[ Lizhong Wan (Regulate the Middle Pill) ] The Heart and Lung are located above the diaphragm, belonging to Yang. The Kidney and Liver are located below the diaphragm, belonging to Yin. These are the upper and lower Zang organs. The Spleen and Stomach belong to Earth and reside in the Central Region (Zhongzhou). Among the Five Zang organs, it is called the Solitary Zang; within the San Jiao (Triple Burner), it is called the Middle Jiao. Because the Middle Jiao governs independently in the center, once there is any disharmony, this pill specializes in treating it, hence named Lizhong Wan. Ren Shen (Ginseng Radix) has a sweet flavor and warm nature. The Nei Jing states: "The Spleen desires relaxation; urgently feed it with sweet substances to relax it." To relax the middle and benefit the Spleen Qi, sweet substances must be primary. Therefore, Ren Shen is used as the Sovereign herb. Bai Zhu (Atractylodis Macrocephalae Rhizoma) has a sweet flavor and warm nature. The Nei Jing states: "The Spleen dislikes dampness; sweet flavors overcome dampness." To warm the middle and overcome dampness, sweet substances must assist. Therefore, Bai Zhu is used as the Minister herb. Gan Cao (Licorice Root) has a sweet flavor and neutral nature. The Nei Jing states: "Of the five flavors, sweet enters the Spleen first." For those with Spleen deficiency, use sweet substances to tonify it. To tonify the middle and assist the Spleen, sweet preparations are necessary. Therefore, Gan Cao is used as the Assistant herb. Gan Jiang (Dried Ginger Rhizome) has a pungent flavor and hot nature. The Stomach prefers warmth and dislikes cold. Cold prevents the Middle Jiao from being governed. The Nei Jing states: "When cold pathogen prevails, pacify it with pungent-hot substances." To disperse cold and warm the Stomach, pungent substances must be used first. Therefore, Gan Jiang is used as the Guide herb. The Spleen and Stomach reside in the center; when diseased, pathogenic factors wander up, down, left, and right, nowhere they do not go. Thus, there are various modifications. ○ If there is throbbing below the umbilicus, it indicates Kidney Qi moving. Remove Bai Zhu and add Gui Zhi (Cinnamon Twig). When Qi stagnates and cannot drain, it throbs. Bai Zhu is sweet and tonifies Qi; removing Bai Zhu allows Qi to scatter easily. Gui Zhi is pungent and hot. When Kidney Qi moves and threatens to become Ben Tun (Panic Disease, characterized by Qi rushing upward from the lower abdomen), one must take pungent-hot substances to disperse it. Hence, Gui Zhi is added to dissipate Kidney Qi. The Classics state: "Pungent enters the Kidney and can drain Ben Tun Qi." ○ For excessive vomiting, remove Bai Zhu and add Sheng Jiang (Fresh Ginger Rhizome). When Qi rebels upward, vomiting increases. Bai Zhu is sweet and cloying, unsuitable for those with rebellious Qi. The Qian Jin Fang states: "Those who vomit often should take Fresh Ginger; it is the sacred medicine for vomiting." Sheng Jiang is pungent and dispersing, so it is added for excessive vomiting. ○ For excessive diarrhea, continue using Bai Zhu. When Qi leaks and fails to consolidate, diarrhea increases. Bai Zhu is sweet and consolidating, helping Zheng Qi (Upright Qi) to consolidate and preventing downward leakage. Some say that excess Dampness causes loose stools; Bai Zhu specifically removes Dampness, so it is added for excessive diarrhea. ○ For palpitations, add Fu Ling (Poria). Fluid accumulation causes palpitations. Fu Ling is sweet and promotes leaching/drainage, suitable for latent fluid retention. ○ For thirst desiring water, double the amount of Bai Zhu. Insufficiency of Body Fluids causes thirst. Bai Zhu is sweet and can replenish Body Fluids, so it is added. ○ For abdominal pain, add Ren Shen. Deficiency causes pain. The Nei Jing states: "Tonification can remove weakness," referring to herbs like Ren Shen and foods like mutton. ○ For excess cold, add Gan Jiang. Pungent-hot substances can disperse cold. ○ For abdominal fullness, remove Bai Zhu and add Fu Zi (Aconite Lateralis Radix Praeparata). The Nei Jing states: "Sweet substances cause central fullness." Bai Zhu is sweet and cloying/tonifying, so it is removed for abdominal fullness. Fu Zi is pungent and hot. Qi stagnation and depression cause abdominal fullness. Use heat to overcome cold and pungency to disperse fullness. Hence, Fu Zi is added. The Nei Jing states: "Treat heat with cold, treat cold with heat." This refers to this principle.

[Jianzhong Tang (Establish the Middle Decoction)] The Nei Jing states: "The Liver grows on the left, the Lung stores on the right. The Heart is positioned above, the Kidney resides below." The four Zang organs occupy the left, right, upper, and lower positions. The Spleen belongs to Earth, corresponds to the Center, resides in the midst of the four Zang organs, governs the Middle Jiao, generates Ying (Nutrient) and Wei (Defensive) Qi, and circulates Body Fluids. Once there is disharmony, Ying and Wei lose their source of generation, and Body Fluids lose their pathway of circulation. One must use this decoction to warm the middle and benefit the Spleen, hence named Jianzhong Tang. Jiao Yi (Malt Sugar) is sweet and warm; Gan Cao is sweet and neutral. The Spleen desires relaxation; urgently feed it with sweet substances to relax it. Establishing the Spleen requires sweet substances as primary. Thus, Jiao Yi is the Sovereign herb, and Gan Cao is the Minister herb. Gui Zhi is pungent and hot, possessing pungent-dispersing and moistening properties. For insufficiency of Ying and Wei, use moistening and dispersing methods. Shao Yao (Peony Root) is sour and slightly cold. Sourness implies gathering and draining. For inadequacy of Body Fluids, use gathering and circulating methods. Therefore, Gui Zhi and Shao Yao are Assistants. Sheng Jiang is pungent and warm; Da Zao (Jujube Fruit) is sweet and warm. The Stomach is the source of Wei Qi; the Spleen is the root of Ying Qi. The Huangdi Zhenjing states: "Ying Qi emerges from the Middle Jiao, Wei Qi emerges from the Upper Jiao." Wei Qi belongs to Yang; insufficiency must be supplemented with pungent substances. Ying Qi belongs to Yin; insufficiency must be supplemented with sweet substances. Sweet and pungent combine, strengthening the Spleen and Stomach and unblocking Ying and Wei. Thus, Sheng Jiang and Da Zao are Guides. Some ask: Guizhi Tang releases the exterior, with little Shao Yao; Jianzhong Tang warms the interior, with much Shao Yao. They fail to understand the distinction between near and far formulations. Pathogens in the skin are near, so the dosage is small. In Guizhi Tang, Shao Yao assists Gui Zhi for dispersion, differing from Jianzhong Tang. Pathogens in the heart and abdomen are far, so the dosage is large. In Jianzhong Tang, Shao Yao assists Jiao Yi to establish the Spleen, not used together with Gui Zhi as in Guizhi Tang. The Nei Jing states: "For near diseases with odd/even combinations, make the dosage small; for far diseases with odd/even combinations, make the dosage large." This is what is meant. Those who vomit should not use this decoction because of its sweet taste.
