Gao Shiqian, a clerk in the Yannan Hebei Circuit Judicial Intendant's Office, was over forty years old. In July of the Wuyin year of the Zhiyuan era, the summer heat had not yet receded. Due to official duties requiring outdoor labor and excessive drinking, he suffered from severe fever and thirst in the afternoon, which could not be relieved by drinking ice water. His condition was slightly lighter in the morning, and previous medications were ineffective, so he summoned me for treatment. Upon examining his pulse, it was wiry and rapid (Xian Shu). The *Jingui Yaolue* states: "The pulse of malaria naturally presents as wiry." A wiry and rapid pulse indicates abundant heat. The *Suwen: Malun* chapter states: "The pulse of Dan Malaria (a type of malaria characterized by fever without chills) is rapid. The patient has long harbored internal heat that is vigorous throughout the body. Qi rebels and rushes upward; the middle jiao is full and cannot vent outward. Due to physical exertion, the cou li (pores and interstitial spaces) open, allowing wind-cold pathogens to lodge within the skin and between the muscle layers, causing disease. Upon onset, yang qi becomes excessive and does not decline, thus illness occurs. Since the pathogen does not involve cold, there is only fever without chills. This is because the pathogenic factor hides internally while lodging externally between the muscle layers, causing emaciation and wasting of flesh, hence named Dan Malaria." The *Yueling* chapter of the *Liji* states: "If the practices of summer are followed in early autumn, the people will often suffer from Dan Malaria." Zhang Jiegu stated: "Acquired through movement, it is named Summerheat Stroke." It should be treated with Baihu Jia Zhizi Tang (White Tiger plus Gardenia Decoction). Gao Shiqian had traveled far and labored, suffering injury from summerheat. The heat from alcohol and summerheat fought each other, aided by the yang energy of the afternoon hours, resulting in severe fever and thirst, feeling as if inside a steamer. I first administered Chaihu Yinzi (Bupleurum Drink) in an amount of one liang to purge the excess, followed by Baihu Jia Zhizi Tang, taken one liang per dose. After several doses, he recovered.
Da Te'emuer, Vice Commander of the Southern Expedition Army, was ordered by imperial decree in the Jiyi year to garrison Xizhou. The area was low-lying and damp, and many soldiers suffered from malaria and dysentery. I compounded Chen Sha Dan (Cinnabar Pill) and Baizhu Anwei San (Atractylodes Macrocephala Stomach-Calming Powder), which cured most cases effectively.
[Chen Sha Dan] Treats malaria with great efficacy.
Zhusha (Cinnabaris [Zhu Sha], half incorporated into the pill, half used as coating) Xin Pi (Realgar mixed with Arsenic Trioxide [Red Orpiment]) Xionghuang (Realgar) (each 5 qian).
Grind the above three herbs into powder, mix with 6 qian of white flour, grind evenly, and make pills using water drop method, sized like paulownia seeds. Coat with Zhusha. Take one pill at auspicious astrological times, delivered with Wugen Shui (rainwater or dew). Avoid wet noodles and hot-natured foods.
"Changfeng Zhilou" (Intestinal Wind and Hemorrhoids/Fistula) is a general term. Differentiated specifically, they differ. If ulcerated, it is called Zhilou. Constipation and dryness cause severe pain. This arises from wind-heat attacking due to overeating leading to qi stagnation, forcing the large intestine to malfunction. Thus, the classics state: "Due to overeating, sinews and vessels relax horizontally, and the intestines become hemorrhoidal." The pathogen received is Dryness (Zao Qi); the disease-causing factor is Stomach Heat (Wei Re). Stomach fire penalizes the large intestine, transforming into dry-fire, attacking the excess of dry-heat. Wind attaches to heat; this is the combination of Dampness, Heat, Wind, and Dryness. Therefore, lumps at the anal verge indicate Dampness; severe pain indicates Wind; dry and hard stool indicates the main disease combined with Fire-Heat attack. One must eliminate these four pathogens. The Lung belongs to the West, governs all qi, and its nature is descending, but it may also assist the pathogen. It is necessary to combine with qi-breaking herbs. The treatment method is then complete. Do not make pills; instead, decoct and administer as soup. Efficacy is swift and miraculous.
[Qinjiao Cangzhu Tang]
Qin Jiao (1 qian, remove sprouts) Ze Xie (2 fen) Dang Gui Wei (3 fen, soaked in wine) Cang Zhu (7 fen)
Fang Feng (0.5 qian) Da Huang (a small amount; although stool is dry, do not add too much) Bing Lang (1 fen, powdered)
Tao Ren (soaked in hot water, peeled, 1 qian, ground) Zao Jiao Ren (charred to retain property, peeled, 1 qian, powdered) Huang Bo (peeled, 4 fen, washed in wine; if the anal verge feels heavy, indicating dampness excess, increase according to judgment; if weather is very hot, increase according to judgment; if the patient is irritable and prefers cold, increase according to judgment).
Grind the above ten herbs, mix evenly, and prepare as one dose. Add five cups of water, decoct to one cup and two fen, strain, add Tao Ren, Bing Lang, and Zao Jiao Ren. Return to fire, decoct to one cup, strain.
Take warm on an empty stomach. Wait a moment, then eat a good meal to settle the stomach and prevent damage to gastric qi. During medication, avoid raw, cold, hard foods, alcohol, wheat products, vegetables, and strong spices. Violating these renders the medicine ineffective. If white pus is present, add five white Hibiscus flowers (removing calyx), finely cut, and 0.5 qian Qing Pi (not removing the white pith), decoct together with the main herbs. Add 0.5 qian Mu Xiang powder, mix with Zao Jiao powder, and add. Follow the above method. For severe cases, another dose will cure.
