(Pus discharge from carbuncle and gangrene on the back). Although warm herbs are used to restore Yang Qi, healing can only occur after the necrotic bone has completely detached. If the normal bone rots, it will result in disability; if the accessory bone detaches, the condition will stabilize.
When a dorsal carbuncle or gangrene first appears and has not yet suppurated, applying a paste of *Bauhinia purpurea* (Zijingpi) powder mixed with wine around the lesion can naturally dissipate it. Alternatively, adding *Angelica dahurica* (Baizhi) creates "Yisheng Gao" (One Victory Ointment).
Another method uses only *Paeonia lactiflora* (Chishaoyao) and *Lithocarpus glaber* (Molaxiang, Molax Hong) for internal resolution, taken or applied with wine, called "Sansheng Gao" (Three Victories Ointment).
For treating chronic injuries, add powders of *Aconitum carmichaelii* processed (Nanxing) and *Aconitum kusnezoffii* (Caowu), mixing them equally with this formula. Apply with hot wine; the effect is miraculous.
Biao Ju (Acute Carbuncle) is caused by excessive Heart Fire and Heat Toxin manifesting as pain in the Five Centers (both palms, both soles, and the chest). The appearance resembles herpes but with dark red blood. Although the external manifestation is small, the internal heat toxin is severe. If the toxin is located between the Heart and Diaphragm, it is extremely difficult to treat; if it is in the limbs or palms/soles, it is treatable. However, treatment must be initiated early. Delay allows the toxin to penetrate the Heart-Diaphragm area, causing muscle necrosis, at which point even a divine physician cannot save the patient. For lesions on the palm, apply Hongbao Dan externally to intercept the congested blood, and administer Chonghe Gao internally to retain the head and conclude treatment. For lesions on the sole, apply Hongbao Dan externally around the tibia and ankle bones for a width of one to two cun, and administer Chonghe Gao internally to conclude treatment.
Huiyang Yulong Gao Modification Method
Treats various Yin-type dorsal carbuncles, flowing abscesses (Liuzhu), Drum-peg Wind (Guzhui Feng), chronic injury pain, cold Bi syndrome, wind-dampness, various types of Hong Kong Foot (Charcot foot-like symptoms) with cold swelling without redness, unbearable pain, stubborn numbness in the feet, and women's cold blood wind syndromes. Since this medicine is warm in nature, it is the premier effective formula for treating various Yin syndromes.
*Aconitum kusnezoffii* (Caowu) (3 liang, stir-fried), *Arisaema erubescens* (Nanxing) (1 liang, stir-fried), *Zingiber officinale* processed (Junjiang) (2 liang, baked), *Angelica dahurica* (Xiang Baizhi), *Paeonia lactiflora* (Chishao) (1 liang, stir-fried), *Cinnamomum cassia* (Rougui) (5 qian).
Grind all into fine powder. Mix with hot wine for external application. Usage is as follows:
Human Qi and Blood circulate throughout the body, completing cycles without interruption. If one Zang or Fu organ is deficient, external pathogens such as Wind, Cold, Summer-heat, or Dampness may invade the meridians, stagnating the Blood vessels. Additionally, emotional disturbances (Seven Emotions) interacting internally, along with Phlegm-Saliva obstruction, can cause alternating chills and fever. Over time, Blood vessels become blocked, and the tunnels obstructed, leading to disease. Meridians in the hands and feet have deficiency and excess, cold and heat patterns. Heat patterns are easy to treat; Cold patterns are difficult to cure. Therefore, mediocre physicians who do not understand deficiency/excess or distinguish cold/heat are like blind men riding blind horses walking into an abyss at night—how could they not be in danger? Physicians studying this book must recognize meridian locations, differentiate cold/heat and deficiency/excess, and observe the correspondence between exterior and interior. Then, one will rarely miss. Moreover, this formula contains Junjiang and Rougui sufficient to resist cold and generate warm Blood; Caowu and Nanxing can break down malignant accumulations, dispel wind, resolve toxins, revitalize dead tissue, relieve bone pain, and dissolve masses; Chishao and Baizhi can disperse stagnant Blood, relieve pain, and promote tissue regeneration. Adding wine promotes the action of the herbs. Even in severe cold cases, recovery is assured. It is like igniting cold ashes or bringing spring to dead wood. Generally, cold syndromes cause Yin rotting of muscles, resulting in loss of sensation; if pain is felt, it often attaches to the bone. Prolonged pain invades the Bone Marrow, beyond the reach of ordinary medicines. Only this formula can dispel Yin toxins, restore Yang Qi, and stop bone pain with miraculous effect. Dosage should be adjusted flexibly as follows:
