The *Classic of Medicine* (Huangdi Neijing) states: Changtan (Intestinal Polyp/Tumor) is caused by cold pathogenic qi invading the exterior of the intestines, struggling with Wei Qi (Defensive Qi), preventing Qi from nourishing tissues. Consequently, Qi stagnates and adheres internally, evil Qi arises, and polyps are formed. Initially, it grows to the size of a chicken egg, gradually increasing in size until it resembles pregnancy. It feels firm upon pressure and does not move when pushed, yet menstruation continues on schedule; this is its characteristic presentation. This is a disease of Qi without involvement of Blood, hence menstruation remains uninterrupted. Muxiang Tongqi San (Wood Fragrance Qi-Regulating Powder), a formula of great acridity and heat, should be used to treat it (this involves Qi stagnation in the Large Intestine, constituting a Qi disorder).
*Zhe Gong Lu* (Records of Breaking the Arm) records: The daughter-in-law, Ms. Shen, suffered from amenorrhea due to excessive depression and anger. Her abdomen gradually enlarged, leading to the assumption that she was pregnant. After more than ten months without delivery, various symptoms emerged, causing suspicion. Physicians suspected blood stasis (Xue Yu) and considered purging, but because her constitution was weak and could not withstand harsh treatment, they opted for gradual dissolution using blood-invigorating and regulating herbs over a long period, which ultimately proved fatal. Later, reading Dr. Sheng Qidong’s case of treating the Crown Prince’s Consort, one deeply regrets such hesitation. During the Yongle reign, Empress Zhang of the Crown Prince had amenorrhea for over ten months. Physicians assumed it was pregnancy. As abdominal distension worsened, the Emperor ordered Dr. Qidong to diagnose her. The condition perfectly matched blood stasis. His prescription consisted of blood-breaking agents, resulting in the discharge of several dou of blood, after which the illness resolved. The daughter-in-law’s illness was identical. Had physicians boldly employed purging methods after confirming non-pregnancy beyond the tenth month, she might have survived. Alas, lacking such courage and insight, physicians failed her—how sorrowful! (This involves blood stasis at the cervical os, constituting a Blood disorder).
Various antenatal syndromes are caused by fetal Qi. Both fetal movement (Tai Dong) and fetal leakage (Tai Lou) involve vaginal bleeding, but fetal movement is accompanied by abdominal pain, whereas fetal leakage is not. Therefore, fetal movement requires Qi-regulation, while fetal leakage requires clearing heat. Ezu (Nausea and Vomiting of Pregnancy) refers to nausea obstructing food intake; obesity is often due to Phlegm, while emaciation is often due to Heat. Erchen Tang (Two-Cured Decoction) with modifications is suitable. Zifan (Fetal Irritability) refers to restlessness and mental agitation. Zi Xian (Fetal Epilepsy) refers to salivary overflow, convulsions, upward deviation of the eyes, and locked jaw. Zi Zhong (Fetal Edema) refers to puffy face and limbs. Zi Qi (Fetal Qi Swelling) refers to edema of both feet. Zi Lin (Fetal Strangury) refers to scanty and difficult urination. Zhuan Bao (Twisted Bladder) refers to urinary retention. Zi Xuan (Fetal Suspension) refers to irregular fetal Qi rushing to the heart and chest, causing distension and pain. Generally, the Spleen governs the transportation and transformation of water and grain. When a woman is pregnant, the transformation of water and grain becomes impaired, generating Dampness; Dampness generates Phlegm; Phlegm generates Heat; Heat generates Wind. Zi Zhong and Zi Qi belong to Dampness; Ezu belongs to Phlegm; Zifan and Zi Lin belong to Heat; Zi Xian belongs to Wind; Zi Xuan belongs to Qi stagnation; Zhuan Bao belongs to Deficiency. Dampness is treated by leaching; Phlegm by resolving; Heat by clearing; Wind by calming; Qi stagnation by dispersing; Deficiency by tonifying. The primary goal is always to eliminate pathogens and preserve the fetus. The method for preserving the fetus is to nourish the Spleen and Stomach before the third month, and to strengthen the waist and kidneys, nourish Blood, and regulate Qi with heat-clearing adjuvants after the fourth month; this is the general principle. However, regulation must particularly be based on the individual’s Yin-Yang balance and Qi-Blood excess or deficiency.
