(Attachment: Ju)
Xiong Yong (Hypochondriac Carbuncle) presents with redness, high swelling, and pain. Ju (Deep-seated Abscess) is characterized by hardness, sinking, diffuse swelling, coldness, and lack of redness. Both conditions belong to the constraint of Liver and Gallbladder fire. Delayed ulceration with discharge of foul, thin pus and signs of cold deficiency indicates a ominous prognosis.
[Note] This condition arises in the soft rib area; the part with hard bone is called "Li" (Rib), and the soft flesh below the ribs is called "Ji Xie" (Hypochondrium). Both Yong and Ju are caused by the condensation of Liver and Gallbladder fire. They often occur in individuals with weak constitution. Initially, it resembles a plum or prune, gradually growing to the size of a bowl or basin, with red color, pain, and high swelling. After fourteen days, if it ulcerates with thick pus, it is classified as Yong. If it is hard, flat, diffusely swollen, numb and painful, without redness or heat, and ulcerates after more than a month with thin pus, it is classified as Ju. If treatment fails, and it does not ulcerate on time, forced attack to induce suppuration results in swelling like a drum. Ulceration discharging foul fluid and foul-smelling pus is considered a reverse syndrome (intractable). At the initial swelling stage, both Yong and Ju should immediately take Chaihu Qinggan Tang (Bupleurum Liver-Clearing Decoction) to resolve depression and drain fire. If already formed, take Tuoli Tou Nong Tang (Interior-Supporting Pus-Permeating Decoction). When pus is mature and causing distending pain, use a recumbent needle to open it. After ulceration, the key is to drain remaining pus and tonify Qi and Blood. My commentary: In the treatment methods for Yong and Ju ulcerations, if supplementation yields no response, it is difficult to treat.
Fei Yong (Lung Carbuncle) occurs when Lung Heat is compounded by Wind injury. The Lung develops a carbuncle causing hidden chest pain. Symptoms resemble Shanghan (Cold Damage) but with severe dry cough. Sputum is thick, turbid, and smells foul like pus. Before ulceration, use Shegan Mahuang Tang (Belamcanda and Ephedra Decoction) to induce sweating. For Qi obstruction preventing lying down, use Tingli Dazao Tang (Descurainia Seed and Jujube Decoction) to purge. After ulceration, thick pus with ability to eat is auspicious; clear pus mixed with blood and inability to eat is ominous.
[Note] This condition results from heat accumulation in the Lung, compounded by Wind pathogen, leading to long-term stagnation and formation of a carbuncle. This causes hidden pain in the Zhongfu acupoint within the chest, chills, rapid pulse, symptoms resembling Shanghan, dry throat without thirst, cough with wheezing and chest fullness, expectoration of thick sticky yellow sputum mixed with foul-smelling pus and blood. During treatment, before ulceration, while pus is not yet fully formed and Wind is constrained at the exterior, the method should be dispersing. Use Shegan Mahuang Tang to induce sweating. If there is Qi obstruction, wheezing, and inability to lie down, urgently take Tingli Dazao Tang to purge. If cough is accompanied by slight fever, irritability, chest fullness, and skin roughness (lichenification), indicating impending pus formation, use Qianjin Weijing Tang (Thousand Gold Reed Shoot Decoction) to induce vomiting. If foul-smelling pus resembling rice porridge is vomited, use Jiegeng Tang (Platycodon Decoction) to drain remaining pus. If foul-smelling pus is vomited with cough and chest fullness, use Waitai Jiegeng Bai San (External Standards Platycodon White Powder) to open blockages. If hemoptysis with pus and blood occurs with afternoon fever and irritability, use Jin Li Tang (Golden Carp Decoction) as the main treatment, along with drinking child's urine. If after ulceration, hidden pain persists in the chest, diaphragm, and hypochondrium, with dry mouth and throat, irritability, excessive thirst, spontaneous and night sweats, insomnia, and coughing up thick foul sputum, this indicates incomplete pus drainage combined with interior deficiency. Use Ningfei Jiegeng Tang (Lung-Calming Platycodon Decoction) as the main treatment. If the carbuncle has ulcerated, with wheezing, foul smell, resolution of turbid sputum, but only cough, dry throat, hemoptysis, slight hypochondriac pain, and inability to lie for long periods, this belongs to unhealed ulceration of Fei Yong. Use Ziwan Rong Tang (Aster Root Decoction) for clearing and tonifying. If thirst is severe, remove Banxia (Pinellia) and add Shigao (Gypsum). If after ulceration, cough is incessant, pus sputum is not cleared, and the body and Qi are emaciated, use Qingjin Ningfei Wan (Clear Metal Lung-Calming Pill) as the main treatment. Generally, for treating this condition, auspicious signs include warm body, fine pulse, sticky pus and blood, bright sputum color, good appetite, gradual cessation of pus and blood, and moist stool. Ominous signs include rough palm skin, surging and rapid pulses after ulceration, shortness of breath, malar flush, foul pus mixed with white blood, anorexia, and constipation.
