论曰伤寒口舌生疮，此脾受热，熏于口舌，又或因冬时发汗吐利后，重亡津液，表里俱
虚，热势不已，毒瓦斯攻于上焦，致口中糜烂生疮也。
治伤寒口舌疮赤烂，升麻汤方
升麻（一两） 麦门冬（去心焙三两） 牡丹皮 甘草（炙锉各半两）
上四味，粗捣筛，每服五钱匕，水一盏半，竹叶三七片，枣二枚劈破，煎至八分，去滓
食后温服。
治伤寒少阴病，咽中生疮，语声不出，苦酒汤方
半夏（汤洗七遍切十四枚） 鸡子（一枚去黄留白入苦酒在内）
上二味，纳半夏于苦酒内，以鸡子壳，置剪刀环中，安火上，煮二沸去滓，少少含咽。
治伤寒后，下冷上热，口舌生疮，大青煎方
大青 升麻 射干（去毛） 苦竹叶 山栀子仁（各一两） 黄叶（去粗皮蜜炙半两） 玄
参（坚者三分） 蓄薇根（二两） 生地黄汁 白蜜（各半斤）
上一十味，将八味锉如麻豆大，用水五升，煎至一升，去滓下蜜地黄汁，搅匀再煎如稠
饧，以净器盛，每服半匙，含化咽津，不拘时。
治伤寒后心脏热，舌裂口生疮，黄柏汤方
黄柏（去粗皮蜜炙） 大青 龙胆 玄参（坚者） 生干地黄（各半两） 升麻（一两）
射干（三分去毛）
上七味，粗捣筛，每服五钱匕，水一盏半，竹叶三七片，煎至八分，去滓入蜜一合，搅
匀食后含化咽津。
治伤寒热病后，余热上冲，口舌生疮，羚羊角汤方
羚羊角（镑） 射干（去毛） 麦门冬（去心焙） 芦根（各一两） 升麻 芍药（各
三分） 木通（锉一两半）

上八味，粗捣筛，每服五钱匕，水一盏半，生姜一枣大拍碎，豉一百粒，葱白五寸，煎
至一盏，去滓食后温服。
治伤寒后口舌生疮，黄连汤方

上七味，粗捣筛，每服五钱匕，水一盏半，葱白五寸切，豉一百粒，煎至八分，去滓食
后温服。
治伤寒后心脏虚热，满口生疮，犀角汤方
犀角（镑） 玄参（各一两） 胡黄连 黄芩 甘草（炙锉各半两）
上四味，粗捣，每服五钱匕，水一盏半，竹叶三七片，煎至半盏，去滓食后温服。
治伤寒后脾胃热壅，唇口常有疮，蒺藜散方
蒺藜子（炒去角） 白扁豆（炒各一两）
上二味，捣罗为散，每服一钱匕，茶点不拘时。
治伤寒后心热，口疮久不瘥，黄柏煎方
黄柏（一两）
上一味，捣罗为末，入蜜三两和匀，慢火煎如稀饧，每取少许含化，良久吐涎，日三五
度，不拘时，咽津亦得，觉胸中似有疮者，即用蜜酒调下二钱匕。
治伤寒心热，口舌生疮，地黄煎方
生地黄汁（三合） 蜜（五合）
上二味，搅匀，慢火煎如稠饧，每服半匙含化，徐徐咽津，不拘时。
论述说：伤寒病导致口舌生疮，这是因为脾脏受热，热气熏蒸于口舌；又或者因为冬季时发汗、催吐、泻下后，严重耗伤津液，导致表里两虚，热势不止，毒气攻冲上焦，致使口中糜烂生疮。
治疗伤寒引起的口舌赤烂生疮，升麻汤方：
升麻（一两）、麦门冬（去心焙干三两）、牡丹皮、甘草（炙后锉碎各半两）。
以上四味药，粗捣筛过，每次服用五钱匕，用水一盏半，竹叶三十七片，枣二枚劈破，煎煮至八分满，滤去药渣，饭后温服。
治疗伤寒少阴病，咽喉中生疮，声音不出，苦酒汤方：
半夏（用汤洗七遍切十四枚）、鸡蛋（一枚去黄留白，将苦酒放入其中）。
以上两味药，将半夏放入苦酒中，用鸡蛋壳作为容器，置于剪刀环中，放在火上，煮沸两次后滤去药渣，少量含咽。
治疗伤寒后，下寒上热，口舌生疮，大青煎方：
大青、升麻、射干（去毛）、苦竹叶、山栀子仁（各一两）、黄柏（去粗皮蜜炙半两）、玄参（质地坚硬的三分）、蓄薇根（二两）、生地黄汁、白蜜（各半斤）。
以上十味药，将前八味锉成如麻豆大小，用水五升，煎煮至一升，滤去药渣，加入蜂蜜和生地黄汁，搅拌均匀再煎至稠饧状，用干净的容器盛装，每次服用半匙，含化咽津，不限时间。
治疗伤寒后心脏有热，舌裂口生疮，黄柏汤方：
黄柏（去粗皮蜜炙）、大青、龙胆、玄参（质地坚硬的）、生干地黄（各半两）、升麻（一两）、射干（三分去毛）。
以上七味药，粗捣筛过，每次服用五钱匕，用水一盏半，竹叶三十七片，煎煮至八分满，滤去药渣，加入一合蜂蜜，搅拌均匀，饭后含化咽津。

治疗伤寒后口舌生疮，黄连汤方：
黄连（去须炒一两）、大黄（锉碎炒）、大青、升麻、黄芩（去黑心）、甘草（炙后锉碎各三分）。

以上七味药，粗捣筛过，每次服用五钱匕，用水一盏半，竹叶三十七片，煎煮至八分满，滤去药渣，加入半合蜂蜜，再煎一两沸，饭后温服。
治疗伤寒后，口内生疮，小便颜色红赤，手足烦热，秦艽汤方：
秦艽（去苗土）、柴胡（去苗）、大青（各一两）、升麻、黄芩（去黑心）、甘草（炙后锉碎各三分）、虎杖（半两）。
以上七味药，粗捣筛过，每次服用五钱匕，用水一盏半，葱白五寸切碎，豆豉一百粒，煎煮至八分满，滤去药渣，饭后温服。
治疗伤寒后心脏虚热，满口生疮，犀角汤方：
