余长孙端甫，六岁时，左目之下，患暑疖大如覆杯。捣马齿苋敷之，疖虽消而疟作，经月不已，服药不应。忽于鼻孔内涌出花红脓盈碗，疟不复作。盖暑伏于营，凝结而为疖，散布而为疟。凡遇有疖不化脓，而疟不已，诸药不应者，宜参入排脓治疖之法，书此以识一异。 《周礼·天官疾医职》曰：秋时有疟寒疾。
《吕氏春秋孟秋纪》曰：孟秋行夏令，民多疟疾。（《礼记》《月令》同。）

瘴疟发作时，若指爪青暗，肢体麻木，适宜在瘴气门及沙蜮门中参考相关法治之。
疟疾发作数次后，忽然四肢冰冷，脉搏潜伏摸不到的，属于不治之症。这是气脱的缘故。
夜光丸，主治截断一切痎疟，老少强弱男女都适宜。

共研为细末，用神曲稀糊和药丸如梧桐子大，每早服二钱，晚服二钱，开水送下，病愈即止，不必服完所有药剂，不过七天必定痊愈。
小儿加鸡胗内膜（鸡内金）一两，每次服一钱。
孕妇常山减半。
体虚之人及老人用鲜首乌三五钱，煎汤送服，每早晚服钱半；壮年人及年轻人每早晚服三钱。
朱月樵（字土龙），诗歌冠绝一时，善于谈论医理。他的表兄孙吉人，患疟疾且热势很重，自己吃了几斤荸荠，导致大量腹泻黄水数日，疟疾不再发作，但身体沉重困倦反而加重，小便浑浊发红，大便也排不出，拖延到十天，小便时突然尿道疼痛，只能排出点滴尿液，多次使用通淋药物无效，病情转为危急紧迫，邀请我诊治。当时朱月樵在场，说这是燥屎阻塞逼迫膀胱，不是普通的癃淋（排尿困难）可比。我因此先用大黄通便，接着服用龙胆泻肝汤送服滚痰丸，排出大便一斗多，小便随即通畅。痊愈十天后，阴囊先冷后热，小便仍然像之前那样点滴作痛，每天按时发作，痛苦万分，过一段时间才停止，停止后小便恢复正常。朱月樵说这又与前症不同，大概此人素来容易患疟疾，现在又在疟疾之后，应当是疟邪未尽，趁着州都（膀胱）虚弱，陷入膀胱之腑。阴囊寒热，就像疟疾有寒热一样。尿道疼痛小便淋漓，淋漓停止后如常，就像疟疾有出汗，汗透则解一样。于是商议用天水散合小柴胡汤，加羌活、青皮，服用后出现寒战，过一会儿寒已退复发热，仍归于疟疾，出一身汗而痊愈。附上此案以广见闻。
我的长孙端甫，六岁时，左眼下患暑疖，大小如倒扣的杯子。捣碎马齿苋外敷，疖肿虽然消退但疟疾发作，持续一个月不止，服药无效。忽然从鼻孔内涌出花红色脓液盈满一碗，疟疾不再发作。这是因为暑邪伏于营分，凝结而成疖，散布而成疟。凡是遇到疖肿不化脓，而疟疾不止，各种药物无效的，适宜参考加入排脓治疖的方法，写下此条以记录这一奇异案例。
《周礼·天官疾医职》记载：秋季时有疟寒疾病。
《吕氏春秋·孟秋纪》记载：孟秋时节施行夏令政令，百姓多患疟疾。（《礼记》《月令》内容相同。）
《说文》记载：疟，是冷热交替发作的疾病。痁，是热疟。痎，是隔两天发作一次的疟疾。
《九峰医案》记载：疟邪之后，余热未除，先天之本固不足，后天之本也不振作。肾为先天，脾为后天，脾肾不足以化生精微，酿生湿热，湿气郁蒸发黄，五液不充盈，热留于阴分，导致产生潮热。阳明气至则咬牙，肾虚肝热则抽搐，脉象滑数无神，滋补肾阴，调理阳明，化解湿热，清除余热，顺应其性以调治。

"Jie" refers to San Malaria (chronic, recurrent malaria). Malaria must involve latent summer-heat internally and external contraction of wind-cold, ultimately resolved through sweating.

