San Zou Liu Zhu Fa (Scattered Flowing Injection Carbuncle)
This carbuncle arises on the back. If the pathogenic factor flows and spreads to the four limbs, it indicates that wind-heat pathogen is extremely severe. It is also known as Mu Zi Fa (Mother-Child Carbuncle), generated by wind-heat affecting the Foot-Taiyang Meridian (Bladder Meridian of Foot-Taiyang). Due to the severity of the evil, it transmits reversely to the Spleen. The Spleen governs the four limbs; thus, the toxin flows into the buttocks and arms, and can also reach the hands and feet. This condition inevitably leads to death. Urgent treatment should employ methods to disperse wind and reduce heat, so that the qi may naturally subside.
Huan Xiang Fa (Neck-Ring Carbuncle)
This carbuncle arises encircling the neck, named Huan Xiang Fa (Neck-Ring Carbuncle), also known as Luo Tou Yong (Head-Dropping Abscess). First, observe where the swelling originates. If it starts from Tian Zhu (BL10, Heavenly Pillar), an acupoint of the Foot-Taiyang Meridian involving the Du Mai (Governing Vessel), the patient will present with aversion to cold and fever, stiffness and fatigue, chest oppression, restlessness, terror, and anorexia. If the anterior neck also swells, it indicates the toxin has flowed into the Foot-Shaoyang (Gallbladder) and Foot-Yangming (Stomach) meridians, connecting to ear swelling. Treatment should be based on the meridian patterns. If the condition does not improve, death ensues.
Zuo Da Jian Fa (Left Shoulder-Draping Carbuncle)
This carbuncle arises on the shoulder and back, involving the Hand and Foot Taiyang Meridians, which are characterized by abundant blood and deficient qi. If the sore appears near Ge Shu (BL17, Diaphragm-Shu) or Hun Men (BL47, Soul Gate) acupoints without spreading, it is easy to treat; if it spreads, it is difficult to stabilize. Whenever there is a Right Da Jian Fa (Right Shoulder-Draping Carbuncle), the left side must have spreading. There are also cases of bilateral involvement, which are more severe. One must observe the auspicious or inauspicious signs and employ methods to support the interior and promote pus discharge, along with purgation. The subtlety lies in the clinician's judgment during practice.
Shen Shu Fa (Kidney-Shu Carbuncle)
This carbuncle arises beside the Shen Shu (BL23, Kidney-Shu) acupoint. It is caused by taking hot minerals/metals drugs and rich, greasy foods. Internally, it injures the renal fascia; externally, it manifests as this carbuncle. Urgent treatment requires kidney-tonifying herbs, combined internally with methods to support the interior and resolve carbuncle toxins. Applying external plasters is advisable. Strictly avoid sexual intercourse and anger. Recovery takes one hundred days. Violation of these prohibitions renders the condition incurable.
Pang Guang Shu Fa (Bladder-Shu Carbuncle)
This carbuncle arises at Pang Guang Shu (BL28, Bladder-Shu) and Zhong Lv Nei Shu (BL27, Middle Intestine Inner-Shu). These areas are characterized by abundant qi and deficient blood. In the early stage, fever and swelling occur, accompanied by pain in all joints, lethargy, and loss of consciousness. If pus can be discharged, it is treatable. If bloody fluid oozes and the wound fails to close, internal support and tonification are necessary; otherwise, recovery is difficult.
