Shanqi (Hernia Qi) reverses upward to the heart and abdomen, causing pain that is unbearable, with spasms and stiffness preventing flexion and extension. The abdomen feels cold and heavy like a stone, accompanied by spontaneous sweating; this is caused by Cold Evil. Use Shan Zhizi (Gardeniae Fructus) 4 qian, Fuzi (Aconiti Lateralis Radix Praeparata) 1 qian, decocted in salt water for oral administration.
When Shanqi extends to the lower abdomen causing pain, use Muxiang (Aucklandiae Radix) and Chenpi (Citri Reticulatae Pericarpium), each 1 liang; Liangjiang (Alpiniae Officinarum Rhizoma), Ganjiang (Zingiberis Rhizoma), Hezi Pi (Terminaliae Chebulae Pericarpium), Chishao (Paeoniae Radix Rubra), and Zhishi (Aurantii Fructus Immaturus), each 5 qian; Cao Kou (Amomi Fructus Rotundus), Heiqiu (Bungeae Semen Preparatum), and Chuanxiong (Ligustici Chuanxiong Rhizoma), each 3 liang. Grind all into powder, and take 2 qian with warm boiled water.
Yinshan (Yin Hernia) radiating pain to the lower abdomen: use Jili (Tribuli Fructus), Fuzi, and Zhizi, each 1 liang. Grind into powder. Take 2 qian per dose, decocted in water.
When Shanqi occurs on either the left or right side, it is named Pian Shen (Oblique Kidney Hernia). Use Liangjiang and Muli (Ostreae Concha) ground into powder, mixed with Huojiu (Fire Wine) for external application.
Cold-Dampness Shanqi: Cangzhu (Atractylodis Rhizoma) 8 liang (washed with rice washing water, then stir-fried separately with Ginger Juice, Scallion Juice, Human Milk, and Green Salt, one fen each); Chi Fuling (Poria with Red Hosta Root) 2 liang; Shan Zhizi stir-fried with wine 1.5 liang; Xiaohui (Foeniculi Fructus) 1 liang; Lizhihe (Litchi Seed) 1 liang; Qingyan (Green Salt) 2 qian; Chenxiang (Aquilariae Lignum Resinatum) 3 qian. Make pills using Yuanmi paste (rice paste). Take 30 pills orally on an empty stomach with salt water.
Clinical Case:
A patient suffered from Shanghan (Cold Damage Syndrome). Sweating was limited only to below the waist, and the testicles were swollen and enlarged. This indicates Damp-Heat pouring downward into the Liver Meridian. Use Pogu Zhi (Psoraleae Fructus) 5 fen, Baizhu (Atractylodis Macrocephalae Rhizoma) 5 fen, Fuling (Poria) 1 qian, Zexie (Alismatis Rhizoma) 3 qian, Wuyu (Evodiae Fructus) 5 li. Decoct in water and administer orally.
Qi movement below the umbilicus indicates Kidney Qi flooding upward, failing to return to its root. This reflects deficiency of the Kidney and Spleen-Stomach. Use Liuwei Wan (Six-Ingredient Pill with Rehmannia) minus Zexie, plus Wuweizi (Schisandrae Chinensis Fructus), Paojiang (Prepared Ginger), Renshen (Ginseng Radix et Rhizoma), and Heche (Placenta Hominis). Removing Zexie prevents the leakage of Kidney Qi; adding Renshen and Jiang secures the Middle Jiao; adding Heche and Wuweizi assists the Kidney.
This "Qi movement" pertains to Liver Qi. Since Water fails to generate Wood, the Wood becomes dry and the Liver becomes hyperactive, causing movement. Guizhi Tang (Cinnamon Twig Decoction) with added Guizhi (Cinnamomi Ramulus) is appropriate.
Clinical Case:
A person frequently injured their Qi, presenting with Qi movement below the umbilicus. This indicates deficiency of both the Lung and Kidney. Qi rushes upward to the chest, causing apparent pain, accompanied by lower back pain. This represents depletion of Kidney Water and hyperactivity of Liver Wood. Use Fuling, Gancao (Glycyrrhizae Radix et Rhizoma), Shanyao (Dioscoreae Rhizoma), Wuweizi, Baishao (Paeoniae Radix Alba), and Pogu Zhi to tonify the Lung, support the Kidney, and calm the Liver, leading to recovery.
