[Cellular Carbuncle with Two Heads]
This type of sore, developing simultaneously at the upper and lower ends, is the most acute. It involves the Du Meridian (Governing Vessel) and the Foot-Taiyang Bladder Meridian. The pathogenic toxin erupts in the center of the spine, appearing small at both ends with diffuse swelling around the four sides. This is caused by a diet rich in strong flavors and greasy foods. Qi and food are interrelated; combined with Yin deficiency, it forms a cellular pattern, indicating that the number of sores exceeds fifty or seventy. The term "Lotus Seed" refers to sores with fewer heads, not exceeding ten to twenty. For any sore with a wide and expansive opening, urgent tonification and support (Tuo Bu) therapy should be applied.
[Double Eruption on Both Hypochondria]
This type of sore occurs on the hypochondrium, either on the left side, the right side, or on both sides simultaneously. These areas belong to the Qimen (LR14) and Zhangmen (LR13) acupoints of the Foot-Jueyin Liver Meridian, which is characterized as having abundant blood but scarce Qi. Alternatively, it may extend upward to the Yuanye (GB22) and Zhejin (GB23) acupoints, which belong to the Foot-Shaoyang Gallbladder Meridian, characterized as having abundant Qi but scarce blood. This condition arises from Qi deficiency and must be treated with substantial nourishment of Yin. Hot-natured agents must be avoided to prevent damage to the periosteum; caution is advised.
[Heart-Opposing Carbuncle]
This type of sore occurs on the back, positioned opposite the Heart. It belongs to the Du Meridian (Governing Vessel), with the toxin erupting at the Jinsuo (GV9) acupoint. It arises because Heart Fire is hyperactive to an extreme degree. Urgent reduction of Heart Fire is required. If pus discharges with a red-yellow color and the swelling is localized without spreading, the condition is treatable. However, if the toxin flows to other areas, the pus is thin and green-black, accompanied by extreme thirst and delirious speech, and if muscle necrosis and membrane perforation occur, the condition is untreatable.
[Lotus Seed Carbuncle]
This sore is named "Lotus Seed" because it has fewer heads compared to the Cellular Carbuncle, typically not exceeding ten to twenty, hence the name. It also occurs in the regions of the Foot-Taiyang Bladder Meridian and the Du Meridian (Governing Vessel). Treatment requires aggressively clearing Heart Fire and applying external medication to intercept the toxin, preventing it from attacking the Heart. If it causes swelling across the entire back, it is incurable. It is advisable to use fire needles three or four times. A powder prepared from Arisaema Cum Bile (Nanxing), Aconitum Carmichaelii Lateral Root Preparata (Caowu), Strychnos Nux-vomica (Miebae), and Paezia Radix (Beimu) should be mixed with rice vinegar for external application.
[Yin Back Carbuncle]
This type of sore occurs on the back. It results from imprudence in sexual activity and the erroneous use of cold-natured medicines by incompetent physicians for external application and internal ingestion. This causes blood stasis and Qi aggregation, leading to failure of dispersion, tissue decay, putrefaction, foul odor, dark black discoloration, and muscle death, ultimately resulting in fatality. Urgent external application of Yulong Gao (Jade Dragon Ointment) is required. Internally, Da Bu Tang (Great Tonifying Decoction) for interior support should be modified with added Aconiti Lateralis Radix Preparata (Fuzi), Cinnamomi Ramulus (Guizhi), and Zingiberis Rhizoma (Ganjiang) to restore Yang. This promotes the circulation of blood and the unblocking of Qi, which is the miraculous method for regenerating muscle tissue.
