Danxi stated: There is food stagnation (stagnation caused by dietary retention), and heat stagnation (stagnation caused by internal accumulation of heat). When prolonged fever becomes severe, it leads to *zaochao* (a sensation of hunger mixed with discomfort in the stomach, neither true hunger nor pain), which is caused by fire disturbing phlegm. Erchen Tang (Two-Cured Decoction) combined with Coptidis Rhizoma (Huanglian, 黄连) stir-fried with ginger juice and Gardeniae Fructus (Zhizi, 栀子) stir-fried are essential medicines, with additional Qingdai (Indigo Naturalis).
If the pulse at both cun positions is wiry and slippery, there is retained fluid in the chest. If the cun pulse extends horizontally, there is horizontal accumulation above the diaphragm. If the right guan pulse is severely wiry and rapid, liver wood is overacting on spleen earth, which may develop into fanwei (regurgitation or vomiting of undigested food). If the guan pulse is floating and choppy, it indicates stagnation.
In summary, phlegm is stirred by fire, causing a sensation of heartburn resembling hunger during illness, but it is not actual hunger. Obese individuals have more phlegm and should take increased amounts of Pinelliae Rhizoma (Banxia, 半夏) and Atractylodis Macrocephalae Rhizoma (Cangzhu, 苍术); thin individuals have more heat and should take increased amounts of Coptidis Rhizoma (Huanglian, 黄连) stir-fried with ginger and Gardeniae Fructus (Zhizi, 栀子).
For damp-phlegm and qi stagnation leading to loss of appetite, Sanbu Wan (Three-Tonifying Pill) combined with Cangzhu (Atractylodis Macrocephalae Rhizoma), Xiangfu (Cyperi Rhizoma), Banxia (Pinelliae Rhizoma), etc., is used.
For food stagnation accompanied by heat, Erchen Tang (Two-Cured Decoction) combined with Shenqu (Massa Medicata Fermentata, 神曲), Xiangfu (Cyperi Rhizoma), and Huanglian (Coptidis Rhizoma) stir-fried is used.
When treating zaochao (*zaochao*) with excessive phlegm-resolving and fire-clearing medicines yields no effect, one must employ the method of following the disease's false nature (treating according to the apparent nature of the pathology). Add Hongdoukou (Alpiniae Katsumadai Semen, 红豆蔻) and Huanglian (Coptidis Rhizoma) to the phlegm-resolving and fire-clearing formula, stir-frying them with extra ginger juice.
Sansheng Wan (Three-Saints Pill) is highly effective for treating zaochao (*zaochao*).
Baizhu (Atractylodis Macrocephalae Rhizoma, 白术) (stir-fried, 4 liang), Huanglian (Coptidis Rhizoma, 黄连) (stir-fried, 5 qian), Juhong (Citri Reticulatae Pericarpium Rubrum, 橘红) (1 liang).