If there is no sweat, add Notopterygii Rhizoma et Radix (1 qian), Saposhnikoviae Radix (1 qian), and Ligustici Chuanxiong Rhizoma (1 qian).
If there is excessive sweating, add Ostreae Concha (3 qian).
If chills are severe, add Cinnamomi Ramulus (0.8 fen), Zingiberis Rhizoma (0.5 fen). In severe cases, add Processed Aconiti Lateralis Radix Praeparata, removing Zingiberis Rhizoma Recens and Scutellariae Radix. Pin Malaria (malaria characterized by predominant chills) uses the same method.
If heat is severe, add Bambusae Folium (30 leaves), Gypsum Fibrosum (5 qian). Or use Scrophulariae Radix, Anemarrhenae Rhizoma, etc. Dan Malaria (malaria characterized by predominant high fever) uses the same method.
If Qi deficiency, add Ginseng Radix et Rhizoma (1 qian), Astragali Radix (2 qian), Atractylodis Macrocephalae Rhizoma (1.5 qian).
If Yin deficiency, add Rehmanniae Radix (4 qian), or Ophiopogonis Radix, etc.
If excess syndrome, add Arecae Semen (1 qian), Magnoliae Officinalis Cortex (0.8 fen), Tsaoko Fructus (0.5 fen).
If constipation, add Rheum Palmatum Radix et Rhizoma (3 qian), Mirabilite (2 qian).
If urinary retention, add Plantaginis Semen (3 qian), Akebiae Caulis (1 qian).
For chronic malaria, add Testudinis et Carapax et Plastrum (3 qian), Achyranthis Bidentatae Radix (3 qian).
For Phlegm Malaria, add Processed Dichroae Radix fried in wine (3 qian).
For Food Stagnation Malaria, add Hordeum Vulgare Fructus Germinatus (2 qian), Mass Medicamentum Fermentatum Chinense (2 qian). Or use Hordeum Vulgare Fructus Germinatus, Crataegi Fructus, etc.
In spring, add Saposhnikoviae Radix (1 qian), Perillae Caulis (1 qian).
In summer, add Moslae Herba (1 qian), Coptidis Rhizoma (0.8 fen).
In autumn, add Citri Reticulatae Pericarpium Viride (1 qian).
In winter, add Cinnamomi Cortex (0.5 fen).
For treating San Malaria, add Polygoni Multiflori Preparata (fresh) (0.5 fen), Ginseng Radix et Rhizoma (1 qian), Testudinis et Carapax et Plastrum (3 qian), Zingiberis Rhizoma Recens (3 slices), Jujubae Fructus (black) (3 pieces), remove Scutellariae Radix.
To intercept malaria, add Processed Dichroae Radix fried in wine (3 qian), Mume Fructus (3 pieces). Or use Yeguang Wan (3 qian).
Malaria Mother (mass under the hypochondrium formed after prolonged malaria) is suitable for Jin Gui Biejia Jian Wan, or Jisheng Biejia Yin.
When Zhang Malaria occurs, if fingernails are cyanotic and dark, and limbs are numb, it is appropriate to refer to methods from the Zhang Men chapter and Shayu Men chapter.

