，其为肝郁无疑。上年夏日曾得淋症，误服六味酸甘化阴，致令其湿热稳伏久踞，故证现庞
杂无伦，治法以宣通三焦，使邪有出路，安胃能食为要。

寒热往来乎？六脉俱弦，古谓脉双弦者寒也，指中焦虚寒而言，岂补水之生熟地所可用哉！
现下寒水客气，燥金司天，而又大暑节气，与柴胡二桂枝一法。

周姓患者，十四岁。壬戌年八月十六日就诊：伏暑之邪在体内发作，又外感新凉之气。脉象右手大、左手弦，身体发热如烙铁般烫手，无汗，咳出胶结之痰，舌苔满布且色黄，病情不宜再出现泄泻。口不渴，腹部胀满，少腹疼痛，这称为阴阳并病，太阳经气相互争斗，属于难治的病症。拟先用清利上焦湿热的方法，因为气化则湿亦化。
处方：杏仁泥（三钱）、金银花（二钱）、白通草（一钱）、滑石（三钱）、芦根（二钱）、淡竹叶（一钱）、生薏苡仁（钱半）、厚朴（二钱）、大贝母（一钱）、连翘（二钱）、梨皮（二钱）。
今晚服一剂，明早服一剂。
十七日复诊：病情与前日相同。
处方：连翘（二钱）、芦根（二钱）、杏仁泥（钱半）、金银花（二钱）、薄荷（八分）、厚朴（钱半）、梨皮（钱半）、桑叶（一钱）、苦桔梗（钱半）、知母（三钱，炒制）、鲜荷叶边（一张）、滑石（三钱）、白扁豆皮（二钱）。
中午服一剂，晚上服一剂，明早服一剂。
十八日复诊：两次使用清利上焦之法，热势已减半。手心比手背热，这叫里热；舌苔红黄而浓，这是实热。应当宣散邪气，采用苦辛寒法。再分析：暑邪必夹带湿气，根据腹部疼痛胀满的症状，所以不得不暂时使用苦燥之法。
处方：杏仁泥（三钱）、小枳实（钱半）、黄连（钱半，姜汁炒制）、木通（二钱）、厚朴（钱半）、黄芩炭（一钱）、广木香（一钱）、瓜蒌仁（八分）、广陈皮炭（一钱）、小茴香炭（钱半）、炒知母（钱半）、槟榔（八分）。
十九日复诊：腹部的疼痛和胀满都减轻了，但舌苔干燥呈黄黑色，嘴唇肌肉颜色深红，呛咳且痰液粘稠。幼童阴气尚未坚固，应当给予滋养阴液、退去虚热的方法。
处方：麦冬（六钱，连心）、牡丹皮（五钱）、玄参（五钱）、沙参（三钱）、炒知母（二钱）、犀角（三钱）、蛤粉（三钱）、杏仁（三钱）、生甘草（一钱）、细生地（四钱）、石膏（四钱）。
二十日复诊：津液稍微恢复，宿便仍未排出，潮热是因为邪气返还体表所致，右脉仍然浮大，不可使用攻下法，应当保护津液，护理因火克金（肺）引起的咳嗽。
处方：焦白芍（四钱）、沙参（三钱）、牡蛎粉（钱半）、麦冬（六钱）、柏子霜（三钱）、杏仁粉（二钱）、细生地（六钱）、霍石斛（三钱）、石膏（三钱）、玄参（六钱）、犀角（一钱）。
煮取三杯药液，陆续服用。
二十一日：各种症状全部解除，仅有轻微潮热，舌质绛红、舌苔黑，说明深入血分的余热未尽除，采用育阴法。
处方：焦白芍（四钱）、天冬（钱半）、柏子霜（三钱）、大生地（五钱）、麦冬（六钱）、丹皮（三钱）、沙参（三钱）、炙甘草（二钱）、牡蛎（三钱）。
头煎取两杯，二煎取一杯，分三次服用。
二十二日：津液消亡，舌黑干刺，采用复脉法。
处方：炒白芍（六钱）、丹皮（四钱）、柏子霜（四钱）、生鳖甲（六钱）、麻仁（三钱）、麦冬（六钱）、生牡蛎（四钱）、阿胶（三钱，冲服）、大生地（六钱）。
头煎取两杯，今日服用；二煎取一杯，明早服用。
二十三日：右脉仍然数急，余邪陷入肺中，咳嗽剧烈且痰难咯出，议定用甘润兼凉肺气的方法。
处方：甜杏仁泥（三两）、桑叶（三钱）、菊花（三钱）、苦桔梗（三钱）、麦冬（一两，连心）、玉竹（三钱）、大贝母（三钱）、梨皮（三钱）、细生地（五钱）、沙参（三钱）、甘草（三钱）、丹皮（二钱）。
煎取四茶杯药液，分四次服用。
二十四日：舌黑苔退，脉仍数，仍然咳嗽，腹部微胀。
处方：杏仁粉（三钱）、象贝母（二钱）、郁金（钱半）、茯苓（三钱）、沙参（三钱）、藿梗（二钱）、生扁豆（三钱）、丹皮（三钱）、细生地（五钱）、桔梗（二钱）、麦冬（五钱）。
头煎取三杯，二煎取一杯，分四次服用。
二十五日：昨晚排出黑色粪便，潮热消退，但唇舌仍呈绛红色。热邪经过之处，阴液必然受伤，采用复脉法以恢复其阴液。
