Do not worry about the patient's deficiency. When wind-pathogen invades and causes anhidrosis (absence of sweating), it indicates that the interstices are closed, preventing the pathogen from being expelled through sweat; thus, treating wind-stroke with anhidrosis is appropriate. However, if Ephedra Decoction (Mahuang Tang) is used for patients with spontaneous sweating, it will cause excessive loss of body fluids, resulting in greater harm. Although Zhang Zhongjing prescribed Guizhi Tang (Cinnamon Twig Decoction) for wind-stroke with spontaneous sweating, for those who suffer from continuous convulsions, deviation of the mouth and eyes, and profuse sweating all over the body, conventional treatments are insufficient. In such cases, Duhuo Tang (Angelica Pubescens Decoction) and Xuming Zhusan (Xuming Boiled Powder) are indispensable as they restore Ying-Wei (Nutritive and Defensive Qi) and expel wind-pathogens.
[Xue Ji] For the aforementioned conditions involving spontaneous sweating due to insecure interstices, use Guizhi Tang (Cinnamon Twig Decoction) or Fangfeng Baizhu Muli Tang (Saposhnikovia, Atractylodes Macrocephala, and Oyster Shell Decoction). If spontaneous sweating results from the overuse of wind-expelling drugs, use Baizhu Fangfeng Tang (Atractylodes Macrocephala and Saposhnikovia Decoction). If caused by Yang-Qi deficiency, use Fuzi Tang (Aconite Decoction). If accompanied by night sweats, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) administered with Liuwei Wan (Six-Ingredient Pill with Rehmannia). If ineffective, use Danggui Liuhuang Tang (Angelica Sinensis and Rhizoma Chuanxiong Hexa-Huang Decoction).
Duhuo Tang: Treats dizziness and confusion due to wind-deficiency, loss of bodily awareness, spasms of the hands and feet, restlessness in sitting and lying, or alternating chills and fever. It is particularly suitable for patients with blood deficiency who cannot tolerate diaphoretic herbs, and for wind-stroke with spontaneous sweating.
Ingredients: Chuan Duhuo (Radix Angelicae Pubescentis), Qianghuo (Rhizoma et Radix Notopterygii), Renshen (Radix Ginseng), Fangfeng (Radix Saposhnikoviae), Danggui (Radix Angelicae Sinensis) - remove rhizomes from five ingredients; Xixin (Herba Asari), Fushen (Poria with Hostwood) - remove wood; Banxia (Rhizoma Pinelliae) - washed seven times in hot water, sliced; Guixin (Cortex Cinnamomi); Baiwei (Radix Cynanchi Atrati); Yuanzhi (Radix Polygalae) - remove heart; Shichangpu (Rhizoma Acori Tatarinowii); Chuanxiong (Rhizoma Chuanxiong) - each half liang; Gancao (Radix Glycyrrhizae) - seven qian half, roasted.
Preparation and Administration: Coarsely crush the herbs. Take eight qian per dose, add one and a half zhan of water and five slices of fresh ginger, decoct until reduced to seven tenths, strain, and take warm at any time.
Xuming Zhusan: Treats stagnation of wind-qi causing mental confusion, weakness in the four limbs, deviation and twitching of the mouth and eyes, occasional convulsions, loss of body fluids, thirst, and desire to drink. This formula supports Ying-Wei and dispels deficient wind, being especially suitable for wind-stroke with spontaneous sweating and postpartum wind-stroke with spontaneous sweating.
Ingredients: Fangfeng (Radix Saposhnikoviae), Duhuo (Radix Angelicae Pubescentis), Danggui (Radix Angelicae Sinensis), Renshen (Radix Ginseng), Xixin (Herba Asari), Gegen (Puerariae Lobatae Radix), Shaoyao (Radix Paeoniae Alba), Chuanxiong (Rhizoma Chuanxiong), Gancao (Radix Glycyrrhizae), Shudihuang (Radix Rehmanniae Preparata), Banxia (Rhizoma Pinelliae), Yuanzhi (Radix Polygalae) - remove heart; Jingjiesui (Perilla Frutescens Inflorescence) - each half liang; Guixin (Cortex Cinnamomi) - seven qian half.