Grind into fine powder, mix with a thin paste made from Mass Medicamentum Fermentatum Chinense to form pills the size of tung tree seeds. Take 2 qian every morning and 2 qian every evening with warm water. Stop when the disease is cured; do not finish the entire dose. It will definitely heal within seven days.
For children, add Gallus Gallus domesticus Gigerii Endothelium Corneum (1 liang), take 1 qian per dose.
For pregnant women, reduce Dichroae Radix by half.
For deficient persons and the elderly, decoct fresh Polygoni Multiflori Preparata (3-5 qian) in water to take with the pill, 1.5 qian morning and evening; for robust adults and youths, take 3 qian morning and evening.
Zhu Yueqiao (courtesy name Tulong), whose poetry was unparalleled at the time, was skilled in discussing medical principles. His cousin, Sun Jiren, suffered from malaria with severe heat. He ate several jin of water chestnuts himself, causing severe diarrhea of yellow fluid for several days. The malaria stopped recurring, but his condition became heavy and lethargic, urine became turbid and red, and defecation ceased. After ten days, sudden pain occurred in the urethra during urination, with only drops passing. Multiple administrations of diuretic drugs were ineffective, and the situation became critical. I was invited to diagnose. At that time, Zhu Yueqiao was present and stated that this was dry stool blocking and pressing the bladder, not comparable to ordinary strangury. I therefore first used Rheum Palmatum Radix et Rhizoma to promote bowel movement, then administered Longdan Xiegan Tang (Gentiana Liver-Draining Decoction) to send down Guntan Wan (Rolling Phlegm Pill). More than one dou of stool was passed, and urination subsequently became unobstructed. Ten days after recovery, the scrotum experienced cold followed by heat, and urination remained drop-by-drop with pain, occurring daily at regular intervals, causing extreme suffering until relief came after some time, after which urination returned to normal. Zhu Yueqiao said this was different from the previous condition. Presumably, the person had a constitution prone to malaria. Now, following malaria, it should be that the malaria pathogen remains incomplete, taking advantage of the deficiency of the Zhou Du (bladder) to sink into the bladder organ. The cold and heat in the scrotum is like the chills and fever in malaria. The urethral pain and dribbling, stopping normally afterwards, is like sweating in malaria; once sweat penetrates, resolution occurs. Thus, we discussed using Tiansan San combined with Xiaochai Hu Tang (Minor Chest Harmony Decoction), adding Notopterygii Rhizoma et Radix and Citri Reticulatae Pericarpium Viride. After taking it, chills occurred. After a while, chills subsided and heat returned, reverting to malaria, and healed with a sweat. This case is attached to broaden knowledge.
My eldest grandson, Duanfu, at six years old, developed a large summer boil under the left eye, the size of an inverted cup. Crushed Portulaca Oleracea was applied externally. Although the boil subsided, malaria started and persisted for a month without response to medication. Suddenly, a bowlful of flower-red pus gushed from the nostrils, and the malaria ceased. This is because summer-heat was latent in the Ying (Nutrient) level, condensing to form boils and dispersing to cause malaria. Whenever a boil does not suppurate and malaria persists despite various medications, it is appropriate to incorporate methods for draining pus and treating boils. This is recorded to note a unique case.

The *Lüshi Chunqiu · Mengqiu Chapter* states: If summer policies are enacted in early autumn, the people will suffer much from malaria. (Same content in *Book of Rites* and *Monthly Ordinances*.)
The *Shuowen* states: Malaria is a disease where heat and cold alternate. Shan is heat malaria. Jie is malaria occurring every two days.
*Jiufeng Yi'an* records: After malaria pathogen, residual heat remains unremoved. The congenital foundation is insufficient, and the acquired foundation is also weak. Kidney is congenital, Spleen is acquired. Insufficiency of Spleen and Kidney fails to transform essence and micro-nutrients, brewing damp-heat. Dampness steaming causes jaundice, five fluids are not filled, heat stays in the Yin level, causing tidal fever. When Yangming Qi arrives, teeth gnashing occurs; Kidney deficiency with Liver heat causes convulsions. Pulse is slippery and rapid without spirit. Nourish Shaoyin, regulate Yangming, resolve damp-heat, clear residual heat, and adjust according to its nature.

Dry mouth, slight thirst, frequent urination, yellow but not turbid, pulse still slightly wiry. Yellow tint invades white of eyes from canthus. Dampness harbors residual heat not cleared. Classics say running fast causes sweat from Kidney, galloping causes much sweat, panting and tinnitus. Left hernia large as goose egg, nose often flows clear mucus. This is the original constitution, indicating Lung and Kidney insufficiency. Plan to peacefully tonify the Three Yin, make into pills for slow treatment.