处方：沙参（三钱）、麦冬（一两）、阿胶（二钱，冲服）、炙甘草（三钱）、炒白芍（六钱）、生鳖甲（五钱）、丹皮（三钱）、玄参（三钱）、大生地（八钱）、麻仁（三钱）、生牡蛎（五钱）。
用八碗水，煮成三碗，分三次服用。二煎煮一碗，明早服用。
二十六日：又排出宿便，邪气已退八九成，只是正气阴虚，所以不想吃东西，晚间干咳无痰。
处方：大生地（八钱）、阿胶（三钱）、生白芍（五钱）、麻仁（三钱）、炙甘草（三钱）、牡蛎粉（三钱）、沙参（三钱）、麦冬（六钱）、天冬（二钱）。
外用梨汁、藕汁、荸荠汁各一黄酒杯量，隔水炖温后频频服用。
二十七日：热邪损伤津液，大便干燥，微有潮热，干咳舌赤，采用甘润法。
处方：玄参（六钱）、知母（二钱，炒制）、阿胶（二钱）、沙参（三钱）、麻仁（三钱）、细生地（五钱）、麦冬（六钱）、郁李仁（二钱）、梨汁（一杯冲服）、地栗汁（一酒杯，冲服）。
二十八日：伏邪在体内溃散，续发白色痧疹，脉象较前稍和缓，第二次观察舌苔未化，不大便。
处方：连翘（二钱，连心）、麻仁（三钱）、牛蒡子（三钱，炒研）、炒金银花（二钱）、阿胶（钱半）、沙参（三钱）、玄参（三钱）、生甘草（一钱）、大生地（五钱）、麦冬（六钱）。
服用此方后，当晚即得大便。
九月初四日：潮热再次发作，不大便，燥粪重新积聚，给予增液承气汤轻微调和。
处方：玄参（五钱）、细生地（五钱）、麦冬（五钱）、甘草（一钱）、生大黄（二钱）。
服用此方后排出黑色粪便，潮热消退，脉象平静。此后一律以养阴收功。
陈姓患者，二十八岁。左脉洪大数实，右脉阳气微弱，阴阳逆乱，伏暑症状类似疟疾，最难立即痊愈。议定引导邪气外出的方法。
处方：生鳖甲（三两）、青蒿（四钱）、桂枝（三钱）、麦冬（八钱）、焦白芍（三钱）、甘草（钱半）、沙参（三钱）、丹皮（三钱）、知母（三钱，炒制）。
服用三帖即愈。
十四日复诊：伏暑引起的寒热已愈，但不吃不饥不便，胸中痞闷，九窍不和，都属于胃病。
处方：半夏（五钱）、广陈皮（钱半）、青皮（钱半）、桂枝（钱半）、郁金（二钱）、生薏苡仁（五钱）、茯苓（五钱）、党参（三钱）。
服用三帖。
十七日复诊：久病导致真阳虚则胸部疼痛，余邪化热则口苦，正气不复则肢体疲倦。
处方：西洋参（二钱）、桂枝（三钱）、茯苓（三钱）、半夏（三钱）、黄芩炭（钱半）、焦白芍（三钱）、生姜（两片）、广陈皮炭（钱半）、炙甘草（钱半）、大枣（二枚）。
某患者，乙丑年八月二十二日就诊：不兼湿气的伏暑被误治，导致津液消亡，致使热邪不肯退去，嘴唇开裂舌头干燥，四十多天不解，咳嗽胶痰，谵语口渴。可先服用牛黄清心丸，清心包络而搜剔伏邪；汤药采用存阴退热法。
处方：细生地（三钱）、麦冬（五钱）、白芍（三钱，炒制）、甘草（一钱）、沙参（三钱）、生牡蛎（五钱）、生鳖甲（五钱）、生扁豆（三钱）。
二十四日复诊：暑邪偏于热者，误用治疗伤寒足经的药物，导致津液消亡。昨日使用存阴法，兼用芳香开窍以开通络中闭伏之邪，已见大效。现因小便赤涩短少，热虽减但未除，议定用甘苦合化阴气法。
二甲复脉汤，加黄芩（三钱）。如有谵语，牛黄丸仍继续服用。
二十六日复诊：昨日使用甘苦合化阴气法，服药后大量出凉汗，现热已除，脉象减缓，舌苔尽退，但六脉重按全无，舌仍干燥。议定遵循“热之所过，其阴必伤”的原则，使用二甲复脉汤，重用鳖甲、生甘草，共八帖。
某患者，乙丑年九月十六日就诊：夏季感受湿邪，冬季必定咳嗽。况且六脉俱弦，肝木旺盛克制脾土，脾土受克则泄泻，胃土受克则不欲饮食且欲呕，先前曾腹胀，现在转为胸痞，舌苔白滑，这是寒湿病。但脉象反而数急，思凉思酸，这是物极必反的现象，岂是浅显易见的疾病！急切宜戒除恼怒，小心谨慎为要。
处方：半夏（三钱）、旋复花（二钱）、杏仁泥（四钱）、白蔻皮（一钱）、生薏苡仁（五钱）、滑石（三钱）、郁金（二钱）、茯苓皮（五钱）、通草（一钱）。
用水五杯，煮取两杯，药渣再煮一杯，分三次服用，共两帖。
十八日复诊：脉数甚急，思凉，这是湿中生热的缘故。
