Pure Yin syndrome (referring to sores of a yin-cold nature) at the initial stage is often unnoticed by the patient. The sore appears like millet grains, forming hard and stiff nodules. There is no redness or swelling, and no pain is felt; there is only slight heat and minimal sweating, requiring vigilant caution. After seven days, the body feels fatigued, the base of the sore is flat and wide, and there is a preference for drinking hot soups. The apex is not elevated, and the root is unstable; the color is dull and lacks luster, and upon putrefaction, no pus is generated. The lesion is sunken and soft, with no pus, merely an empty accumulation. The pulse is floating, scattered, large yet thin, or weak and flying. There is no appetite, and the body trembles. Even when attempting to drink soup to quench thirst, the mind is filled with panic and haste. Dead skin resembling shed armor forms on the sore, and hard knots within the pores resemble budding flowers. Abundant pus with a foul odor makes defecation difficult. Movements are frantic, and the voice is short and unsteady. If the sore turns purple-black, the complexion changes to greenish-yellow, spirit becomes confused and drowsy with snoring, speech is incoherent without response, mouth is dry, tongue is stiff, and the onset of phlegm and panting leads to death. These all belong to Pure Yin syndrome, mostly untreatable cases; among a hundred people, all may reach the realm of the dead.
Yin and Yang syndromes intersect, with life and death coexisting, and outcomes can be anticipated. Symptoms include slight heat and cold, slight redness and swelling, half-comatose and half-conscious states, and sores that are half-flat and half-elevated. The pulse is rapid but mostly powerless. Although food is consumed, digestion is poor. Swelling without ulceration indicates Spleen deficiency; ulceration without swelling indicates excessive pus. Frequent loose stools and frequent urination occur. Sweat on the upper body while the lower limbs appear scorched. Although Five Good signs are present, Seven Bad signs have not fully escaped. Thirst with a desire for tea, abdominal pain, facial edema, aversion to drinking, and high sole of the foot. Restlessness preventing sleep, mental confusion, fear of sound, and anxiety. Prescribing medicine that matches the disease is essential; conversion from Yin to Yang allows gradual regulation. This is true creation, making your life firm and secure. Encountering this condition, even with the hands of immortals, it is difficult to prevent escape from half-death and remaining life (indicating a very poor prognosis).
Heart Good: Spirit is bright and clear, speech is distinct, tongue is moist and fresh. Sore pain exists without thirst, and one sleeps peacefully and wakes up comfortable. Liver Good: Body is light and agile, irritability naturally disappears. Fingertips are red and vibrant, sitting and rising feel healthy. Spleen Good: Lips are moist, bedding and clothes smell of orchid and musk. All food tastes good, pus is thick and yellowish-white. Lung Good: Voice is resonant, breathing rhythm is long, skin is smooth and moist, bowel movements are normal. Kidney Good: Indeed crucial, like deep water where fire descends naturally. Mouth is harmonious and not thirsty, urine is diluted and prolonged.
First Bad: Confusion and dizziness, restlessness, dry tongue. Sores are mostly purple-black, muttering speech. Second Bad: Stiff waist and body, eyes looking askance at others. Sores bleed fluid, palpitations indicate Liver damage. Third Bad: Emaciation, clear and foul-smelling pus. Sores are soft and sunken, Spleen failure causes lack of pain sensation. Fourth Bad: Dry skin, hoarse voice, short breath. Excessive phlegm with urgent panting, nasal flapping indicates impending Lung death. Fifth Bad: Development of Xiao Ke (wasting-thirst), drinking followed immediately by dryness. Appearance is grim and black, retraction of the scrotum indicates Kidney pathology. Sixth Bad: Body edematous and heavy, borborygmus, frequent vomiting and hiccups. Intestinal sliding diarrhea, indicating collapse of Zang-Fu organs. Seventh Bad: Inverted and sunken sores, shape resembling skinned eels. Cold and reversed extremities, continuous discharge of turbid fluid.
Good signs belong to Fu organ diseases, with mild illness and shallow pathogenic factors. With careful daily routine and dietary restraint, recovery can occur without medication. Bad signs belong to Zang organ diseases, mostly due to Qi-Yuan deficiency, or improper sweating/purging treatments injuring Stomach Qi, or overuse of cold/cooling drugs causing pathogenic factors to become more solid. The treatment method should focus purely on tonifying Stomach Qi, with many chances for survival. Treatment should be applied according to symptoms within various formulas for treating ulcers. Additionally, after ulceration, fever, chills, thirst, palpitations, insomnia, locked jaw, red and painful head and eyes, spontaneous or night sweats, severe chills with teeth grinding, limp hands with body heat, pulse large and forceful but empty on pressure, or body heat with aversion to clothing, desire to throw oneself into water, pulse floating and large but thin on pressure, feeling cold despite thick clothing—this is extreme Qi deficiency transforming into bad syndromes. Cold extremities, abdominal pain, diarrhea with borborygmus, inability to eat, hiccups and vomiting—this is Yang Qi deficiency with invasion by Cold pathogens. Sweating without chills, or no sweating with chills, locked jaw with cold feet, opisthotonos, stiff neck—this is extreme Qi and Blood deficiency transforming into spasms. All require urgent use of Renshen (Ginseng Radix), [missing character, likely Huangqi or Baizhu], Danggui (Angelicae Sinensis Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), and Fuzi (Aconiti Lateralis Radix Praeparata) to rescue. Occasionally, some may survive; do not abandon treatment simply because the symptoms are severe.
