A long iron wire fishhook inserted into the anus: The back of the hook must be round, allowing it to enter, while the hook point faces outward, catching the internal flesh. It is difficult to pull out, causing endless pain. Use a thin bamboo shoot, matching the diameter of the patient's anus, clear the nodes inside to form a hollow tube, approximately one chi (approx. 33 cm) long, and smooth one end of the bamboo. Place the tube over the exposed handle of the hook, insert it into the anus until the hook point is enclosed within the bamboo tube, then pull the bamboo tube out; the hook will emerge along with the tube.
(If it is an Yin abscess [Yin Ju], external treatment should preferably employ Garlic-partitioned Moxibustion [Ge Suan Ai Jiu] to restore Yang.)
For the elderly and those with weak constitutions who develop various sores such as carbuncles and furuncles [Yong Ju], Dakbei [Back Carbuncle], or Duikou [Opposite Mouth Ulcer], although the skin surface may appear red and hard without significant swelling, and the pulse is floating, large, and forceless, or deep and weak, this is due to the decline of True Yang [Zhen Yang], with floating fire [Fu Huo] venting outward. Initially, it is appropriate to administer several doses of Jingfang Baidu San (Scutellaria and Saposhnikovia Decoction for Overcoming Toxicity) or Weisheng Tang (Health Preservation Decoction) to disperse the pathogenic factors subsequently. One must not rigidly adhere to the notion that all sores belong to Fire [Huo]. If tonifying medicines are used indiscriminately, they will assist the fire and close off the pathogens, making it difficult for the toxin to exit. Do we not know that when former sages examined symptoms and determined them to be truly deficient, they relied on Ginseng Radix et Rhizoma [Renshen] and Astragali Radix [Huangqi] to tonify Qi, Angelicae Sinensis Radix [Danggui] and Paeoniae Radix Alba [Baishao] to nourish Blood, with small amounts of Aconiti Lateralis Radix Praeparata [Fuzi] and Cinnamomi Cortex [Rougui] to steam and brew Yuan Yang [Primal Yang], support Zheng Qi [Upright Qi], and push out the toxin? Otherwise, the toxin often sinks inward, failing to achieve complete success. For those whose Yin deficiency fails to constrain Yang, the Ba Wei Wan method [Eight-Ingredient Pill Method] should be used to nourish Yin to warm Yang. If it is Excess Fire [Shi Huo], the pulse must be surging, rapid, and forceful, accompanied by irritability, thirst, and constipation; in such cases, Neishu Huanglian Tang (Internal Purging Coptis Decoction) should be taken. However, for Ding Chuang [Carbuncle/Boil] and Syphilis [Meidu], there is never any rationale for using tonification throughout the course of the disease.
When a girl reaches fourteen sui (years), Tian Gui [Heavenly Water / Menstrual Essence] arrives, and menstruation occurs regularly. Upon pregnancy, the menstrual blood remains internally to nourish the fetus. After childbirth, this fluid ascends to become breast milk. If pregnancy occurs again, the remaining previous breast milk is steamed and constrained by fetal Qi, transforming entirely into hot toxins and clear fluids. If a child suckles this milk, they will inevitably develop conditions such as internal heat, yellow complexion, diarrhea, and frequent crying.
In the elderly and those with chronic illnesses, difficulty in defecating and urinating with dryness, blockage, and distension arises from sinking of Deficient Qi [Xu Qi], which fails to separate the turbid residues and transport water turbidity, preventing their discharge through the lower orifices. If the scrotum and penis swell and appear bright without subsiding, this indicates injury to the Spleen [Pi], which loses its ability to control water, leading to water dampness pouring downward into the scrotum. Such conditions are incurable; do not expect recovery.
