Shangfeng (Wind injury after trauma) refers to wind pathogen invasion following contusions and fractures. It is appropriate to employ the method of nourishing blood and dispersing wind; Siwu Tang (Four Substances Decoction) added with Qianghuo (Notopterygii Rhizoma et Radix) and Fangfeng (Saposhnikoviae Radix) is effective.
Case Record
Consort Zhou, in late December, crushed two toes. After three months, the wound had healed flatly, but half a month later, severe pain returned in the hand, accompanied by fever and generalized swelling, dry stool, and an inability to rest her head on a pillow. This is Shangfeng; treating it as Shangsang (Cold damage) would inevitably lead to death. The prescription consisted of Qianghuo, Fangfeng, Wuyao (Linderae Radix), Dangguishen (Radix Angelicae Sinensis - root part), Shengdi (Rehmanniae Radix), Baizhi (Angelicae Dahuricae Radix), Fuling (Poria), Gancao (Glycyrrhizae Radix et Rhizoma), Banxia (Pinelliae Rhizoma), Xiangfu (Cyperi Rhizoma), and Zhiqiao (Aurantii Fructus). She recovered after taking five doses.
Yangdianfeng (Epilepsy/Schizophrenia-like symptoms attributed to wind-phlegm) results from congenital deficiency of Primordial Yin, leading to Liver Wood overacting on Spleen Earth and injuring the Heart. Erchen Tang (Two-Cured Decoction) is used to eliminate phlegm from the body; Cinnabaris (Zhusha) is added to calm Heart Fire; Shichangpu (Acori Tatarinowii Rhizoma) is used to open the Heart orifices; Mudanpi (Moutan Cortex) and Qingpi (or Qinghao/Clearing agents depending on interpretation, here interpreted as clearing liver heat) are used to pacify the Liver. Once phlegm is eliminated and Heart and Liver fires are pacified, turbid qi will not block the clear pathways to produce bleating sounds.
Datoufeng (Big Head Wind / Erysipelas of the face) is a seasonal epidemic disease. It is appropriate to use Mahuang (Ephedrae Herba), Xingren (Armeniacae Semen Amarum), Gancao (Glycyrrhizae Radix et Rhizoma), Shigao (Gypsum Fibrosum), Jingjie (Schizonepetae Spica), Fangfeng (Saposhnikoviae Radix), Jinyinhua (Lonicerae Japonicae Flos), Lianqiao (Forsythiae Fructus), Mutong (Akebiae Caulis), Shengjiang (Zingiberis Rhizoma Recens), and Congbai (Allii Fistulosi Bulbus), decocted in water and taken hot.
Wei syndrome (Flaccidity syndrome) has distinctions between Feng (Wind) and Wei (Flaccidity). Pain indicates Wind; absence of pain indicates Wei. Generally, pain signifies Excess, while absence of pain signifies Deficiency. When one's Qi and Blood are substantial, Wind-Cold lodges in the collaterals, causing conflict between pathogenic factors and righteous Qi, resulting in pain; when weak, Phlegm-Fire arises within the limbs, righteous Qi fails to overcome pathogens, and Wei-Bi (flaccidity and numbness) occurs. One method disperses pathogens, the other tonifies deficiency; treatment methods differ, so caution is essential! Soreness and weakness below the waist and knees mostly belong to Damp-Heat. If stool is dry and hard, Siwu Tang (Four Substances Decoction) added with Cangzhu (Atractylodis Rhizoma), Huangbai (Phellodendri Cortex), Hugu (Pantheris Os - *Note: Modern ethical/legal constraints apply, historically used*), Guiban (Testudinis Carapax et Plastrum), and Hanfangji (Stephaniae Tetrandrae Radix) may be used. For Spleen-Stomach deficiency, Sijunzi Tang (Four Gentlemen Decoction) plus the aforementioned herbs is used; for abdominal distension, Cangzhu boiled with Baizhu (Atractylodis Macrocephalae Rhizoma) can be added, or Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) modified may also be employed. For heat in the bone marrow, add Zhimu (Anemarrhenae Rhizoma), Duzhong (Eucommiae Cortex), and Niuxi (Achyranthis Bidentatae Radix); Zhimu and Duzhong tonify insufficient Spleen Yin and guide action to the bones, while Niuxia guides out pathogenic heat from the bone marrow, assisting the other herbs in achieving success.