处方：通草（二钱）、郁金（二钱）、滑石（六钱）、茯苓皮（六钱）、白蔻仁（钱半）、藿梗（三钱）、生薏苡仁（六钱）、半夏（四钱）、杏仁泥（六钱）、小枳实（钱半）、黄芩（二钱）。
用水八碗，煮取三茶碗，药渣再煮半碗，分四次服用，白天三次夜间一次，共两帖。
二十日复诊：伏暑必挟带火与湿，不能单顾一边。服药后反而觉得不快，是因为体质虚弱久病，不耐受开泄之法的缘故。想喝凉水的是火邪，得水则停留的是湿邪。
处方：生石膏（六钱）、半夏（三钱）、杏仁泥（六钱）、炒知母（钱半）、蔻仁（一钱）、黄芩（一钱）。
煮取三杯，分三次服用，共两帖。
二十二日复诊：去掉蔻仁，加入：
通草（钱半）、石膏（四钱）、滑石（四钱）、知母（五分）、藿梗（三钱）。
二十七日复诊：饮邪停留在右胁，不能平卧，格拒心火，不能下通于肾，所以咽喉干燥。
处方：杏仁粉（三钱）、苏子（三钱，去油）、小枳实（三钱）、香附（三钱）、广陈皮（二钱）、旋复花（三钱）、半夏（五钱）、茯苓皮（三钱）、藿梗（三钱）。
十月初二日：小便不通，在前方内加入：
滑石（三钱）、通草（钱半）、生薏苡仁（三钱）。
前后共九帖。
初六日复诊：小便已通，在前方内去掉滑石、通草、薏苡仁，服用三帖。
巴姓患者，二十二岁。面目青黄，其为湿热无疑；右脉单独弦象，其为伏饮无疑；脘部痞闷胸痛，结合脉弦，其为肝郁无疑。去年夏天曾患淋症，误服六味地黄丸等酸甘化阴之品，致使湿热稳固潜伏长久占据，所以证候呈现庞杂无序，治法以宣通三焦，使邪气有出路，安胃能食为关键。
处方：生石膏（八钱）、半夏（五钱）、旋复花（三钱）、滑石（一两）、蚕砂（三钱）、香附（三钱）、生薏苡仁（五钱）、茯苓皮（五钱）、郁金（三钱）、通草（二钱）、杏仁泥（三钱）、萆薢（四钱）。
初六日复诊：其人本有饮症，又加上内暑外凉，在经之邪，似疟而未成，在腑之邪泄泻不止，恐成痢疾，急切以提引邪气外出为要。按六脉俱弦之泄泻，古时称之为木泄，即以柴胡汤为主方，何况加之寒热往来呢？六脉俱弦，古称脉双弦者为寒，指中焦虚寒而言，岂是补水用的生熟地黄所可用的吗！现下寒水客气当令，燥金司天，而又值大暑节气，采用柴胡二、桂枝一的方法。
处方：柴胡（六钱）、炙甘草（一钱）、桂枝（三钱）、黄芩（二钱）、半夏（六钱）、生姜（三钱）、焦白芍（二钱）、大枣（二钱）、藿梗（三钱）、广陈皮（二钱）、青蒿（二钱）。
寒热停止即停药。
初八日复诊：寒暑兼受，成疟则轻，成痢则重。前用柴胡二桂枝一法，现下面色青，热退，痰多而稀，舌之赤色亦淡，脉之弦劲者微细，口不渴，可知阳虚，给予桂枝柴胡各半汤，减去黄芩加入干姜。

煮取三杯，分三次服用。
初九日复诊：内暑外寒，相搏结成疟疾，大便溏泄，恐致成痢。口干但不渴，经书说自利不渴者属太阴，结合腹痛更可知道。仲景说表急急急当救表，里急急急当救里。现表里无偏急之象，议定两救之法。救表仍用柴胡桂枝各半法，因为太阳、少阳均有邪气；救里给予理中法。
处方：桂枝（四钱）、黄芩炭（二钱）、生薏苡仁（五钱）、白芍（二钱，炒制）、干姜（三钱）、炙甘草（钱半）、川椒炭（三钱）、柴胡（四钱）、良姜（二钱）、半夏（六钱）、白蔻仁（钱半）、生姜（五钱）、大枣（二个）。
初十日复诊：昨日用两救表里之法，已见小效，今日仍宗前法而减轻剂量，因为脉中阳气已有生动之机故也。不可性急，反而导致坏事。
处方：桂枝（三钱）、黄芩（钱半，炒制）、厚朴（二钱）、白芍（二钱，炒制）、干姜（二钱）、炙甘草（钱半）、川椒炭（二钱）、柴胡（三钱）、煨草果（一钱）、半夏（六钱）、生姜（五钱）、大枣（二个）。
十一日复诊：内而痰饮盘踞中焦，外而寒暑扰乱胃阳。连日已夺去成痢疾之路，一律以和中蠲饮为要。盖无形之邪，常借有形之物作为依附。

十二日复诊：杂受寒暑，再三分析，方成疟疾，寒多热少，脉沉弦，乃邪气深入，采用在两分阴阳之中偏于温法的治疗。

十四日复诊：寒热减少，胸痞严重，去掉甘味药加入辛味药，去掉大枣加入生姜。
十六日复诊：脉弦细，指尖冷，是阳气微弱不及四肢末端的缘故。兼之腹痛便溏，痰饮咳嗽，更可以知道了。
