[Symptoms]
The condition known as "gan chuang" (a chronic ulcerative lesion occurring in the lower body) is caused by deficiency-cold of the Kidney, with wind-pathogenic toxic qi invading externally from beside the Zusanli (ST36) acupoint, pouring into the side of the Yinjiao (RN7) acupoint, causing wind-heat toxic qi to flow and accumulate in both feet. This results in sores, swelling, and ulceration, accompanied by pain, foul odor, and difficulty walking. Sores arising on the tibia are particularly severe because there is little muscle and thin skin above the bone, making them difficult to heal. In some cases, they persist for years without cessation, with open sore mouths, rotting skin exposing muscle, and a terrifyingly foul odor.
[Treatment Principles]
Treatment should first involve removing parasites, followed by applying external plasters. Internal administration of medicines such as La Fan Wan (Wax and Alum Pill) is also appropriate. The patient must sit upright with legs elevated and avoid excessive walking, so that complete recovery can be achieved.
[Formulas]
Huang Bai San (Phellodendron and Calomel Powder): Treats gan chuang damp-toxin and heat sores all over the body.
Huang Bo (Phellodendri Cortex, 1 liang) Qing Fen (Calomel, 3 qian)
Grind the above ingredients into powder. Mix with pig bile for topical application; if the area is moist, sprinkle the dry powder directly.
San Xiang Gao (Three Fragrances Ointment, transmitted by Zhao Gusong): Treats gan chuang anywhere on the body that has ulcerated down to the bone, accompanied by pain.
Ru Xiang (Frankincense, 2 qian) Song Xiang (Pine Rosin, 3 qian)
Grind the above ingredients into fine powder. Mix with pure sesame oil. Prick holes densely in a leaf from the Zizania latifolia reed (Jiao Ruo Ye). Spread the medicine on the leaf, apply it to the affected area with the medicine centered, cover with a whole leaf, bind securely with cloth, and pain will cease immediately.
Jia Zhi Gao (Paper-Clamped Plaster, transmitted by Zhang Huishan): Treats stubborn gan chuang.
Song Xiang (Pine Rosin) Dan Zhu (Red Lead) Bi Ma Zi (Castor Bean Seed, shelled, equal parts)
Grind the above ingredients into powder. Mix with sesame oil. Spread the medicine between layers of oil-proof paper and apply to the affected area.
Shicun Liu Dayin Gao (Ointment of Grand Administrator Liu from Shicun)
4 liang of Sesame Oil (Sesamum indicum Oleum), fried in a copper pot until oil is extracted. Add 3 stalks of White Onion Bulb (Allii fistulosi Bulbus) and boil until black, then remove. Add 1 liang of Sichuan Pepper (Zanthoxyli Pericarpium, eyes removed) and boil until black, then filter out the residue. Add 1 liang of Indigo Naturalis (Qing Dai) powder. Stir together with ten Huai Zhi (Stigma Sophorae Flavescentis twigs, each one chi long). Burn until one section is charred, cut off one section, and continue until finished. Then add 1 liang of Borneolum Syntheticum (Bai Fan - note: typically Bai Fan is Alum, but text says Bai Fan which usually refers to Alum in this context, though Borneol is Bing Pian. Given "Bai Fan" literally means White Alum/Alum, I will translate as Alum) and 5 qian of Beeswax (La Xiang). Boil for a long time, pour into a bowl to solidify into an ointment. Each time, take one spoonful of ointment sandwiched between two sheets of oil-paper. Puncture densely with a silver needle. First wash the sore mouth with strong tea. Apply the ointment on top. When changing repeatedly, adhere to this method. Pain will stop; use once or twice for immediate effect. For long-standing cases, no more than five to seven applications are needed. Store the remaining ointment; prolonged use yields better effects.
Another Prescription (transmitted by Liu Chuanzhou)
1 liang of Sesame Oil (Sesamum indicum Oleum), fried in an iron spoon. Add 5 qian of Beeswax (La Xiang) and melt. Take 3 qian of Cuprum Acetatum (Tong Lv) ground into extremely fine powder. Scrape the powder using a copper coin and slowly add it to the iron spoon, adding it in over fifty separate portions. Place the previous copper and iron materials aside in a bowl. Pour the medicine into the bowl and let it cool and solidify. Wash the sore clean. Use a piece of felt matching the size of the sore. Spread the medicine onto the felt. Frequently apply to the affected area. Change once a day, ensuring it is washed clean regularly.
