Choutianluo (Stinky River Snail), is caused by the attack and infiltration of damp-heat fire in the Foot-Yangming Stomach Meridian. This condition mostly occurs on the toes and between the feet, initially presenting with white patches that ulcerate; it first feels itchy then painful, breaks open to discharge foul-smelling fluid, and resembles a snail shell in shape. In severe cases, the entire dorsum of the foot swells, accompanied by aversion to cold and fever. Initially, cleanse the area with Gancao Tang (Licorice Decoction), apply Chansu Bing (Toad Venom Cake) for three days (three cakes total). Subsequently, apply Zhenzhu San (Pearl Powder) mixed with pig spinal cord as an ointment cover, and fumigate the swelling with Zhenjun Miaotie San (Miraculous Lord's Adhesive Powder). The swelling will gradually subside. Avoid walking during this period.
Niuchengjian (Ox Hoof Callus), results from rushing along a journey, stepping into water with hot feet, and subsequently being exposed to wind. This leads to qi stagnation and blood depletion, forming hard nodules. The skin and muscles fail to receive nourishment from Ying and Wei (Nutritive and Defensive Qi), gradually developing swelling and pain; the swelling protrudes high, making it difficult to bear weight on the foot. Over time, it may rupture, discharging pus and fluid. Wash the area daily with warm water and apply Niujiao San (Horn Powder). If internal pus accumulates and the skin is tough and difficult to penetrate, puncture it with the head of a large linear needle; releasing the pus brings relief. For hardened skin, break it and cut it away with scissors; if flesh does not regenerate, apply Yuhong Gao (Jade Red Ointment) to promote growth; if flesh fills the wound but epidermis fails to grow, apply Zhenzhu San (Pearl Powder) until the skin regenerates and heals.
Jianglangzhu (Mantis Worm Infestation), mostly occurs in the finger joints. It is neither red nor hot, with swelling resembling the abdomen of a cicada. It arises from the stagnation of phlegm and qi in the Hand-Shaoyin Heart Meridian. Initially painless, pain develops after a long duration; prolonged pain leads to putrefaction, yet the swelling persists. Apply Chansu Bing (Toad Venom Cake) paste, which gradually transforms into thin pus. The course lasts about one year for recent cases or up to three years for distant ones; this condition is common in individuals with weak constitutions. Internal administration of tonifying agents is necessary to prevent transformation into Laozhai (Consumption Disease).
