The Master said: The pulse of malaria naturally presents as a string-like (Xian) pulse. A string-like and rapid pulse indicates mostly heat; a string-like and slow pulse indicates mostly cold. A string-like, small, and tight pulse can be treated with purgation; a string-like and slow pulse can be treated with warming methods; a string-like and tight pulse can be treated with diaphoresis and acupuncture/moxibustion; a floating and large pulse can be treated with emesis. A string-like and rapid pulse indicates wind disease onset, which can be managed by observing dietary influences.
A question was asked: If malaria begins on the first day of the month, it should heal by the fifteenth day. If it does not heal, it should resolve by the last day of the month. If it still does not heal, what should be done? The Master said: This is because the pathogenic factor has knotted into a mass, named "Malaria Mother" (Nuemo), which requires urgent treatment. Biejia Jian Wan (Turtle Shell Decoction) can be used.
[Biejia Jian Wan]
Bie Jia (Turtle Shell, 12 fen, roasted) Chi Xiao (Red Nitre, 12 fen) Qiang Lang ( Dung Beetle, 6 fen, roasted) Chan Chong (Ground Beetle, processed)
Mu Dan Pi (Tree Peony Bark, heart removed, each 5 fen) Feng Wo (Honeycomb, processed, 4 fen) Wu Shan (Black Cardamom/Orchid Tuber, burned) Huang Qin (Scutellaria Root)
Chai Hu (Bupleurum Root) Gan Jiang (Dried Ginger, processed) Da Huang (Rhubarb Root, each 3 fen) Shao Yao (Peony Root, 5 fen)
Gui Zhi (Cinnamon Twig) Hou Po (Magnolia Bark, stir-fried) Zi Wan (Aster Root, alternatively Zi Wei) E Jiao (Donkey-hide Gelatin, processed, each 3 fen)
Tao Ren (Peach Kernel, skin and tip removed) Qu Mai (Dianthus Herb) Shu Fu (Woodlouse, processed) Ting Li (Lepidium Seed, stir-fried, each 2 fen)
Shi Wei (Pyrrosia Leaf, hair removed) Ban Xia (Pinellia Tuber, washed, each 1 fen) Ren Shen (Ginseng Root, rhizome removed, 1 fen)
The above twenty-three ingredients are ground into powder. First, take one dou of ash from a smithy's forge and five hu of clear wine to soak the ash. When the wine has absorbed half, place the Bie Jia (Turtle Shell) into it and boil until it becomes soft and gelatinous like lacquer. Strain to obtain the juice, add all other medicines, and decoct into pills the size of tung tree seeds. Take seven pills on an empty stomach, three times a day. *Qianjin Fang* uses twelve slices of Bie Jia, 3 fen of Hai Zao (Seaweed), 1 fen of Da Ji (Euphorbia Root), and 5 fen of Meng Chong (Blister Beetle). Without Shu Fu or Chi Xiao, mix with Bie Jia decoction to form pills.
[Xiaochaihu Qu Banxia Jia Gualougen Tang] Treats malaria with thirst during attacks (Also effective for loose stools in both lower limbs according to *Waitai Miyao* and *Jingxin Lu*; formula from *Jingui Yaolue*).
Chai Hu (Bupleurum Root, 8 liang) Ren Shen (Ginseng Root) Huang Qin (Scutellaria Root) Gan Cao (Licorice Root, roasted)
Sheng Jiang (Fresh Ginger, sliced, each 3 liang) Gua Lou Gen (Trichosanthes Root, 4 liang) Da Zao (Jujube, 12 pieces, split)
The above seven ingredients are chopped. Decoct in one dou two sheng of water to obtain six sheng. Remove dregs, then decoct again to three sheng. Take one sheng warm, three times a day.
