Stir-fry Amomum fruit (Sha Ren) into powder, take 2.5 grams mixed with wine.
For pregnant women suffering from smallpox accompanied by fetal pain, this formula is indicated.
Fetal pain is caused by heat leading to Qi stagnation. Sha Ren (Amomum fruit) is pungent and warm, promoting free flow without causing stagnation; therefore, it can regulate Qi and calm the fetus.
Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Rhizoma, stir-fried), Fu Ling (Poria), Gan Cao (Licorice Root), Shao Yao (Peony Root, wine-fried), Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Sha Ren (Amomi Fructus), Zi Su (Perillae Folium), Huang Qin (Scutellariae Radix, wine-fried), Chen Pi (Citri Reticulatae Pericarpium), Xiang Fu (Cyperi Rhizoma, vinegar-fried), Da Fu Pi (Arecae Pericarpium, washed clean).
This formula may be used alternatively for pregnant women suffering from smallpox.
When Qi and Blood are deficient, the fetus is unsettled. When Qi and Blood are hot, the fetus is unsettled. When Qi and Blood are stagnant, the fetus is unsettled. In this formula, Ren Shen, Bai Zhu, Fu Ling, and Gan Cao are used to tonify Qi. Dang Gui, Chuan Xiong, and Shao Yao are used to nourish Blood. Huang Qin is used to clear Heat. Sha Ren, Xiang Fu, Zi Su, Chen Pi, and Da Fu Pi are used to promote movement and resolve stagnation.
Narration: Women's miscellaneous diseases are similar to men's, except for menstruation, pregnancy, and childbirth. What is examined here pertains only to formulas for menstruation, pregnancy, and childbirth. For other conditions, one should refer to complete formulas to find the syndrome differentiation and treatment.
Dang Gui (washed with wine), Chuan Xiong, Bai Shao Yao (Peony Root, wine-fried), Shu Di Huang (Rehmanniae Radix Praeparata).
For irregular menstruation in women, this formula serves as the principal base for modification.
The true essence of Wuji (Limitless), the refined essence of Yin and Yang and the Five Elements, combine wondrously to condense. The Way of Heaven produces males, and the Way of Earth produces females. Females operate according to the Way of Earth; thus, treating female disorders prioritizes Yin. At age fourteen (two sevens), Tian Gui (menstrual essence) arrives, and menstruation occurs on time. This is because females obtain the Yin of Kun (Earth); within Yin, there must be Yang. Hence, the cycle is based on seven: at one seven, teeth change; at two sevens, Tian Gui arrives. Humans are born receiving the Qi of Heaven and Earth, thereby mirroring them. The moon represents the Yin of Heaven; it waxes and wanes with the lunar phases. Similarly, a woman's blood descends every thirty days. Since the descent of blood resembles the moon, it is named "Yue Shi" (Monthly Affairs). The Classic states: "When menstruation comes on time, conception is possible." Therefore, for irregular menstruation, this formula is appropriate. It should be modified according to Cold, Heat, Deficiency, and Excess to harmonize menstruation. When menstruation is regular, Yin and Yang are in harmony, allowing for the generation of life, which is the path to conception. In this formula, Dang Gui, Shao Yao, and Di Huang are substances with rich flavors. Rich flavor belongs to Yin within Yin, thus benefiting Blood. Analyzed separately: Dang Gui is pungent and warm, activating Blood; Shao Yao is sour and cold, astringing Blood; Shu Di Huang is sweet and moist, tonifying Blood. Furthermore: Dang Gui enters the Heart and Spleen meridians; Shao Yao enters the Liver meridian; Shu Di Huang enters the Kidney meridian. As for Chuan Xiong, it penetrates from top to bottom, moving Qi within the Blood. This is why Si Wu Tang (Four Substances Decoction) is considered the essential medicine for women, and must be the primary formula for regulating menstruation. If the pulse is rapid and the blood color is purple-black indicating Internal Heat, add Huang Qin and Huang Lian to the base formula. If the pulse is slow and blood is coagulated indicating Cold, add Guan Gui (Cinnamon Twig) and Fu Zi (Aconite Lateralis Radix Praeparata) to the base formula. For obese individuals with Phlegm, add Ban Xia (Pinelliae Rhizoma), Chen Pi, and Nan Xing (Arisaematis Rhizoma). For thin individuals with Fire, add Shan Zhi (Gardeniae Fructus), Huang Bo (Phellodendri Cortex), and Zhi Mu (Anemarrhenae Rhizoma). For those with depression, add Xiang Fu, Cang Zhu (Atractylodis Rhizoma), Sha Ren, and Shen Qu (Massae Fermentatae Medicamentosa). For those with stagnation, add Tao Ren (Persicae Semen), Hong Hua (Carthami Flos), Yuan Hu Suo (Corydalis Rhizoma), and Rou Gui (Cinnamomi Cortex). Early menstruation indicates Heat; late menstruation indicates Cold, Depression, Qi Stagnation, or Phlegm. For Qi Deficiency, add Ren Shen and Huang Qi (Astragali Radix). For Qi Excess, add Zhi Ke (Aurantii Fructus Immaturus) and Hou Po (Magnoliae Officinalis Cortex). A question arises: Are there cases where Si Wu Tang is unsuitable? I reply: Yes. For those with faint Qi, Chuan Xiong is unsuitable, lest its pungent and aromatic nature further deplete True Qi. For those with loose stools, Dang Gui is unsuitable, lest its moistening and slippery nature exacerbate downward discharge. For those with slow pulse and abdominal pain, Shao Yao is unsuitable, lest its sour and cold nature increase Middle Jiao Cold. For those with chest and diaphragm fullness/stuffiness, Di Huang is unsuitable, lest its sticky and cloying nature increase obstruction. Those who understand will apply it correctly; those who do not will misuse it.
