然转胞之病,亦不尽此,或中焦脾虚,不能散精归于胞;及上焦肺虚,不能下输布于胞;或胎重压其胞;或忍溺入房;
There is dried blood in the uterus. (What causes the continuous discharge? It is due to damp-heat causing putrefaction and transformation.) The discharged white matter, (commonly known as leukorrhea in popular terms. An external treatment method should be used, primarily with Fanshi Wan [Alum Pill].) This addresses the condition of amenorrhea caused by dried blood in the uterus, which transforms into white discharge upon encountering damp-heat; it provides the prescription for this treatment.
Women suffering from sixty-two types of wind pathogens, experiencing stabbing pain in the abdomen due to blood and Qi stagnation, are treated with Honghua Jiujie [Safflower Wine Decoction]. This addresses cases where women have abdominal stabbing pain due to wind pathogens affecting blood and Qi. The mention of "blood and Qi" serves to distinguish this condition from cold colic (Hanshan). The specific meaning of "sixty-two types" remains unclear. Zhang Yin'an stated: Safflower (Honghua), being red and juicy, is a substance that generates and moves blood. Tao Hongjing noted that Safflower treats postpartum blood syncope, colic from incomplete expulsion of lochia, and fetal death in utero. *Jin Gui Yao Lue* states that Honghua Jiujie treats sixty-two types of wind in women and also malaria. Mr. Linchuan said: To treat wind, first treat the blood; when blood flows, wind extinguishes itself. Wind is a Yang pathogen, while blood is a Yin fluid; this represents a counter-treatment approach. Safflower flowers and stems/leaves are hairy and thorny, possessing the image of firm Metal, thus capable of subduing Wind-Wood. Men and women share similar blood and Qi natures; Zhongjing specifically treats sixty-two types of wind in women for good reason. Generally, women have excess Qi but deficient Blood; what is lacking is the Chong and Ren meridians, which disperse between the skin and muscle interstices, nourishing the flesh, warming the muscles, and generating hair; men grow mustaches on the upper lip, while women menstruate regularly, hence women often have deficiencies. The flower's nature ascends, its blooms spread vines, mainly dispersing blood in the skin, thereby supplementing women's deficiency; thus, it treats wind in women. When blood is deficient, the skin and flesh interstices are not tight, making one prone to wind generation. This herb governs the Chong and Ren, so it specifically treats lochia after childbirth. *Ling Shu Jing* states: For those who drink wine, Wei Qi (Defensive Qi) acts first in the skin; therefore, decocting with wine aids the medicinal properties; pathogens also hide in the membrane source interstices, so wine can guide them out. Blood circulates within the meridians and scatters outside in the skin; the governing herbs differ accordingly. For example, Danggui [Angelicae Sinensis Radix] and Dihuang [Rehmanniae Radix] nourish blood within the vessels; Honghua generates blood outside the vessels; Chuanxiong [Ligustici Rhizoma et Radix], Shaoyao [Paeoniae Radix], Danpi [Moutan Cortex], and Hongqu [Levisticum Fructus] act as internal-external combined agents. If practitioners can comprehend the profound intent of the sages in prescribing herbs, they will have grasped more than half of the essence.
"For various pains in the abdomen of women, Danggui Shaoyao San [Angelica and Peony Powder] is the primary treatment." This provides the prescription for various abdominal pains in women. Pathogens such as cold, heat, deficiency, excess, Qi stagnation, and food retention all cause abdominal pain; the implication is that modifications can be made to this formula based on these factors, rather than using this single formula to rule all abdominal pains. You Zaijing stated: Women are governed by blood, and blood is governed by Middle Qi. Middle Qi refers to Earth Qi (Spleen/Stomach Qi). If Earth is dry, it cannot generate things; if Earth is damp, it cannot generate things. Xiongqiong [Chuanxiong] and Shaoyao [Paeoniae Radix] nourish the blood; Fuling [Poria], Baizhu [Atractylodis Macrocephalae Rhizoma], and Zexie [Alismatis Rhizoma] treat dampness. When dryness and dampness are balanced, Earth can generate, and all pains are eliminated.