[Ruxiang Wan] Treats various hemorrhoids with bleeding, flesh growth at the anal verge, nodules with swelling and pain, ulcers with itching and pain, difficult defecation, and rectal prolapse.
Also treats Changfeng bleeding, regardless of new or chronic cases, and roots in the zang-fu organs; all can be treated.
Zhi Qiao (remove albedo, stir-bran fried) Mu Li (fire-roasted) Bi Cheng Qie Da Huang (steamed and dried)
He Shi (fried) Yuan Qing (remove head, wings, feet, stir-fried with glutinous rice until rice turns yellow, each 0.5 liang) Ruxiang (ground)
Bai Ding Xiang (ground, each 1 fen).
Grind into powder, bind with millet paste into pills sized like paulownia seeds. Take 10-15 pills per dose. For Changfeng, deliver with aged tea clear liquid; for various hemorrhoids, deliver with Scallion White Decoction; for other conditions, deliver with Iron Filings Decoction. Take before meals.
[Shenyng Heiyu Gao] Treats men and women with chronic or acute Changfeng and Zhilou, unbearable anal pain. Effective after 3-4 doses.
Initially, the disease causes itching or pain, with hard nodules around the anus. This is hemorrhoid; upon rupture, it becomes fistula. Bleeding alone is Changfeng. All result from alcohol, sexual indulgence, wind-dampness, and excessive consumption of five pungent spices. People often use external surgical applications, unaware that internal medicine exists for intestinal diseases. Without removing the root, the disease persists. This medicine has miraculous effects.
Zhu Xuan Ti (20 pieces) Ci Pi (1 liang 1 qian) Niu Jiao Sai (chopped and stir-fried, 1 liang 2 qian) Bai Zong (8 qian)
Luan Fa (washed, dried) Huai Jiao (each 6 qian) Ku Lian Gen (5 qian) Lei Wan
Zhi Ma (Sesame) (each 4 qian).
Chop the above nine herbs, char in a porcelain jar until carbonized, grind to fine powder. Add 2 liang Ruxiang and 8 qian Shexiang, grind evenly. Bind with wine-flour paste into pills sized like paulownia seeds. Take 8 pills per dose. Chew one walnut first, swallow with warm wine. Take twice daily on an empty stomach (morning and evening). Severe cases take three times daily. Strictly avoid other medicines. Root can be removed within three days.
[Diao Chang Wan] Treats chronic and acute hemorrhoids, anal swelling and pain, ulcers with itching, occasional pus and blood, Changfeng bleeding, and anal prolapse.
Suitable for all such conditions.
Luan Fa (washed, charred) Ci Pi (two pieces, chopped, charred in jar) Ji Guan Hua (chopped, lightly stir-fried to retain property)
Bai Fan (slightly burnt) Lv Fan (burnt) Hu Tao (kernel, 15 liang, charred in jar)
Zhi Qiao (remove albedo, stir-bran fried) Fu Zi (remove skin and umbilicus, raw) Bai Fu Zi (raw) He Zi (roasted, remove seed)
Ban Xia Tian Nan Xing (each 2 liang).
Grind to fine powder, bind with vinegar-boiled flour paste into pills sized like paulownia seeds. Take 20 pills per dose, on an empty stomach and before bed, with warm wine. For chronic cases unhealed for years, see results in ten days. Long-term use removes the root completely. For mild Changfeng within 1-2 years, ten doses cure permanently without recurrence.
[Linxie Yao] Treats Changfeng and Zhilou that have persisted without healing, continuous suppuration from wounds, parasites in wounds, and unceasing itching/pain. Suitable for washing.
Zhi Qiao (stir-bran fried) Weiling Xian (clean soil) Jingjie Sui (clean soil) Ruxiang (each 1 liang)
Fengyan Cao (2 liang) Xi Xin (remove sprouts, 2.5 qian).
Grind the above six herbs into coarse powder. Use 3 liang per dose, decoct in one large bowl of water to one sheng. Strain. Wash the affected area while slightly warm. Rewarm when cool, wash again once. Do not reuse. After washing, dry with soft cloth and apply topical medicine. If the wound is ruptured, no topical application is needed, only washing.
[Linxie Diyu San] Treats anal itching, pain, or swelling.
Di Yu Shuo Luo Jingjie Ku Shen
She Chuang Zi (equal parts).
Grind into coarse powder. Use one spoon per dose, decoct in one bowl of water for 2-3 boils. Strain. Wash the affected area at a comfortable temperature in a draft-free place.

Ji Guan Hua Fengyan Cao (each 1 liang).
Grind into coarse powder. Use half liang of coarse powder per dose, decoct in half a bowl of water for 3-5 boils. Apply hot compress/wash to the affected area.
[Jieyin Dan] Treats Changfeng, Zangdu (visceral toxin) bleeding, and various lower gastrointestinal bleeding disorders. (The following three formulas were transmitted from Huji Jiao, Medical Supervisor of various circuits, and are highly effective.)
Zhi Qiao (stir-bran fried) Huang Qi Weiling Xian Chen Pi (remove albedo)
He Shou Wu Jingjie Sui Chun Gen Bai Pi (equal parts).
Grind the above seven herbs into powder, bind with wine-paste into pills sized like paulownia seeds. Take 50-70 pills per dose. Deliver with aged rice water mixed with a little vinegar, boiled briefly, cooled to warm temperature. Take at dawn. Taking on an empty stomach is also excellent.
[Linxie Weiling Xian San] Treats Zhilou, anal itching/pain, or swelling/fullness.