[Chaihu Guijiang Tang] (From *Sanyin Fang*)
Chai Hu (Bupleurum Root, 8 liang) Gui Zhi (Cinnamon Twig, bark removed) Huang Qin (Scutellaria Root, each 3 liang) Mu Li (Oyster Shell, processed)
Gan Cao (Licorice Root, roasted) Gan Jiang (Dried Ginger, processed, each 2 liang) Gua Lou Gen (Trichosanthes Root, 4 liang)
Decoct in one dou two sheng of water to six sheng. Remove dregs, then decoct again to three sheng. Take one sheng warm, three times a day. Mild irritability may occur after the first dose; sweating and cure follow the second dose.
The Master said: When Yin qi is isolated and exhausted, and Yang qi acts alone and flares up, symptoms include fever, shortness of breath, irritability, fullness, hot hands and feet, and a desire to vomit. This is named "Dan Nue" (Malaria with High Fever). If there is only heat without chills, the pathogenic factor resides internally in the Heart and Lungs and externally in the muscle interspaces, causing muscle wasting and emaciation.
"Wen Nue" (Warm Malaria) presents with a pulse similar to normal, no chills but only heat, pain in bones and sinews with irritability, and frequent vomiting. It is primarily treated with Baihu Jia Guizhi Tang (White Tiger plus Cinnamon Twig Decoction).
[Baihu Jia Guizhi Tang]
Shi Gao (Gypsum Fibrosum, 1 jin, crushed, wrapped in cotton) Zhi Mu (Anemarrhena Rhizome, 6 liang) Gui Zhi (Cinnamon Twig, 3 liang) Gan Cao (Licorice Root, 2 liang)
Jing Mi (Glutinous Rice, 6 he)
Chop the ingredients. Decoct in one dou two sheng of water until the rice is cooked. Remove dregs, then decoct to three sheng. Take one sheng warm, three times a day. Cure follows sweating.
Malaria characterized by excessive cold is named "Pin Nue" (Yin-type Malaria), primarily treated with Shuqi San (Shuqi Powder).
[Shuqi San]
Shu Qi (Prepared Perilla Leaf, washed to remove fishy smell) Yun Mu (Mica, roasted for three days and nights) Long Gu (Fossilized Bone, equal parts; some versions use Yun Shi instead of Yun Mu)
Grind into powder. Before the attack, take one qian with half a cup of sour water. For Wen Nue, add half a qian of Shu Qi; take at least one qian just before the attack.
[Muli Tang] Treats Mu Nue.
Mu Li (Oyster Shell, processed) Ma Huang (Ephedra Stem, nodes removed, each 4 liang) Shu Qi (Perilla Leaf, washed) Gan Cao (Licorice Root, roasted, each 2 liang)
Chop the ingredients. Decoct in eight sheng of water. First boil Ma Huang and Shu Qi to six sheng, skim off the foam, add other herbs, and decoct to two sheng. Remove dregs. Take one sheng warm. If vomiting occurs, do not administer further.
[Yijian Jin] Treats alternating chills and fever in malaria; considered a holy medicine for malaria.
Liu Huang (Sulfur) Pi Shuang (Arsenic Trioxide, equal parts)
Grind together into powder. Hand-roll a pinch of powder onto crimson silk, wrap it like a bean, tie tightly with fine thread, and cut with scissors. Ensure tight binding to prevent loss. Take one pill at an auspicious astrological time, swallowed with fresh cool water on an empty stomach. Keep secret. Administer the night before the expected attack.
On the morning of the actual attack, go early to a clean, secluded outdoor place to avoid detection; a spacious room is also acceptable.
[Kexiao Bingzi] Treats all types of malaria.
Long Nao (Borneol) She Xiang (Musk, each 0.5 liang) Zhu Sha (Cinnabar, 1.1 liang) He Ye (Lotus Leaf)
Lu Dou Fen (Mung Bean Flour) Zhi Gan Cao (Roasted Licorice Root, each 5 liang) Pi Shuang (Arsenic Trioxide, roasted, 2.5 liang) Ding Fen (Lead Carbonate, 0.5 liang)
Jin Bo (Gold Foil, 25 pieces for coating)
Grind into fine powder. Mix with refined honey to form pills. Each liang makes twenty pills, shaped into cakes, coated with gold foil. Take one cake per dose, dissolved in fresh cool water. For daily attacks, take before onset; for every-other-day attacks, take on non-attack nights; for attacks every few days, take the night before; for consecutive daily attacks, take at dawn.
