Yi Yin (Overflowing Drink) presents with hardness and pain in the hypochondrium, treated by Zhongwan (CV12).
Coldness in the waist, stiffness of the spine, flaccidity and weakness of the four limbs, irritability, cough with shortness of breath, depression causing difficulty in breathing, cold extremities with inability to raise the shoulders, and Mach Dao Xia Ying (scrofula/cervical lymphadenopathy), treated by Zhangmen (LR13).
Yi Yin obstructs the water pathways, presenting with yellow urine, lower abdominal pain, internal tension, edema, severe diarrhea (Dong Xie), and body pain, treated by Jingmen (GB25). (Another view states hip pain radiating to the back). Thirst due to fluid retention, body pain, and excessive salivation, treated by Yinbai (SP1).
Cold damaging the middle after overeating, leading to indigestion, distension of the five Zang organs, fullness and support in the heart, abdomen, chest, and hypochondrium; when the pulse is deficient, hundred diseases may arise, treated by Shangwan (CV13).
Abdominal distension with borborygmus, Qi rushing up to the chest, inability to stand for long periods, rumbling abdominal pain; if exposed to cold in winter, diarrhea occurs, pain around the umbilicus, cutting pain from wandering Qi in the intestines and stomach, indigestion, loss of appetite, body swelling (or heaviness), and tension beside the umbilicus, treated by Tianshu (ST25).
Acupoint Selection Method:
First, determine two acupoints. Have the patient stand upright. Take a thin rope (coated with wax to prevent stretching or shrinking) and place it straight along the sole of the foot. (Left foot for males, right foot for females). Align the front end of the rope with the tip of the big toe, and the back end with the center of the heel. Extend the rope backward along the heel up to the popliteal crease, then cut it. Next, have the patient untie their hair and part it to reveal the midline suture. From the fontanelle, divide equally to the back of the head. Then, have the patient sit upright. Take the previously cut rope, align one end with the tip of the nose, extend it upward along the head's midline suture to the back of the head, let it hang down, and follow the spinal column downward to the end of the rope. Mark this point on the spine with ink (the ink mark is not the moxibustion site). Take another rope, have the patient close their mouth, and place the rope across the mouth from corner to corner. Lift the center of the rope to the root of the nasal bridge, forming an arch, and cut it level with the corners of the mouth. Straighten this rope and measure horizontally from the previous ink mark on the spine, ensuring it is level (not high or low). (Fold the rope in half and mark the center with ink; unfold and measure horizontally so that the ink mark on the rope aligns exactly with the ink mark on the spine; ensure both ends are level). Mark the two ends of the rope with cinnabar (these cinnabar marks indicate the moxibustion points). These are the first two points determined.
Second, determine two more points. Have the patient sit upright and slightly retract the neck. Take a rope, wrap it around the neck hanging forward doubly, aligning with Jiwei (CV15). (Jiwei is the xiphoid process; if absent, measure one cun below the bifurcation of the costal margin). Cut the rope doubly. Reverse the ends towards the back of the neck. Find the center of the rope and align it with the laryngeal prominence. Let the two ends hang doubly along the neck, following the spine downward until the ends meet. Mark this point on the spine with ink (not the moxibustion site). Take another rope, have the patient close their mouth, measure horizontally across the mouth corners, and cut. Return to the ink mark on the spine and measure horizontally as before. Mark the two ends of the rope with cinnabar (these are the moxibustion points). These are the second two points. In total, there are four points, which should be moxibusted simultaneously. Moxibustion seven Zhuang per point daily, accumulating to twenty-one Zhuang (3x7), continuing up to one hundred Zhuang, or ideally one hundred fifty Zhuang. When the moxibustion sore is about to heal, apply the subsequent method to moxibust two more points.
Third, determine two more points. Use the rope used for measuring the mouth corners in the second step. On the ink mark where the double rope ends met in the second step, mark directly down the spine. Align the center of the rope with this ink mark. Mark the top and bottom ends of the rope with cinnabar to identify the two points (these are the moxibustion points). These are the third two points, known as "Si Hua Jiu" (Four Flowers Moxibustion). Moxibustate each point one hundred Zhuang. The three steps yield six points in total. For each measurement, ensure proper distance from the sun. After measuring, begin moxibustion. Mugwort collected on the third day of the third lunar month is best. Within one hundred days after recovery, avoid improper diet and sexual activity, remain calm, and rest quietly. If symptoms persist after one hundred days, re-moxibustate the original points.