犀角（镑片）、玄参（各一两）、胡黄连、黄芩、甘草（炙后锉碎各半两）。
以上四味药，粗捣，每次服用五钱匕，用水一盏半，竹叶三十七片，煎煮至半盏，滤去药渣，饭后温服。
治疗伤寒后脾胃热壅，唇口常有疮，蒺藜散方：
蒺藜子（炒去刺）、白扁豆（炒各一两）。
以上两味药，捣罗成散剂，每次服用一钱匕，茶调服，不限时间。
治疗伤寒后心热，口疮久不痊愈，黄柏煎方：
黄柏（一两）。
以上一味药，捣罗成粉末，加入三两蜂蜜调和均匀，慢火煎至稀饧状，每次取少许含化，良久吐出涎水，每日三到五次，不限时间，咽下津液也可以，若感觉胸中似有疮痛者，即用蜜酒调下二钱匕。
治疗伤寒心热，口舌生疮，地黄煎方：
生地黄汁（三合）、蜜（五合）。
以上两味药，搅拌均匀，慢火煎至稠饧状，每次服用半匙含化，徐徐咽下津液，不限时间。
Commentary states: Herpes lesions on the mouth and tongue caused by Cold Damage (Shanghan) result from Heat afflicting the Spleen, steaming upwards to the mouth and tongue. Alternatively, it may occur due to severe depletion of Body Fluids following sweating, vomiting, or purging during winter, leading to deficiency in both the exterior and interior. Unceasing Heat and toxic factors attack the Upper Jiao, causing erosion and ulceration in the mouth.
Decoction of Cimicifugae Rhizoma (Shengma Tang) for treating redness, erosion, and ulcers of the mouth and tongue in Cold Damage:
Cimicifugae Rhizoma (Shengma, 升麻) [1 liang], Ophiopogonis Radix (Maidong, 麦门冬) [3 liang, heart removed, roasted], Paeoniae Moutan Cortex (Mudanpi, 牡丹皮), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [0.5 liang each, roasted and chopped].
The above four herbs are coarsely crushed and sieved. Take 5 qianbi per dose, with 1.5 zhan of water, 37 leaves of Bambusae Folium (Zhuye, 竹叶), and 2 dates split open. Decoct until reduced to 0.8 zhan, strain, and take warm after meals.
Bitter Wine Decoction (Kujie Tang) for treating ulcers in the throat and loss of voice in Shaoyin disease of Cold Damage:
Pinelliae Rhizoma (Banxia, 半夏) [14 pieces, washed seven times in hot water and sliced], Gallus domestica Ovum (Jizi, 鸡蛋) [1 egg, yolk removed, white retained, mixed with Kujie (bitter wine)].
Place the Pinellia into the bitter wine. Use the eggshell as a vessel, place it within the ring of scissors, set over fire, boil twice, strain, and swallow slowly while holding in the mouth.
