease heals. However, is Zhuan Bao always caused by Lower Jiao Kidney deficiency and failure of Qi transformation? It could also be due to Middle Jiao Spleen deficiency failing to distribute essence to the bladder, or Upper Jiao Lung deficiency failing to transmit fluids downward to the bladder. Or it may result from the weight of the fetus pressing on the bladder, or holding urine while engaging in sexual intercourse. All these factors can cause this disease; one must seek the underlying cause to treat it.
Eight liang of Gan Di Huang (Rehmanniae Radix Preparata), four liang of Shu Yu (Dioscoreae Rhizoma), four liang of Shan Zhu Yu (Corni Fructus), three liang of Ze Xie (Alismatis Rhizoma), three liang of Fu Ling (Poria), three liang of Mu Dan Pi (Moutan Cortex), one liang of Gui Zhi (Cinnamomi Ramulus), and one liang of Fu Zi (Aconiti Lateralis Radix Praeparata, processed).
These eight ingredients are powdered, mixed with refined honey to form pills the size of wutong seeds. Take fifteen pills with wine initially, increasing gradually to twenty-five pills, taken twice daily.
[Collected Explanations]
Li [Name obscured] said: The formula is named Shen Qi Wan (Kidney Qi Pill) because Qi belongs to Yang, aiming to tonify the true Yang Qi of the Kidney. It contains the components of Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) to strengthen Kidney Water and nourish the source of urine. Fu Zi (Aconite) and Gui Zhi (Cinnamon Twig) supplement the Fire of the Gate of Vitality (Ming Men) to transform Bladder Qi. This steams and vaporizes body fluids, unblocking the water passages so that urination becomes smooth. This is why Wu Ling San (Five-Ingredient Powder with Poria) is not used, but Shen Qi Wan (Kidney Qi Pill) is chosen.

For women suffering from coldness in the external genitalia, warm internal sitz medicines should be used. She Chuang Zi San (Cnidium Seed Powder) is the primary treatment.
[Annotation]
Yin Han (Yin Cold) refers to coldness in the anterior yin (external genitalia), treated with warm internal sitz medicines. She Chuang Zi (Cnidii Fructus) has a warm nature and can strengthen Yang; therefore, it is inserted into the vagina to assist Yang and dispel Yin cold.
[Collected Annotations]
Shen Mingzong said: This is a formula for treating pulling pain in the yin region and aversion to cold in the lower abdomen. When the Bao Men (gate of the uterus) suffers from Yang deficiency and cold invasion, the manifestations vary and are not limited to just lower abdominal aversion to cold. However, since the cold enters through the vaginal opening and does not exit from the exterior, the site of pathogenic invasion must be warmed for the disease to heal. Therefore, only She Chuang Zi (Cnidii Fructus), a herb with strong heat properties to warm and assist Yang, is used. It is inserted into the yin to warm the uterus, allowing the pathogen to leave and the disease to heal spontaneously.