以和胃阳，温中汤，逐痰饮立法。
处方：半夏（六钱）、良姜（二钱）、杏仁（三钱）、川椒炭（三钱）、干姜（二钱）、炒广陈皮（三钱）、桂枝（三钱）、蔻仁（二钱）、生薏苡仁（五钱）、生姜（三片）。
张某，十七日复诊：伏暑酒毒，遇寒凉而发，九日不愈，脉缓而有饮邪，滞下（痢疾），身热，谵语，湿热发黄，先清湿热，开心包。
处方：茵陈（五钱）、茯苓皮（五钱）、黄连（二钱）、栀子炭（二钱）、通草（一钱）、黄柏炭（三钱）、滑石（五钱）、生薏苡仁（三钱）、黄芩（三钱）。
十八日复诊：热退，滞下已愈，黄疸未解。

Patient Zhou, 14 years old. On the 16th day of the 8th month in the Renxu year: Latent summerheat (Fushu) erupts internally, compounded by external new coolness (Xinliang). The pulse is large on the right and wiry on the left; body heat feels like a branding iron; no sweating; sticky phlegm expectoration; tongue coating is full and yellow; diarrhea should not occur again. No thirst, abdominal distension, and lower abdominal pain indicate concurrent Yin and Yang diseases (Yinyang Bingbing), with mutual contention between the two Taiyang channels, a difficult-to-treat condition. After taking, passed black stool, tidal fever subsided, pulse quiet. Thereafter, focus solely on nourishing Yin to conclude treatment.
Patient Chen, 28 years old. Left pulse is flooding, large, rapid, and excess; right pulse Yang is weak; Yin and Yang are chaotic; latent summerheat resembles malaria, hardest to cure immediately. Propose guiding pathogens outward.

Certain Patient, August 22nd in Yichou year: Latent summerheat without dampness was mis-treated, causing fluid depletion, resulting in persistent heat, cracked lips, dry tongue, unresolved for over 40 days, cough with sticky phlegm, delirium, thirst. First administer Niuhuang Qingxin Wan (Calculus Bovis Clearing Heart Pill) to clear Pericardium and search out latent pathogens; decoctions use Preserve Yin and Reduce Heat method.