*Jianbian Fang* (Simple and Effective Prescriptions) treats habitual abortion or miscarriage at three or four months. Two months prior to the expected date, use eight liang of Du Zhong (Eucommia Bark), soaked in glutinous rice decoction, stir-fried to remove silk fibers; two liang of Xu Duan (Dipsacus Root), soaked in wine, dried, and powdered; five to six liang of Shan Yao (Dioscorea Rhizome), powdered. Mix with paste to make pills the size of wu zi (wu-tzu seeds). Take fifty pills each time with rice water on an empty stomach.
Conception relies on the deficiency of the Liver organ. Liver Qi can only generate the fetus and has no other function. If Blood is insufficient to nourish the Liver, Liver deficiency occurs, leading to cravings for sour foods.
For antenatal fetal preservation, Bai Zhu (Atractylodes Macrocephala Rhizome) and Huang Qin (Scutellaria Baicalensis Root) are excellent herbs. Tiao Qin (Baikal Skullcap Root) is the sacred herb for stabilizing the fetus. Laypeople do not understand this, fearing its cold nature, and instead believe warm-hot herbs nourish the fetus. They fail to realize that clearing heat during pregnancy allows Blood to follow the meridians without reckless flow, thereby nourishing the fetus. Only patients with deep-slow pulses and Spleen-Stomach deficiency are unsuitable for this.
Childbirth is like carrying a boat over a dam. Difficult labor is mostly due to Qi and Blood deficiency, sometimes caused by lack of caution in the ninth or tenth month, or by Qi and Blood stagnation failing to circulate. Therefore, within nine months, one must regulate Qi and harmonize the middle Jiao by taking Dasheng San (Reaching Life Powder) for about ten doses, making labor easier at term.
If a pregnant woman’s pulse is fine and regular, the fetal origin is complete, resembling the fragrance of ripe fruit; delivery will be easy. A large, floating, and slow pulse indicates severe Middle Qi deficiency due to nurturing the fetus and dissipated Fire Qi; delivery will be difficult. Thus, the antenatal pulse should be surging and large, while the postnatal pulse should be deep and small. Gender differentiation is determined by the left and right pulses: rapid left pulse indicates male, rapid right pulse indicates female. Deep and solid left pulse, floating and large right pulse indicates female then male; this can be predicted in advance. Li Jing (Departing from the Channel) pulse with six beats, deep, fine, and slippery, with labor pains reaching the waist, indicates immediate fetal expulsion. Ban Chan (Miscarriage) with leakage is indicated by Ge Mai (Drum-like Pulse); Ruo Mai (Weak Pulse) indicates Blood exhaustion and imminent danger.
A woman with prolonged vomiting during pregnancy, refusing all medicines, was cured by a prescription from Zhang Lihou. Both Cun (Inch) pulses were surging, large, and rapid, especially on the right. Metal constitution is naturally dry, now even drier. Left Guan (Gate) pulse is surging and wiry, indicating Liver storing Blood is deficient as there is no Blood to store. Right Guan pulse is wiry, fine, and rapid, indicating chronic generation of Grain Qi, with Liver deficiency Fire insulting, hence wiry and rapid. The fineness indicates extreme depletion of Stomach Qi. Left Chi (Cubit) pulse is extremely fine, almost absent; Right Chi pulse is surging, large, and rapid. These indicate fluid nourishment is exhausted and dry. Shaoyin (Lesser Yin) deficiency Fire floats upward, Jueyin (Terminal Yin) depressed Fire reaches up, Shaoyang (Lesser Yang) latent Fire ascends. Dry Earth scorched by the Three Fires cannot moisten and nurture. The Classic states: "All vomiting and acid regurgitation belong to Heat." This is what it means. The specific prescription follows, serving as an emergency symptomatic treatment. After vomiting ceases, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) plus Mai Dong (Ophiopogon Tuber) and E Jiao (Donkey-Hide Gelatin) should be taken as pills for long-term conditioning to treat the root cause.

If habitual miscarriage occurs at three to five months, start taking this pill one month in advance to ensure full term. Even if spotting precedes miscarriage, urgent administration can preserve the pregnancy.