The manifestations of Gu Zheng (Bone Steaming) can be differentiated into twenty-two types, all treated according to the above methods:
1. Bao Zheng (Bladder Steaming): Red and yellow urine.
2. Yu Fang Zheng (Genital Steaming): Nocturnal emission and spermatorrhea in men; irregular or prolonged menstruation in women.
3. Nao Zheng (Brain Steaming): Dizziness and heat sensation.
4. Sui Zheng (Marrow Steaming): Sensation of boiling heat in the marrow.
5. Gu Zheng (Bone Steaming): Blackening of teeth.
6. Jin Zheng (Tendon Steaming): Dry and brittle nails.
7. Xue Zheng (Blood Steaming): Dry and brittle hair.
8. Mai Zheng (Vessel Steaming): Irregular tension and relaxation of vessels.
9. Gan Zheng (Liver Steaming): Occasional blurred vision.
10. Xin Zheng (Heart Steaming): Dry tongue, oral ulcers, occasional chest fullness.
11. Pi Zheng (Spleen Steaming): Dry and cracked lips, oral ulcers.
12. Fei Zheng (Lung Steaming): Dry mouth, oral ulcers.
13. Shen Zheng (Kidney Steaming): Dry and brittle ears.
14. Pang Guang Zheng (Bladder Steaming): Brittle right ear.
15. Dan Zheng (Gallbladder Steaming): Loss of vision.
16. Wei Zheng (Stomach Steaming): Pain under the tongue.
17. Xiao Chang Zheng (Small Intestine Steaming): Loss of vision.
18. Da Chang Zheng (Large Intestine Steaming): Pain in the right nostril.
19. San Jiao Zheng (Triple Burner Steaming): Alternating chills and fever.
20. Rou Zheng (Flesh Steaming): Others feel hot, but the patient feels cold.
21. Pi Zheng (Skin Steaming): Appearance of gooseflesh-like bumps.
22. Qi Zheng (Qi Steaming): Restlessness and heat throughout the body, inability to find comfort.
Measurement Method using Cun:
The size of acupoints varies with the patient's body size. Measure the middle segment of the middle finger (left for males, right for females); the distance between the two transverse creases constitutes one Cun.
Size of Moxa Cone:
The base of the moxa cone must be at least three Fen thick. If less than three Fen, it may not cover the acupoint adequately, preventing the fire Qi from circulating through the meridians, thus failing to expel pathogenic Qi and attract healthy Qi. Even for children, use the middle finger measurement as the standard.
Fire Method:
Huangdi stated: Wood fires from pine, cypress, persimmon, mulberry, jujube, elm, willow, and bamboo harm the muscles and blood when used for moxibustion; they must be avoided. To obtain fire, strike flint, or focus sunlight using crystal; solar fire is optimal. On cloudy days, firewood from locust trees is also good. In emergencies, candles, oil lamps, or igniting mugwort with existing fire are acceptable.
Acupoints for Chuan Shi Fu Lian (Infectious Consumption), Gu Zheng (Bone Steaming), Pi Ku (Masses), etc.:
For Chuan Shi Fu Lian, Gu Zheng, Pi Ku, Gui Qi (Ghost Qi), aversion to cold resembling malaria, moxibustate one point above Dazhui (GV14), and two points near the lower sides of Dazhui, aligned with the vertebral interspaces, 1.5 Cun apart. Also moxibustate two points under the hypochondrium named Zhangmen (LR13). Also moxibustate two points on the spine opposite the heart, 1 Cun lateral to the midline. Total seven points. Moxibustate daily at noon, seven Zhuang per point. Gradual improvement occurs at one hundred Zhuang; disease resolves at five hundred Zhuang.
For Gu Zheng and Pi Ku, moxibustate Jianjing (GB21) bilaterally. If the face appears flushed red, moxibustation brings quick relief. To locate Jianjing: sitting, press from the clavicular notch upwards along the tendon to the level of the middle finger.
Shanglian (Upper Frontier) points: Three Cun below the knee laterally. One view says three Cun below Zusanli (ST36).
Xialian (Lower Frontier) points: Three Cun below Shanglian.
On the spine opposite the heart: Stand upright, place an object on the ground marking the heart center, measure back up the spine to mark the point. Total seven points, moxibustate as before.