Zhongfu acupoint is also known as the Front-Mu point of the Lung, located between the third rib above the nipple.

Fei Yong (Lung Carbuncle)
Lung heat compounded by wind injury, lung carbuncle causes hidden chest pain, symptoms resemble cold damage with severe dry cough, thick turbid sputum smells foul like pus. Before ulceration, Shegan Mahuang induces sweat; Qi obstruction prevents lying, Tingli purges. After ulceration, thick pus with eating is auspicious; clear pus with blood and no eating is ominous.
[Note] This condition is due to heat accumulation in the Lung, compounded by Wind pathogen, stagnating for a long time to form a carbuncle, causing hidden pain in the Zhongfu acupoint in the chest, chills, rapid pulse, resembling Shanghan, dry throat without thirst, cough with wheezing and chest fullness, expectorating thick sticky yellow sputum mixed with foul-smelling pus and blood. In treatment, before ulceration, taking advantage of the fact that pus is not yet formed and Wind is constrained at the exterior, the method should be dispersing. Use Shegan Mahuang Tang to induce sweating. If Qi is obstructed with wheezing and fullness, unable to lie down, urgently take Tingli Dazao Tang to purge. If cough has slight fever, irritability and fullness in the chest, skin roughness like scales, and pus is about to form, use Qianjin Weijing Tang to induce vomiting. If vomiting foul-smelling pus resembling rice porridge, use Jiegeng Tang to drain remaining pus. If vomiting foul-smelling pus with cough and chest fullness, use Waitai Jiegeng Bai San to open stasis and blockage. If coughing up pus and blood with afternoon fever and irritability, use Jin Li Tang as the main treatment, and drink child's urine. If after ulceration, hidden pain in the chest, diaphragm, and hypochondrium persists, dry mouth and throat, irritability and excessive thirst, spontaneous and night sweats, inability to sleep, coughing up thick foul sputum, this is because pus is not exhausted and interior deficiency exists. Use Ningfei Jiegeng Tang as the main treatment. If the carbuncle pus has ulcerated, with wheezing, fullness, foul smell, and resolution of turbid sputum, but only cough, dry throat, hemoptysis, slight hypochondriac pain, and inability to lie for long, this belongs to the unhealed ulcer site of Lung Carbuncle. Use Ziwan Rong Tang to clear and tonify. If thirst is severe, remove Banxia and add Shigao. If after pus ulceration, cough is ceaseless, pus sputum is not exhausted, and body and Qi are emaciated, use Qingjin Ningfei Wan as the main treatment. In treating this condition, only those with warm body, fine pulse, sticky pus and blood, bright sputum color, delicious food intake, gradual cessation of pus and blood, and moist stool are auspicious. Those with rough palm skin, surging and rapid pulses in all six positions after ulceration, shortness of breath, malar flush, foul pus mixed with white blood, anorexia, and dry constipation are ominous.
The Zhongfu acupoint is also named the Lung Mu point, located in the intercostal space of the third rib above the nipple.