[Wenpi San] Treats alternating chills and fever in malaria that persists for a long time.
Gan Cao (Raw Licorice Root, 0.5 liang) Lu Dou (Mung Bean, 1 liang) Chai He Che (Human Placenta, 1 liang) Pi Shuang (Arsenic Trioxide, 1.5 qian, separately ground)
First pound the Human Placenta and others into powder, mix evenly with Arsenic Trioxide, and sieve into fine powder. Take 0.5 qian per dose, swallowed with a little fresh cool water. For every-other-day attacks, take late at night on a favorable day. For frequent attacks, take late at night before sleep. Avoid meat, seafood, fruits, melons, alcohol, wheat products, fish, meat, and raw/cold/hard foods. Within three days, if the pathogen is deep, one dose suffices to stabilize. Those over ten years old take one "zi" amount; those aged three to ten take half a "zi". Pregnant women must not take this. Please prepare and administer it confidently; do not fear the arsenic. I have used this medicine for thirty to fifty years, saving many lives. Although Arsenic Trioxide is toxic, the cooling nature of Mung Bean and other ingredients neutralizes it, as does fresh cool water. This medicine does not cause vomiting. Some patients with phlegm-malaria due to accumulation vomit food or saliva upon attack even without medication; this is not drug-induced. For reference, Jin Ye Dan uses Sulfur; Zi Shuang Wan uses Ba Dou (Croton Seed) and Xing Ren (Apricot Kernel), which are highly toxic and hot but effective when processed. These can be used for children. However, they may not suit this specific case, hence the discussion.
[Changshan Yinzi] Treats malaria.
Zhi Mu (Anemarrhena Rhizome) Chang Shan (Dichroa Root) Cao Guo (Black Cardamom, each 2 liang) Gan Cao (Licorice Root) Wu Mei (Mume Fruit, each 3 liang) Liang Jiang (Galangal Root, 1.3 liang)
Chop the ingredients. Take 3 qian per dose with one cup of water, three slices of Fresh Ginger, and two Jujubes. Decoct to 0.8 portion, remove dregs, and take warm at any time.
[Caoguo Yinzi] Treats Spleen-cold malaria.
Cao Guo (Black Cardamom) Chuan Xiong (Ligusticum Rhizome) Zi Su Ye (Perilla Leaf) Bai Zhi (Angelica Dahurica Root) Liang Jiang (Galangal Root) Zhi Gan Cao (Roasted Licorice Root) Qing Pi (Citrus Peel, white part removed) Chen Pi (Aged Tangerine Peel, white part removed)
Grind into coarse powder. Take 2 qian per dose with one cup of water. Decoct to 0.7 portion, remove dregs, and take warm. Reserve the dregs to combine for the next dose. On the day of attack, take two doses; otherwise, take at any time.
[Jiaojiao Shuangjie Yinzi] Treats malaria, dispels miasma, and is highly effective.
Rou Dou Kou (Nutmeg) Cao Dou Kou (Cardamom, each two pods; one raw, one wrapped in flour and roasted until red, then remove flour)
Hou Po (Magnolia Bark, 2 cun; 1 cun raw, 1 cun ginger-processed) Gan Cao (Licorice Root, 2 liang; half raw, half roasted)
Sheng Jiang (Fresh Ginger, two pieces the size of jujubes; one raw, one wrapped in wet paper and roasted)
Chop evenly and divide into two doses. Decoct one bowl of water in a clay pot to one qian volume. Remove dregs. On the day of attack, take warm on an empty stomach. If ineffective, take the second dose for certain efficacy. Combine the two residues and decoct as one dose.