For Yin-type dorsal carbuncles with black, rotting ulcers, apply Hongbao Dan around the healthy skin surrounding the lesion, leaving the center open, then apply this ointment. After one night, Yang Qi returns, and black areas turn red. Stop when redness becomes vibrant and penetrating. Conclude treatment with Chonghe Gao. To induce suppuration, add Nanxing and Caowu powders to Chonghe Gao. If Yang Qi has returned and black turns to red, but a central spot remains black, the blood there is dead. Apply a mixture of Nuxio (Puxiao) and Alum (Mingfan) powders. Another method uses Bai Dingxiang, Sand, and Ru Xiang powders mixed with saliva, applied in a circle at the junction of black and red areas, covered with Chonghe Gao. Remove the medication the next morning; the dead black flesh falls off like being cut away. Cleanse with Licorice water before applying tissue-regenerating agents to close the wound. Ensure all black necrotic tissue is removed.
Cold Liuzhu often adheres to the bone, remaining hard and unabsorbed, causing cold bone pain and tendon contraction preventing flexion/extension. Mediocre physicians mistakenly use needles/knives, finding no pus or blood, only clear fluid like dripping roof water, or dark clotted blood. This formula is suitable for application. If the condition is slightly slower, add equal parts of Junjiang, Baizhi, Rougui, and Caowu, mixed with hot wine. This eliminates bone cold, stops pain, warms the Qi, extends tendons, softens flesh, and reduces swelling. Molaxiang (Lithocarpus) should not be omitted due to its excellent ability to break down hard swellings, but avoid excessive amounts to prevent neutralizing other herb properties.
Treating Crane-Knee Wind (Knee Osteoarthritis/Tuberculosis) with unbearable day-and-night pain, caused by Wind-Cold-Dampness and Deficiency. The *Neijing* states: "Wind, Cold, and Dampness combine to form Bi, which is pain." There are three types: 1. Knees pressing together, trembling while walking, slight swelling at the knee-tibia junction. 2. Knee-tibia junction swollen like a drum peg, inner thighs wasted, skin shrinking around the bone. 3. Upper leg swollen, lower shin cold and wasted. The foot and knees belong to the Liver. Liver deficiency allows pathogenic invasion, stagnating Blood with no return, causing upper legs to swell and heat up, while lower shins become cold and emaciated, with tendons contracting. Prolonged cases lead to muscle rot and bone decay. It is said: "Once bone decays, even gods have no remedy." If not broken and muscle not dead, apply this medicine mixed with wine to the swollen upper leg near the knee to stop bone pain and restore Yang Qi. Apply Chonghe Gao to the cold lower shin to promote Qi-Blood flow through meridians. Apply Yulong Gao to the junction of cold and heat to draw out accumulated Yin. Internally administer Zhuifeng Wan with doubled Ru Xiang to extend contracted tendons. Follow this method for universal efficacy. For patients traveling, Wujie San plus Ginger, Cinnamon, Dang Gui, Chuanwu, Niuxi, Binglang, Mugua can be made into pills/powder, taken with tea or wine freely, with miraculous results.
For men and women with chronic cold Bi and Blood Wind causing stubborn numbness and immobility, wrap this medicine in a silk bag and bind tightly around the painful area. Initially feeling like insects crawling indicates efficacy. For severe Bi, add Dingpi, Evodia (Wuzhuyu), Myrrh (Moyao), and large Chuanwu in equal parts inside the wrap. Take Zhuifeng Wan externally and internally to attack the disease from both sides.
For unbearable pain from Wind Hong Kong Foot, take Zhuifeng Wan. Externally, mix this formula with flour and ginger juice for hot compresses for immediate pain relief. Adding Ru Xiang and Moyao powders mixed with wine is optimal.
