Harmonize Ying and Wei (Defensive and Nutritive Qi), nourish Qi and Blood. Treat irregular menstruation, pain in the umbilical and abdominal regions, women's menstrual disorders where menses may come early or late, with varying amounts, and diverse types of pain; pain in the waist, feet, and abdomen; metrorrhagia and metrostaxis (heavy bleeding or prolonged spotting between periods); excessive lochia after miscarriage; retained lochia that fails to discharge; abdominal pain with vaginal bleeding during pregnancy indicating fetal instability; postpartum blood stasis clumps failing to disperse; lochia persisting due to excessive blood loss. Use Siwu Tang (Four Substances Decoction) added with Corni Fructus (Shanzhuyu) decocted and taken orally. If the constitution is Yang-predominant, reduce the dosage of Corni Fructus; if Yin-predominant, increase it. Another prescription adds Cyperi Rhizoma (Xiangfuzi) to treat various deficiencies and insufficiencies, applicable to all these conditions.
Prepared Rehmanniae Radix (Shudihuang) [Nourishes Blood. For umbilical pain, this herb is indispensable as it enters the Kidney Meridian.]
Ligusticum Chuanxiong Rhizoma (Chuanxiong) [Treats wind dispersing Liver Wood. For headache due to Blood deficiency, this herb is indispensable as it enters the Liver Meridian.]
Paeoniae Radix Alba/Rubra (Shaoyao) [Harmonizes Blood and regulates Spleen. For deficiency-type abdominal pain, this herb is indispensable as it enters the Spleen Meridian.]
Angelica Sinensis Radix (Danggui) [Harmonizes Blood. For stabbing blood-stasis pain resembling knife cuts, this herb is indispensable as it enters the Kidney Meridian.]
Grind the above herbs into a coarse powder, decoct with water, and take orally. Modifications are listed below:
For women suffering from bone-steaming fever (Yin-deficiency heat), add Lycii Cortex (Digupi) and Moutan Cortex (Mudanpi).
If there is fetal restlessness with continuous vaginal bleeding during pregnancy, add ten leaves of Artemisia Argyi Folium (Aiye), one piece of Colla Corii Asini (Ejiao), and also add Allii Fistulosi Bulbus (Congbai) and Scutellariae Radix (Huangqin). If the Blood organ is cold and deficient with excessive bleeding from metrorrhagia, also add Colla Corii Asini (Ejiao).
For regular use by women: In Spring, double the dose of Ligusticum Chuanxiong Rhizoma (Chuanxiong) for wiry pulse and headache; in Summer, double Paeoniae Radix (Shaoyao) for flooding pulse and undigested stool diarrhea; in Autumn, double Prepared Rehmanniae Radix (Shudihuang) for deep, choppy pulse and Blood deficiency; in Winter, double Angelica Sinensis Radix (Danggui) for deep, cold pulse and anorexia.

For sudden heavy menstrual bleeding, add Scutellariae Radix (Huangqin) to Siwu Tang; if abdominal pain is present, add Coptidis Rhizoma (Huanglian); do not remove Scutellariae Radix (Huangqin) in summer.
If menstrual blood is dark like black bean juice, add Scutellariae Radix (Huangqin) and Coptidis Rhizoma (Huanglian) (each 1 liang).
If menstrual flow is scanty but color is normal, add Prepared Rehmanniae Radix (Shudihuang) and Angelica Sinensis Radix (Danggui) (each 1 liang) to Siwu Tang.
If menses have just started or stopped, or there are alternating chills and fever, first administer Xiaochaihu Tang (Minor Bupleurum Decoction) to resolve the chills and fever, then harmonize with Siwu Tang.
If chills and fever do not subside, do not take Siwu Tang; this is a changed syndrome where exterior pathogens remain, so Siwu Tang will be ineffective. Follow the previous discussion on changed syndromes and medicate according to the specific presentation.