Decoction of Isatidis Folium (Daqing Jian) for treating upper Heat and lower Cold with mouth and tongue ulcers after Cold Damage:
Isatidis Folium (Daqing, 大青), Cimicifugae Rhizoma (Shengma, 升麻), Belamcandae Rhizoma (Shegan, 射干) [trichomes removed], Leaves of Bambusa vulgaris var. striata (Ku Zhuye, 苦竹叶), Gardeniae Fructus (Shanzhizi Ren, 山栀子仁) [1 liang each], Phellodendri Cortex (Huangbai, 黄柏) [0.5 liang, bark removed, roasted with honey], Scrophulariae Radix (Xuanshen, 玄参) [0.3 liang, hard texture], Polygoni Multiflori Radix (Shouwu Gen, 蓄薇根) [2 liang], Juice of Rehmanniae Radix Fresh (Shengdihuang Zhi, 生地黄汁), Apis Mellifica (Bai Mi, 白蜜) [0.5 jin each].
The first eight herbs are chopped to the size of hemp seeds. Decoct in 5 sheng of water until reduced to 1 sheng. Strain, add honey and fresh Rehmannia juice, stir well, and decoct again until thick like maltose syrup. Store in a clean vessel. Take half a spoonful per dose, dissolve in the mouth and swallow saliva, at any time.
Decoction of Phellodendri Cortex (Huangbai Tang) for treating Heart Heat with cracked tongue and oral ulcers after Cold Damage:
Phellodendri Cortex (Huangbai, 黄柏) [bark removed, roasted with honey], Isatidis Folium (Daqing, 大青), Gentianae Radix (Longdan, 龙胆), Scrophulariae Radix (Xuanshen, 玄参) [hard texture], Dried Rehmanniae Radix (Sheng Dihuang, 生干地黄) [0.5 liang each], Cimicifugae Rhizoma (Shengma, 升麻) [1 liang], Belamcandae Rhizoma (Shegan, 射干) [0.3 liang, trichomes removed].
The above seven herbs are coarsely crushed and sieved. Take 5 qianbi per dose, with 1.5 zhan of water and 37 leaves of Bambusae Folium (Zhuye, 竹叶). Decoct until reduced to 0.8 zhan, strain, add 1 he of honey, stir well, and dissolve in the mouth swallowing saliva after meals.
Decoction of Antelopis Cornu (Lingyangjiao Tang) for treating upward rushing of residual Heat with mouth and tongue ulcers after febrile disease of Cold Damage:
Antelopis Cornu (Lingyangjiao, 羚羊角) [shaved], Belamcandae Rhizoma (Shegan, 射干) [trichomes removed], Ophiopogonis Radix (Maidong, 麦门冬) [heart removed, roasted], Imperatae Rhizoma (Lugen, 芦根) [1 liang each], Cimicifugae Rhizoma (Shengma, 升麻), Paeoniae Radix Alba (Shaoyao, 芍药) [0.3 liang each], Akebiae Caulis (Mutong, 木通) [chopped 1.5 liang].
The above seven herbs are coarsely crushed and sieved. Take 5 qianbi per dose, with 1.5 zhan of water. Decoct until reduced to 0.8 zhan, strain, and take warm after meals.
Decoction of Bupleuri Radix (Chaihu Tang) for treating alternating chills and fever, bone and joint pain, and mouth and tongue ulcers after febrile disease of Cold Damage:
Bupleuri Radix (Chaihu, 柴胡) [aerial parts removed, 1 liang], Cimicifugae Rhizoma (Shengma, 升麻), Paeoniae Radix Alba (Shaoyao, 芍药), Ophiopogonis Radix (Maidong, 麦门冬) [heart removed, roasted], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Scutellariae Radix (Huangqin, 黄芩) [black heart removed, 0.3 liang each], Anemarrhenae Rhizoma (Zhimu, 知母) [roasted], Coptidis Rhizoma (Huanglian, 黄连).
The above eight herbs are coarsely crushed and sieved. Take 5 qianbi per dose, with 1.5 zhan of water, ginger root the size of a date smashed, 100 grains of Sojae Preparata (Douchi, 豆豉), and 5 cun of Allii Fistulosi Bulbus (Congbai, 葱白). Decoct until reduced to 1 zhan, strain, and take warm after meals.
Decoction of Coptidis Rhizoma (Huanglian Tang) for treating mouth and tongue ulcers after Cold Damage:
Coptidis Rhizoma (Huanglian, 黄连) [fibers removed, fried, 1 liang], Rhei Radix et Rhizoma (Dahuang, 大黄) [chopped and fried], Isatidis Folium (Daqing, 大青), Cimicifugae Rhizoma (Shengma, 升麻), Scutellariae Radix (Huangqin, 黄芩) [black heart removed], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [roasted and chopped, 0.3 liang each].