Preparation and Administration: Coarsely crush the herbs. Take five qian per dose, add one zhan of water and three large slices of fresh ginger, decoct until reduced to seven tenths, strain, and take warm at any time. If sweating is excessive and unceasing, add one and a half fen of Muli Fen (Oyster Shell Powder).
Supplementary Fangfeng Baizhu Muli San: Treats unceasing spontaneous sweating due to wind-stroke.
Ingredients: Baizhu (Rhizoma Atractylodis Macrocephalae), Muli (Concha Ostreae) - calcined, Fangfeng (Radix Saposhnikoviae) - equal parts.
Preparation and Administration: Grind into fine powder. Take with wine or rice broth, three times daily. Sweating will cease. If it does not stop, Huangqi Jianzhong Tang (Astragalus Decoction to Establish the Middle) may be taken.
[On Thief-Wind and Hemiplegia]
The Old Man Yaoyin stated: The manifestation of hemiplegia (Piangu) is paralysis of half the body, emaciation of muscles, pain in the bones, while consciousness remains normal. This is called Piangu. The origin of this disease lies in the imbalance and deficiency of Yin and Yang, weakness of the Zang-Fu organs, emptiness of the meridians, and insufficiency of Blood and Qi. Sitting in the wind allows the wind-pathogen to enter via the deficiency. The *Neijing* states: "Sweating on only one side leads to hemiplegia." Analyzing its principle, it is like a tree where one side lacks fluid nourishment and dries up first, then suffers damage from the wind. Similarly, if one side of the human body lacks Blood and Qi nourishment and dries up first, it becomes vulnerable to wind-pathogens.
Wang Ziheng stated: Just as water can float or capsize a boat, wind can nourish or harm the body. This implies that just as leaks let water into a boat, leaks in the body allow wind to cause injury. As the ancients said: "To treat wind, first treat the blood; when blood flows, wind extinguishes itself." Treatment should prioritize nourishing blood before expelling wind, ensuring recovery. Suitable formulas include Da Bafeng Tang (Great Eight-Wind Decoction), Zengsun Yinyu Jiu (Modified Yinyu Wine), and Xuduan Tang (Dipsacus Decoction).
[Grand Theory]
Thief-wind hemiplegia occurs when one side of the body is deficient and receives wind, which lodges in the half-body. Due to labor and injury depleting Blood and Qi, leaving one side weak, wind enters and lodges in the half-body, known as Pianfeng (Partial Wind). If the wind-pathogen penetrates deeply, true Qi departs, and evil Qi remains, resulting in hemiplegia. This arises from the depletion of Blood and Qi, allowing wind to lodge, preventing mutual circulation and nourishment in the muscles, thus causing hemiplegia.
[Xue Ji] The theory "to treat wind, first treat the blood" is correct. Generally, this condition results from improper care during pregnancy or postpartum, leading to dryness of Essence and Blood, and withering of Liver Wood. The treatment method should tonify the source of transformation. According to the *Shengqi Tongtian Lun*: "Wind lodging disturbs Qi, Essence is lost, and evil injures the Liver." The *Yinyang Yingxiang Dalun* states: "Wind Qi connects with the Liver; wind combats causing Heat excess, Heat excess dries Water, dried Water fails to nourish Qi, thus Essence is lost." This explains the genesis of wind diseases. A female patient prone to anger suffered frequent spontaneous sweating and early menstruation. I diagnosed Liver Fire and Blood Heat. She did not believe me and indiscriminately used heat-clearing drugs, causing delayed menstruation. Later, due to fatigue and anger, she developed trismus, groaning, limb paralysis, surging pulses, and red face. I prescribed Bazhen Tang (Eight-Treasure Decoction), Maidong (Ophiopogonis Radix), Wuweizi (Schisandrae Fructus), Zhizi (Gardeniae Fructus), and Danpi (Moutan Cortex). After several doses, she improved. Combined with Xiaoyao San (Free and Easy Wanderer Powder) and Liuwei Wan (Six-Ingredient Pill with Rehmannia) for over thirty doses each, she fully recovered.
