Pulse Diagnosis:
The pulse of Deficient Fire appears at the Deep position, rapid and forceful with Yang hyperactivity, indicating invasion in chronic illness. A Deep and Forceful pulse indicates Yin essence floating outward; urgently consider methods to nourish Water and submerge Yang. In the two Cun positions, a Rapid and hurried pulse shape suggests early intervention with methods to clear Heart and drain Lung. A Wiry and Full pulse at the Left Guan position indicates excessive Liver Fire; a Rapid and Large pulse at the Right Guan position indicates excessive Stomach Yang. Methods should be divided between draining and clearing based on interior/exterior differentiation; sweating or purging methods should be chosen based on superficial/deep pulses. A Deep pulse for Excess Fire is an ominous sign; a Floating pulse for Deficient Fire indicates a critical condition difficult to control. Grasp these distinctions clearly; focus your spirit under the fingers for careful diagnosis.
Plague miscellaneous qi refers to the abnormal qi of heaven and earth. Yang Lishan said: "The arrival of toxic fog has no cause; the emergence of smoke and miasma has no fixed time." Damp-heat steaming... causes... wood... wherever it goes... cannot be restricted.
In the disease of plague, its miscellaneous qi is inhaled from the mouth and nose, travels directly through the central pathway, and spreads throughout the San Jiao (Triple Burner). Some remain in the Upper Jiao without spreading to the Middle Jiao; some pass through the Middle Palace and... meridians... go... deficiency... Jingyue... lower Tai... must... cold... plague... confusion...
One must first observe the tongue coating turning slightly yellow with moisture, then turning gray-black, or transforming into white sand-like coating, accompanied by vomiting or hiccups, severe whole-body fever, excessive thirst, rash appearing immediately upon fever onset, sudden swelling of head and face upon fever onset, sore throat and oral ulceration, bleeding from mouth or nose, body cold as ice but breath hot as fire, or headache as if splitting with mouth pain, limb spasms, and teeth grinding. Symptoms cannot be exhaustively listed; clinical judgment is required. One must smell the breath coming from the mouth as hot as fire, with a sense of stifling oppressive qi; this constitutes the general picture of plague cases. Several prescriptions are briefly provided for selection.