Among the Five Elements, only Fire exists in two forms. Although the two Kidneys belong to Water, they contain one Fire (Ministerial Fire). Yang is often in excess, while Yin is often deficient; hence, it is said that one Water cannot overcome two Fires. Metal resides superiorly and fears Fire; Spleen Earth resides centrally and invades Water. If desires are unregulated, Water loses its nourishment, Fire lacks restraint, and due to its flaming nature ascending, Lung Metal is affected by Heat; if Metal is controlled by Fire, Wood lacks restraint, Liver Wood overacts on Spleen Earth, and Spleen is injured by Wood. Lung injury impairs its ability to govern the whole body; Spleen injury renders the four limbs useless. Purging the South (Heart Fire) clears Lung Metal, ensuring East (Liver Wood) is not excessive, thus preventing Spleen injury. Tonifying the North (Kidney Water) descends Heart Fire, ensuring West (Lung Metal) is not hot, thus preventing Lung Heat. If the Yangming channels are moistened, the tendons are nourished, bones are bound, and joints move freely, how could Wei syndrome occur?
Wei syndrome manifesting as inability to raise the limbs, caused by Qi and Blood deficiency and Wind-Damp invading the limbs to form Phlegm, can be treated with Chuanwu (Aconiti Radix Preparata - *Raw form specified in text, highly toxic*) in any quantity, ground raw into powder. Take 2 qian per dose, cooked with half a bowl of good japonica rice porridge, adding two spoons of white sugar, and drink. For dampness obstruction, add 2 qian of Yiyiren (Coicis Semen) powder, cooked together in porridge, which is very effective.
Wei syndrome manifesting as inability to use the limbs, with a sensation like being bound by ropes throughout the body, indicates urgent Liver Qi. Spleen is controlled by Liver Wood, Earth fails to generate Metal, and Lung is constrained by Fire pathogens. It is appropriate to tonify the Spleen and clear the Lung. Once the Lung is clear and Liver is pacified, the Spleen is free from thief pathogens and heals itself. Pill formula: Bai Zhu (Atractylodis Macrocephalae Rhizoma) 1 jin, Baixiangdou (Amomi Fructus Rotundus) 3 liang, ground together, made into pills with Mulberry juice. Take 50 pills per dose, swallowed with rice broth before noon. Avoid wheat products and alcohol.
Case Record
A man took an examination in June; dampness soaked his lower body, resulting in flaccidity and weakness of both legs below the waist. This is Spleen affected by dampness causing limb dysfunction. Decoction: Sijunzi Tang (Four Gentlemen Decoction) added with Yiyiren (Coicis Semen) and Qianshi (Nelumbinis Semen). Pills: Bai Zhu (Atractylodis Macrocephalae Rhizoma) 8 liang, Fuling (Poria) 2 liang, Yuanmi (ordinary rice) 0.5 sheng, steamed inside a pig stomach until cooked, then pounded into pills, coated with Chenxiang (Aquilariae Lignum Resinatum) powder 3 qian. Take 60-70 pills with warm water.
A woman, startled by a fire outbreak, experienced rising phlegm, generalized pain, flaccidity and immobility of the left upper and lower limbs, intermittent chest pain and trembling, lower back pain, dry mouth, dizziness, diarrhea, and limb weakness. Prescription: Bai Zhu (Atractylodis Macrocephalae Rhizoma), Baifuling (Poria), Niuxi (Achyranthis Bidentatae Radix), Chuanshanxi (Dioscoreae Hypoglaucae Rhizoma), Duzhong (Eucommiae Cortex stir-fried with ginger juice) each 1 qian; Dangguishen (Radix Angelicae Sinensis - root part), Gancao (Glycyrrhizae Radix et Rhizoma) each 0.5 qian; Qinjiao (Gentianae Macrophyllae Radix) 0.7 qian. Decocted with Shengjiang (Zingiberis Rhizoma Recens) and Dazao (Jujubae Fructus) until cured.