For chronic bone/tendon pain from injuries, or fractures/dislocations without bleeding that stagnate internally resembling chicken lungs attached to intercostal spaces, appearing faintly like moss. In youth, harmonious Qi-Blood prevents harm. In old age with declining strength, Wind-Cold-Dampness triggers pain. Apply this medicine with hot wine externally, and take Sousun Xuntong Wan internally for combined treatment. Weak Qi-Blood with pathology in chest/flanks/back/waist is called Degou. Untreated, it becomes Blood-Stasis Labor Disease regardless of age. Severe cases cause whole-body pain; mild cases affect one fist or half an elbow. Treat urgently. This is a root disease; pre-Labor stage is easy to treat, post-Labor is difficult. One dose is insufficient.
A simplified method uses only Nanxing and Caowu with a little Rougui to remove black rot and pus, called Xiao Yulong. This method is highly effective.
For Stone Carbuncle, apply Xiao Yulong with hot wine, surround with Hongbao Dan, and lanced after suppuration.
For Breasted Milk Carbuncle (Puerperal Mastitis), do not use cooling herbs initially, as they may congeal blood and prevent milk transformation. Add Nanxing, ginger juice, and wine to this formula for hot compresses to dissipate quickly. For rapid resolution, add Caowu powder to break cold/malignancy. If already suppurated, treat with Chonghe Gao, optionally adding Caowu and Nanxing. After rupture, determine the source: if Cold, use Chonghe Gao to conclude; if Heat, use Hongbao Gao to reduce heat and regenerate tissue, adding Ru Xiang and Moyao for pain, followed by Shenxiao Gulou San.
For retained Phlegm deviating from its pathway causing non-suppurating swellings, apply this medicine to the head of the lesion to encourage lateral drainage. Otherwise, Yuan Yang depletes into a defeat syndrome. For defeat syndromes, apply Yulong Gao to extract toxin and induce suppuration, internally taking Tongshun San plus Jiegeng, Banxia, Dang Gui, Rougui. If the lesion is red, vibrant, and hot, Chonghe Gao is better. Do not use cooling herbs. This formula extracts toxin and induces suppuration; stop once pus appears. Do not overuse. Using Xianmai Shenji (from Waikye Jingyao) is even better.
Treating Tu Yong (Vomiting Carbuncle/Internal Abscess): 9 out of 10 die. Spleen/Stomach belong to Kun Earth (Yin). Qi-Blood gathers, seeking heat and avoiding cold, thus mostly forming internal abscesses not visible externally. Occasionally slight signs appear, but if touched by cold herbs or cooling medicines, even gods cannot treat. Be cautious. Initial symptom: lower back pain, tender to touch, moving/shifting indicates Qi mass. Fixed root with slight external redness/swelling indicates internal abscess. Urgently use this formula to draw out toxin and convert to external abscess, concluding with Chonghe Gao. Internally take Tongshun San plus Honeysuckle Vine. If the abscess spontaneously externalizes, do not use this formula; use Chonghe Gao alone. Apply Yulong plaster to the head for spontaneous rupture. If pus drainage is slow, apply Hongbao Dan (3 parts) and ginger juice (7 parts) mixed with tea. Once pus is exhausted, internally take Shixuan San to tonify. Externally use Chonghe Gao. This syndrome has much Yin and little Yang, most damaging. When stabilizing, large doses of Qi-Blood tonics aid recovery. Pregnant women should use Zisu Yin to calm the fetus; avoid harsh purgatives. At delivery, fetus and pus pass together; if far from term, pus passes via stool. Early intervention can dissipate it.
Hongbao Gao (Cool Nature)
Treats all Heat-type carbuncles and toxic swellings with intense heat. Use this in conjunction with others. Its cool nature resolves Blood, breaks swellings, and relieves pain. For Yin syndromes/cracks, it aggravates pain, coagulates Blood, kills tissue, and stops bleeding from traumatic wounds, making it the primary herb for such cases. Otherwise, it serves as an assistant to the previous two ointments. Chonghe is Warm, Yulong is Hot, Hongbao is Cold. These three must be studied and applied flexibly based on clinical presentation, like military tactics (main, flank, ambush forces). Only masters of divine skill can utilize them effectively.
