Meridians: The Heart opens into the tongue, and its vessels connect to the tongue. Furthermore, the Liver meridians link to the root of the tongue, and the Lung meridians link to the sides of the tongue. Therefore, the tongue is associated with these three meridians and is not exclusively governed by the Heart.
Purple Swollen Tongue: The tongue swells to fill the mouth, becoming hard and painful. This is caused by excessive fire and blood stagnation. Another view states that Spleen heat leads to swelling and fullness. Treatment involves using a clothing needle fixed to the tip of a chopstick, exposing only a small portion of the point, and pricking the tongue dozens of times to induce bleeding. Red blood indicates a mild condition, while purple blood indicates a severe one. Rinse the mouth with warm water, apply Bingpeng San (Ice Borax Powder), and orally administer Liangge San (Cooling the Diaphragm Powder) added with Niu Bang Zi (Arctii Fructus) and Jing Jie (Schizonepetae Herba). Double the dosage of Zhi Zi (Gardeniae Fructus), which is a key herb for purging fire. Alternatively, remove Xiao Ming Fan (Mirabilite) and Da Huang (Rhei Radix et Rhizoma), and use only Fan Mu Die (Strychni Semen). Scrape off the shell, slice it, and hold a small piece in the mouth to reduce swelling. Or, grind Sesame seed kernel (Ricini Semen) into a paper wick, ignite it to smoke the tongue, which will resolve upon entry. Another method states that if the tongue extends beyond one cun, apply crushed Bing Pian (Borneolum Syntheticum) externally to retract it. Moxibustion can also be applied to Shaochong (HT 9).
Phlegm Bao (Packet): Arises under the tongue, forming a swollen mass like a gourd, smooth and soft as cotton. In severe cases, it fills the mouth. It is formed by fire agitating phlegm-fluids that flow and congeal. It is suitable to use a Pi Zhen (Plum-blossom needle or specific surgical knife) to twist against the packet like drilling until it ruptures, releasing thin saliva resembling egg white, which is thick and unbroken. Wipe clean, apply Shui Tiao San (Water-Mixed Powder), and orally administer Er Chen Tang (Two-Cured Decoction) added with Huang Qin (Scutellariae Radix), Huang Lian (Coptidis Rhizoma), Bo He (Menthae Haplocalycis Herba), etc. Still avoid fried foods and strong alcohol so that recurrence is prevented.
Bingpeng San: Treats tongue swelling, Phlegm Bao, double tongue, wooden tongue, and sore throat with swelling and pain.
Ingredients: Bing Pian (Borneolum Syntheticum, 1 fen), Peng Sha (Borax, 5 qian), Chen Sha (Cinnabar, 1 qian), Ya Xiao (Natrii Sulfas, 1 qian).
Preparation: Grind into fine powder and store. If toxins arise in the throat, add ash from burned Deng Cao (Junci Medullae) for better efficacy.
She Nu (Tongue Hemorrhage): Manifests as small holes on the tongue, small as a hairpin or large as a chopstick head. Some appear black, with blood gushing out. This is caused by Heart fire flaming upward and blood heat. Sheng Ma Tang ( Cimicifugae Decoction) and Xiang Shen Wan (Aromatic Ginseng Pill) are suitable. Another view states that Liver stagnation causes blood to gush, while Liver wind causes tongue stiffness.
Sheng Ma Tang: Treats tongue hemorrhage with blood gushing out.
Ingredients: Sheng Ma (Cimicifugae Rhizoma), Xiao Ji Gen (Cirsii Japonici Radix), Qian Gen (Rubiae Radix) (each 1.5 liang), Ai Ye (Artemisiae Argyi Folium, 0.75 liang), Han Shui Shi (Halloysit Rubra, 3 liang).
Preparation: Grind into powder. Take 3 qian per dose, decoct with one zhong of water until reduced to 0.7 zhong, strain, add Sheng Di Huang Zhi (Rehmanniae Radix Fresh Juice, 1 he), boil briefly, and take warm. Cei Bai Ye (Biota Tops) may be added.
Xiang Shen Wan: Treats the same symptoms as above.
Ingredients: Ren Shen (Ginseng Radix et Rhizoma), Mai Dong (Ophiopogonis Tuber), Dang Gui (Angelicae Sinensis Radix), Sheng Pu Huang (Typhae Pollen Fresh, each 5 qian), Sheng Di Huang (Rehmanniae Radix, 1 liang), Gan Cao (Liquiritiae Radix et Rhizoma, processed, 2.5 qian).
Preparation: Grind all ingredients into powder, mix with refined honey to form pills the size of small marbles. Take one pill per dose, 3-4 times daily, dissolved in plain boiled water.