"For abdominal pain in women, Xiao Jianzhong Tang [Minor Construct the Middle Decoction] is the primary treatment." This provides the prescription for abdominal pain caused by deficiency-cold and interior tension in women.
(Changsun Xin Dian) Commentary: *Shang Han Lun* states: If the Yang pulse is choppy and the Yin pulse is wiry, according to pulse diagnosis, there should be urgent pain in the abdomen, and Xiao Jianzhong Tang [Minor Construct the Middle Decoction] is appropriate. If it does not improve, administer Xiao Chaihu Tang [Minor Bupleurum Decoction].
Question: A woman is ill; her diet is normal, she feels vexed and hot, cannot lie down, and instead leans forward to breathe heavily. What is the cause? The Master said: "(Normal diet indicates the disease is not in the Stomach; vexation and heat indicate Yang Qi failing to transform; leaning forward to breathe heavily indicates water failing to descend.)" This is called "Zhuan Bao" (Displaced Bladder/Uterus), resulting in inability to urinate. Because the bladder ligaments (are twisted and distorted), this disease occurs. (Provided there are no concurrent syndromes,) simply promoting urination will resolve it, as the Qi in the bladder will descend through the urinary pathway, untwisting the ligaments. Treat with Shenqi Wan [Kidney Qi Pill]. This provides the prescription for Zhuan Bao syndrome caused by twisted bladder ligaments leading to urinary retention. "Liao li" is synonymous with "Liao li," meaning the bladder ligaments are tangled and twisted, causing the bladder to rotate; rotation leads to urinary retention. Treating with this formula tonifies the Kidney, restoring Qi transformation; once Qi transforms, water flows, and the patient recovers. However, Zhuan Bao is not solely caused by this; it may result from Middle Jiao Spleen deficiency failing to scatter essences to the bladder; or Upper Jiao Lung deficiency failing to distribute fluids downward to the bladder; or a heavy fetus pressing on the bladder; or holding urine during intercourse. One must investigate the root cause and treat accordingly.
For cold in the female genitals, (warmth should be applied to) the vagina (not via oral administration, but by inserting medication internally, termed "Zuo Yao" [Sitting Medicine]). Shechuangzi San [Cnidium Seed Powder] is the primary treatment. This indirectly follows the previous section regarding pulling pain in the yin region and cold aversion in the lower abdomen, providing the prescription. Since cold enters through the vaginal opening and does not exit via the exterior, warming the affected area resolves the condition. Shechuangzi [Cnidii Fructus] is warm to dispel cold; combined with Baifen [Lead White], it dries dampness, as cold generates dampness.
If the Shaoyin (Kidney) pulse is slippery and rapid, (slipperiness indicates dampness, rapidity indicates heat; damp-heat combines and knots in the Yin division, causing sores to appear in the anterior yin, with erosion and ulceration of the vulva,) Ye Langya Tang [Wild Wolf Tooth Decoction] should be used for washing. This provides the prescription for damp-heat flowing downward to the anterior yin, causing erosive sores. Wild Wolf Tooth Grass [Saussureae Heterophyllae Herba] has a sour and bitter taste, clears evil heat, treats scabies, itching, and malignant sores, and removes roundworms; thus, it is used for treatment. If Wild Wolf Tooth Grass is unavailable, replace it with Yelangdu [Daphne Genkwa].
Stomach Qi leaking downward, (not passing as flatus from the anus, but emitting noise from the anterior yin, termed "Yin Chui" [Vaginal Flatulence]), creates a continuous, noisy sound. This is due to excess of Grain Qi (constipation). Treat with Zhigao Fa Jian [Lard and Hair Decoction]. (Its moistening property unblocks the bowels; thus, Qi exits through the anus, while the anterior passage remains sealed.)