Da Bafeng Tang: Treats wind-stroke hemiplegia, aphonia, hemiplegia, and occasional mental confusion.
Ingredients: Danggui (Radix Angelicae Sinensis) - remove rhizome; Xingren (Pruni Semen) - stir-fried with bran; Gancao (Radix Glycyrrhizae) - roasted; Guixin (Cortex Cinnamomi); Jiangjiang (Zingiberis Rhizoma) - roasted, each two liang; Wuweizi (Schisandrae Fructus); Shengma (Cimicifugae Rhizoma) - each half liang; Chuan Wutou (Aconiti Radix) - roasted, peel and navel removed; Huangqin (Scutellariae Radix); Shaoyao (Radix Paeoniae Alba); Duhuo (Radix Angelicae Pubescentis); Fangfeng (Radix Saposhnikoviae) - remove rhizomes; Chuanxiong (Rhizoma Chuanxiong); Mahuang (Ephedrae Herba) - remove joints; Qinjiao (Gentianae Macrophyllae Radix); Renshen (Radix Ginseng) - remove rhizomes; Shihu (Dendrobii Caulis) - remove roots, slice, soak in wine and roast; Fushen (Poria with Hostwood) - remove wood; Shigao (Gypsum Fibrosum); Huangqi (Astragali Radix); Ziwan (Astern Radix) - each one liang; Dadou (Sojae Semen Preparatum) - three liang, skin removed and roasted.
Preparation and Administration: Coarsely crush. Take five qian per dose, add two zhan of water and one he of wine, decoct until one zhan remains, strain, and take warm. For mental confusion, do not use wine. Another version excludes Fushen and includes Yuanzhi (Radix Polygalae) and Chifuling (Poria cum Rosello).

[On Deviation of Mouth and Eye]
Women suffering from deviation of the mouth and eye due to wind have a deficient constitution allowing wind to enter the tendons flanking the mouth. The tendons of the Foot-Yangming Meridian ascend flanking the mouth; if these tendons are deficient, wind enters via the deficiency, causing unilateral tendon tension and misalignment, resulting in facial deviation.
[Xue Ji] A female patient with emotional depression suffered tendon spasms, bone pain, and a sensation of a lump in the throat. After taking Youyao Shunqi Tang, she developed facial deviation, difficulty extending arms, increased phlegm-saliva, internal heat, afternoon fever, poor appetite, and fatigue. I diagnosed Depression-Fire injuring the Spleen and Blood Dryness generating Wind. I prescribed Jiawei Guipi Tang (Modified Restore the Spleen Decoction) for twenty doses, improving her physique and diet. Then Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) for over ten doses, reducing phlegm-heat and the throat lump. Further, Shengyang Yiewei Tang (Raise Yang and Benefit Stomach Decoction) cured most symptoms. However, arm extension remained impaired due to Liver Blood deficiency causing tendon spasm. I prescribed Liuwei Wan (Six-Ingredient Pill with Rehmannia) to nourish Kidney Water and generate Liver Blood, achieving cure.

Treatment for Facial Deviation
Use seven seeds of Ricinus Communis (Bimazi), remove skins, grind into fine paste, form into cakes, and apply to the palm. (Apply to left hand for right-sided deviation, right hand for left-sided deviation.) Then sit over a copper basin of hot steam. Wash off immediately once the face aligns. (Alternative: Use Croton Seed [Badou] instead of Ricinus. From *Bencao*.)