Prescription: Fine Rehmannia Root (Xi Shengdi, 3 qian), Ophiopogon Tuber (Maidong, 5 qian), White Peony Root (Baishao, 3 qian, stir-fried), Licorice Root (Gancao, 1 qian), Glehnia Root (Shashen, 3 qian), Raw Oyster Shell (Sheng Muli, 5 qian), Raw Turtle Shell (Sheng Biejia, 5 qian), Raw Hyacinth Bean (Sheng Biandou, 3 qian).
24th Re-examination: Summerheat biased towards heat was mis-treated with Shaoyang foot meridian drugs for Cold Damage, causing fluid depletion. Yesterday used Preserve Yin method, combined with aromatic opening to unblock closed latent pathogens in collaterals, showing great effect. Now due to very red and short urine, heat reduced but not eliminated, propose Sweet-Bitter Combined Transforming Yin Qi method.
Erjia Fumai Tang (Two-Shelled Pulse-Recovering Decoction), add Scutellaria Root (Huangqin, 3 qian). If delirium occurs, continue Niuhuang Wan.
26th Re-examination: Yesterday used Sweet-Bitter Combined Transforming Yin Qi method, resulted in profuse cool sweat after taking; now heat eliminated, pulse slowed, tongue coating completely receded, but six pulses vanish upon heavy pressure, tongue still dry. Propose following principle "Where heat passes, Yin is injured," use Erjia Fumai Tang, heavily increase Turtle Shell and Raw Licorice Root, total 8 doses.
Certain Patient, September 16th in Yichou year: Summer injury by dampness leads to winter cough. Moreover, six pulses are wiry, Wood excess insults Earth; Spleen Earth insulted causes diarrhea, Stomach Earth insulted causes anorexia and vomiting; previously abdominal distension, now chest oppression, white slippery tongue coating, this is Cold-Damp disease.
