Yang Pu'an, a Kàogōng (Director of Works), presented with dryness of the mouth and tongue, and frequent urination. This was due to dryness of bladder Yang and deficiency of Yin. Initially, Zishen Wan (Kidney Nourishing Pill) was used to tonify Yin, after which urination normalized; subsequently, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) and Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) were employed to tonify the Spleen and Kidney, thereby stabilizing the condition. If there is excessive sweating with scanty urine, or if the body cannot tolerate cold, this indicates Qi deficiency of the Lung and Spleen.
Yang Wenkui, a Confucian scholar, developed edema in both legs, chest and abdominal distension, and scanty urine following dysentery. Previous treatment with diuretic agents resulted in generalized edema accompanied by asthma. I stated: Edema in both legs is due to sinking of Spleen Qi; chest and abdominal distension is due to Spleen deficiency causing stagnation; scanty urine is because the Lung Metal fails to generate Kidney Water; generalized edema and asthma are because the Spleen Earth fails to generate Lung Metal. Therefore, Buzhong Yiqi Tang with Fuzi (Aconite Lateral Root) was administered, resulting in recovery.
Gu Rongfu, a Dayin (Magistrate), experienced itching at the tailbone area, red and painful urination, and a surging, rapid pulse at the left Chi position. This belonged to deficient heat in the Kidney channel, and the method should have been nourishment. He did not believe this and took Huangbai (Phellodendron Bark) and Zhimu (Anemarrhena Rhizome) for over a year, leading to swelling and pain of the high bone, dribbling urination, and unmanageably surging and rapid pulses in both the Lung and Kidney channels. I said: Both the mother and child organs are ruined; nothing can be done. He indeed died.
My nephew, Ling Yunhan, aged sixteen, suffered from thirst and fever in the summer of the Gengzi year, expectoration, dry lips, and scabies all over his body, especially on the two legs, which were dark-colored and itchy. Symptoms intensified in the late afternoon. In mid-winter, he developed sores on his legs, with a surging and rapid Chi pulse. I said: Scabies belongs to Kidney Chancre (Shen Gan); sores belong to Bone Abscess (Gu Ju); both are deficiency syndromes of the Kidney channel. After acupuncture, pus was discharged with a turbid odor. I predicted that during the peak of fire, he would inevitably develop Tuberculosis (Zai Zheng). Thus, Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) and Shiquan Dabu Tang (Ten-Agent Great Tonifying Decoction) were used. Within ten days, all symptoms resolved. However, tuberculosis symptoms appeared, and he recovered again using the previous medications. Upon marrying in winter, his condition recurred in spring, manifesting in summer. He consumed several jin of Dihuang Wan and 300 doses of decoctions before fully recovering.
Wang Hanzhai, a Shaozai (Vice Minister), presented with dizziness and white turbidity. I treated him with Buzhong Yiqi Tang plus Fangshi (Poria) and Banxia (Pinellia Tuber). After recovery, swelling and pain recurred. He was treated with Shanyao (Dioscorea Rhizome), Shanzhuyu (Cornus Fruit), Wuweizi (Schisandra Fruit), Bibing (Dioscorea Hypoglauca Rhizome), and Yuanzhi (Polygala Root), resulting in immediate recovery. Later, due to mental exertion, night sweats and white turbidity occurred. He was cured with Guipi Tang (Restore the Spleen Decoction) plus Wuweizi. Subsequently, intermittent vertigo occurred, which was completely cured by Bawei Wan (Eight-Ingredient Pill).
Xu Hanggu, a Yintai (Censor), suffered from fever and thirst due to labor, spontaneous urinary incontinence, and occasional urinary obstruction and dysuria. I diagnosed Liver Fire with Blood Deficiency, where Yin protrudes and cannot be controlled. In the morning, he took Buzhong Yiqi Tang plus Shanyao and Shanzhuyu; in the afternoon, he took Dihuang Wan. After more than a month, all symptoms subsided.
A man, widowed for several years and accustomed to hard work, suffered from hemoptysis, fever, and irritability upon exertion. He had taken Xijiao Dihuang Tang (Rhinoceros Horn Rehmannia Decoction), which led to asthmatic breathing and exacerbation of previous symptoms. He also experienced spermatorrhea and white turbidity, physical fatigue, poor appetite, and a surging pulse that remained forceful upon light and heavy pressure. He recovered after taking Shiquan Dabu Tang plus Maidong (Ophiopogon Tuber), Wuweizi, Shanzhuyu, and Shanyao.
A boy, aged fourteen, presented with fever and hemoptysis. I advised using Buzhong Yiqi Tang to nourish the source of transformation. He refused, using cold-natured fire-clearing medicines instead, which worsened his condition. He asked me: "I am an unmarried youth, how could I have Kidney deficiency? Ginseng tonifies Qi, why use it?" I quoted Master Danxi: "The Kidney governs storage, and the Liver governs dispersion. Both viscera contain Ministerial Fire, which connects upward to the Heart. The Heart is Sovereign Fire; when affected by external stimuli, it moves easily. When the Heart moves, Ministerial Fire rises spontaneously. Even without sexual intercourse, essence is secretly consumed." Furthermore, the "Chapter on Essence and Blood" states: "If a male engages in intercourse before his essence is full, draining his essence, the five viscera will have areas of insufficiency, leading to difficult-to-describe diseases in later years." Consequently, he recovered using Buzhong Yiqi Tang and Dihuang Wan.
Zhang Donggu, an astrologer, coughed up one or two mouthfuls of blood while walking out of the courtyard discussing fate. He claimed this had persisted for a long time and was triggered by exertion. I suspected this was Lung Qi injury due to labor, causing blood to scatter, which proved correct. I prescribed Buzhong Yiqi Tang plus Wuweizi, Maidong, Shanyao, Shudi (Prepared Rehmannia Root), Fushen (Poria with Hostwood), and Yuanzhi. He recovered after taking it. He remarked: "Every time I took Siwu Tang (Four Substance Decoction), Huanglian (Coptis Rhizome), or Zhizi (Gardenia Fruit), bleeding increased and fatigue worsened. After your week of treatment, hemoptysis stopped immediately, and my spirit returned to normal. Why?" I replied: "The Spleen controls blood, and the Lung governs Qi. Labor injured the Spleen and Lung, causing reckless blood movement. Therefore, by tonifying Spleen and Lung Qi, the blood was guided back to its source."
