Jinchuang (trauma caused by metal sharp instruments) requires careful examination of the wound. Minor injuries affect the skin and flesh, while severe injuries damage the tendons. Externally apply Rusheng Taohua San; if bleeding is heavy, administer Bazhen Tang or Dushen Tang.
[Note] This condition includes wounds from blades, arrowheads, and porcelain edges. One must assess the depth of the wound: for minor injuries where the skin and flesh are broken and blood flows incessantly, apply Taohua San; for severe injuries where tendons are severed and blood spurts, indicating major vessels are damaged, apply Rusheng Jindao San and bind tightly with silk to stop bleeding. If bleeding recurs after stopping, reapply the powder. If the medicinal scab is too thick causing constriction and pain, apply Shengji Yuhong Gao to the wound; externally paste Tuoseng Gao to promote tendon regeneration, relieve pain, and generate flesh. Regardless of whether the injury is minor or severe with broken skin and bleeding, initially administer Sanhuang Baola Wan; for slight bleeding from broken skin, administer Lidong Wan; if excessive bleeding occurs and the patient presents with a sallow complexion and dark circles around the eyes, do not focus solely on eliminating blood stasis, but rather use Bazhen Tang; in severe cases, use Dushen Tang to first consolidate the root. Both formulas should be combined with Sumu and Honghua to regulate blood stasis. Although this condition involves sudden trauma to healthy flesh, life or death prognosis can be determined by examining the pulse and physical signs. If excessive blood loss results in a deficient, fine, deep, small, or moderate pulse, survival is likely; if the pulse is floating, surging, rapid, large, excess, or deficient and hurried, death is imminent. Injury penetrating the lungs leads to death within fourteen days. Injury to the internal organs under the left hypochondrium, complete severing of the intestines, injury to the internal organs in the lower abdomen, multiple injury sites, crushing of the left thigh in the elderly, injury to the genitalia, penetration of injuries from the shoulder interior or behind the ear into the interior, all result in death. Injuries to the Tianchuang acupoint and eyebrow angle, the pulsating vessels in the back of the head and inner arm, the inner thigh and groin, above and below both breasts, the center below the heart (Jiwei), and the Shu points of the five zang and six fu organs are all fatal. Extrusion of marrow from the back of the head accompanied by inability to speak, fixed gaze, bubbling sounds in the throat, urgent salivation, and random waving of both hands also indicate death. In cases where abdominal skin is broken and intestines protrude, if only half of the intestine is injured, it is treatable. First, boil millet porridge, take the thick decoction, and wash the protruding intestine with warm liquid. Use mulberry bark tip fibers as thread, dip in Huarui Shi San, and suture the intestinal wound. Immediately apply fresh rooster comb blood to the suture site, then coat the intestine with clarified oil to keep it moist, and gently return the intestine to the abdominal cavity. Externally, use human hair to densely suture the internal muscle of the abdominal wound, leave the external skin exposed, and apply Yuebai Zhenzhu San to await flesh generation and wound closure. If the wound is too large for external suturing, protect and paste with Tuoseng Gao, wait for spontaneous suppuration, and then treat according to the methods for ulcerative diseases. After suturing, avoid fright and laughter; drink rice water in small amounts, gradually increasing the quantity. Wait twenty days before consuming thick porridge, and recover through regulation.
