Commentary states: Nasal hemorrhage (epistaxis) in Shanghan disease arises from the accumulation and excess of heat qi, causing blood to overflow and act recklessly. Generally, the Heart governs blood, while the Liver stores it; the Lung governs qi, and the nose serves as its passage. When the Heart and Lung are injured by pathogenic heat, blood follows the movement of qi and thus exits through the nose. Ancients stated that "excess Yang leads to epistaxis," meaning that when Yang is excessive, heat is excessive; when heat is excessive, epistaxis is likely to occur. Therefore, in the Taiyang syndrome of Shanghan, the occurrence of epistaxis indicates resolution of the condition, which refers to this principle. As for Yin syndromes, epistaxis is not appropriate because Yin syndromes inherently lack heat, so how can epistaxis occur?