For Gu Zheng and Pi Ku: Have the patient stand upright on a board. Use a staff to measure vertically from the umbilicus and mark. Measure back to the middle of the spine and mark. Have the patient close their mouth, place an object horizontally across the mouth corners, fold it in half, and mark the fold point for moxibustation. Also moxibustate two points in the intercostal space between the nipples. Total six points, moxibustate as before. Measurements use the flexed middle segment of the ring finger (left for males, right for females) as one Cun. Another method: measure from the proximal end to the tip of the index finger (left for males, right for females). Apply this measurement horizontally on the dorsum of the foot at the shoe-tying crease, on the tibial surface, to locate the point.
Acupoints for Chuan Shi, Nightmare Dreams, etc.:
Shangqiu (SP5): Two points, in the depression slightly anterior to the medial malleolus. Moxibustate seven Zhuang until cured.
Lidui (ST45): Two points, at the tip of the second toe,距 the nail corner width of a leek leaf. Moxibustate one Zhuang for excellent results.
Erjian (LI2): Two points, in the depression on the medial side of the proximal phalanx of the index finger. Moxibustate three Zhuang.
Total six points. Shangqiu treats hypersomnia; Lidui treats lethargy and laziness; Zhangmen treats Ben Tun Qi (Qi surging upward) and distension; treats Wu Lao Qi Shang (Five Exhaustions and Seven Injuries), mountain miasma malaria, heavy back and vexation, heart pain, Zhu Wu Qi Lei (emaciation due to adverse Qi), food failing to nourish muscles, alternating chills and fever, head and neck stiffness, dark and yellow complexion, mental fatigue, chronic wasting, accumulated masses, Ghost Qi, Chuan Shi, Gu Zheng, etc.
Weishu (BL21): Two points, 1.5 Cun lateral to the spinous process of the 12th thoracic vertebra. Moxibustate seven Zhuang daily, or up to one hundred Zhuang, depending on severity.
Shenshu (BL23): Two points, 1.5 Cun lateral to the spinous process of the 14th thoracic vertebra (L2). Moxibustate forty-nine Zhuang (7x7). For deep-seated disease, moxibustate seven Zhuang daily up to one hundred Zhuang.
Zhangmen (LR13): Two points. Moxibustate seven Zhuang initially, gradually increasing to forty-nine Zhuang daily, as before.
Taichong (LR3): Two points, one Cun between the big toe and the second toe, in the depression two Cun distal to the metatarsophalangeal joint. Moxibustate five Zhuang, gradually increasing, daily as before.
Gaohuang (BL43): Two points. Have the patient sit, hunch the back, extend arms, touch knees with straight legs, align thumbs with knees. Support elbows with an object to prevent arm movement. Massage from the upper angle of the scapula to the lower end. There are four ribs and three spaces in between; moxibustate the middle space. Rotate inside the scapula, approximately one finger-width from the lateral edge of the scapula, massage the surface of the erector spinae muscle at the intercostal moxibustion site. The patient should feel traction in the chest and shoulders. Moxibustate one point inside each scapula, five hundred Zhuang each, up to one thousand Zhuang. This causes Qi to flow like water; if there is retained phlegm or old fluid, it will be expelled. This moxibustion method treats all conditions. If the patient is too weak, have them lie on their side, pull the arm forward to locate the point, or sit upright extending the arm while someone pulls it to separate the shoulder blades. Otherwise, the humerus may obscure the point, making it hard to locate. The point is near the fifth thoracic vertebra; seek it by observation.
Ganshu (BL18): Two points, 1.5 Cun lateral to the spinous process of the 6th thoracic vertebra. Moxibustate seven Zhuang daily; up to one hundred Zhuang for deep disease.
Shentang (BL44): One point, in the depression 3 Cun lateral to the spinous process of the 5th thoracic vertebra. Sit upright to locate. Moxibustate seven Zhuang, up to forty-nine Zhuang as before.
Total fourteen points among these seven names. If unable to moxibustate all, prioritize the "tight" or sensitive points: Gaohuang, Weishu, Zhangmen, Shenshu, Taichung. If possible, moxibustate in sequence, reaching one hundred Zhuang per point, which is ideal.
For measuring acupoints, regardless of fat/thin or tall/short stature, use the middle finger of the patient (left for males, right for females), measuring the distance between the two transverse creases as one Cun, known as "Tong Shen Cun" (Same Body Cun).
Moxibustation is best performed at noon. Moxibustation on an empty stomach in the morning damages Qi and causes Blood deficiency. Moxibustation after dinner in the evening makes it difficult to expel pathogenic Qi. However, for acute wind-pathogen diseases, these rules do not apply.