The above six herbs are coarsely crushed and sieved. Take 5 qianbi per dose, with 1.5 zhan of water. Decoct until reduced to 0.8 zhan, strain, and take warm after meals.
Decoction of Paeoniae Radix Alba (Shaoyao Tang) for treating persistent oral ulcers due to Heart Heat after Cold Damage:
Paeoniae Radix Alba (Shaoyao, 芍药), Scutellariae Radix (Huangqin, 黄芩) [black heart removed], Antelopis Cornu (Lingyangjiao, 羚羊角) [shaved], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [roasted and chopped, 1 liang each], Isatidis Folium (Daqing, 大青) [0.3 liang], Cimicifugae Rhizoma (Shengma, 升麻) [2 liang], Phellodendri Cortex (Huangbai, 黄柏) [bark removed, roasted with honey, 0.5 liang].
The above seven herbs are coarsely crushed and sieved. Take 5 qianbi per dose, with 1.5 zhan of water and 37 leaves of Bambusae Folium (Zhuye, 竹叶). Decoct until reduced to 0.8 zhan, strain, add 0.5 he of honey, boil for one or two more boils, and take warm after meals.
Decoction of Kodonopsis Radix (Qinjiao Tang) for treating intraoral ulcers, reddish urine, and restless heat in hands and feet after Cold Damage:
Kodonopsis Radix (Qinjiao, 秦艽) [aerial parts and soil removed], Bupleuri Radix (Chaihu, 柴胡) [aerial parts removed], Isatidis Folium (Daqing, 大青) [1 liang each], Cimicifugae Rhizoma (Shengma, 升麻), Scutellariae Radix (Huangqin, 黄芩) [black heart removed], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [roasted and chopped, 0.3 liang each], Polypogonis Herba (Huzhang, 虎杖) [0.5 liang].
The above seven herbs are coarsely crushed and sieved. Take 5 qianbi per dose, with 1.5 zhan of water, 5 cun of chopped Allii Fistulosi Bulbus (Congbai, 葱白), and 100 grains of Sojae Preparata (Douchi, 豆豉). Decoct until reduced to 0.8 zhan, strain, and take warm after meals.
Decoction of Rhinocerotis Cornu (Xijiao Tang) for treating deficient Heat of the Heart with full-mouth ulcers after Cold Damage:
Rhinocerotis Cornu (Xijiao, 犀角) [shaved], Scrophulariae Radix (Xuanshen, 玄参) [1 liang each], Picrorhizae Rhizoma (Huhuanglian, 胡黄连), Scutellariae Radix (Huangqin, 黄芩), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [roasted and chopped, 0.5 liang each].
The above four herbs are coarsely crushed. Take 5 qianbi per dose, with 1.5 zhan of water and 37 leaves of Bambusae Folium (Zhuye, 竹叶). Decoct until reduced to 0.5 zhan, strain, and take warm after meals.
Powder of Tribuli Fructus (Jili San) for treating frequent lip and oral ulcers due to Heat obstruction of the Spleen and Stomach after Cold Damage:
Tribuli Fructus (Jilizi, 蒺藜子) [fried, spines removed], Dolichoris Semen (Baibian Dou, 白扁豆) [fried, 1 liang each].
The above two herbs are pounded and sifted into powder. Take 1 qianbi per dose, taken with tea, at any time.
Decoction of Phellodendri Cortex (Huangbai Jian) for treating persistent oral ulcers due to Heart Heat after Cold Damage:
Phellodendri Cortex (Huangbai, 黄柏) [1 liang].
The above single herb is pounded into powder, mixed evenly with 3 liang of honey, and slowly decocted until resembling thin maltose syrup. Take a small amount to dissolve in the mouth, expectorate saliva after a while. Apply 3-5 times daily, at any time; swallowing saliva is also acceptable. If chest discomfort resembling ulcer pain is felt, take 2 qianbi mixed with honey wine.
Decoction of Rehmanniae Radix Fresh (Dihuang Jian) for treating mouth and tongue ulcers due to Heart Heat after Cold Damage:
Juice of Rehmanniae Radix Fresh (Shengdihuang Zhi, 生地黄汁) [3 he], Apis Mellifica (Mi, 蜜) [5 he].
Mix evenly and slowly decoct until thick like maltose syrup. Take half a spoonful to dissolve in the mouth, swallowing saliva slowly, at any time.
