[Da] When a woman suffers from cough, it is caused by the weakness of the body and external contraction of cold, heat, wind, and dampness. The Lung is the canopy of the five Zang organs, internally governing the Qi of all organs, and externally connecting with the skin and hair. If injured by cold, heat, wind, and dampness, pathogenic factors enter the Lung from the skin and hair, injuring both the interior and exterior, thus causing cough. Generally, treating cough cannot be uniform; one must differentiate based on the pulse. A floating and wiry pulse indicates onset from wind; a soggy and weak pulse indicates onset from dampness; a surging and rapid pulse indicates onset from heat; a slow and choppy pulse indicates onset from cold. For wind, disperse it; for dampness, dry it; for heat, cool it; for cold, warm it; for deficiency, tonify it. There will be no case that does not heal.
[Xue] Danxi said: In spring, Wood Qi rises; in summer, Fire Qi flares upward; in autumn, damp-heat injures the Lung; in winter, wind-cold comes from outside. Treatment should involve releasing the exterior, moving phlegm, and opening the interstices, using Er Chen Tang (Two-Cured Decoction) added with Mahuang (Ephedrae Herba), Jiegeng (Platycodonis Radix), and Xingren (Armeniacae Semen Amarum). Add herbs according to the type of phlegm-fluids. For cough due to consumptive disease (Lao Sou), Si Wu Tang (Four-Agent Decoction) added with Zhuli (Bambusae Caulis in Taenia) and Jiangzhi (Zingiberis Juice) is suitable. Dry cough is difficult to treat; this pattern involves phlegm stagnation and fire pathogen. Use Jiegeng to open the Lung. In summer, use Yin-nourishing and fire-descending herbs; if unresolved, it becomes consumptive disease. Cough mostly occurring in the morning is due to Stomach fire; use Beimu (Fritillariae Bulbus) and Shigao (Gypsum Fibrosum). Cough mostly occurring in the afternoon is due to Yin deficiency; use Si Wu Tang added with Chaohei Huangbai (Phellodendri Cortex fried black) and Zhimu (Anemarrhenae Rhizoma). Cough at dusk is due to fire floating in the Lung; use Wuweizi (Schisandrae Fructus) and Wubeizi (Galla Chinensis). Cough at the fifth watch (early morning) is due to fire from food stasis flowing into the Lung; use Beimu and Ruang Shigao (Alumen). Lung distension with cough, or pain on either side preventing sleep, is caused by phlegm binding with blood stasis and Qi stagnation; nourish blood, soothe the Liver, and clear phlegm using Si Wu Tang added with Taoren (Persicae Semen), Hezi (Terminalia Chebulae Fructus), Qingpi (Citri Reticulatae Pericarpium Viride), and Zhuli. Cough with hypochondriac pain requires soothing Liver Qi, using Qingpi. If accompanied by excess phlegm, use Baijiezhi (Sinapis Semen). If blood obstructs Qi causing cough, use Taoren, Dahuang (Rhei Radix et Rhizoma), and Jiangzhi pills. Ginger is often used in treating cough because of its pungent dispersing nature. When phlegm moves due to fire and rebels upward causing cough, first treat the fire, then the phlegm, using Zhimu to stop cough, clear the Lung, nourish Yin, and descend fire. Night cough requires formulas clearing the Yin aspect. Using Yingsuke (Papaveris Pericarpium) frequently for severe cough is acceptable, provided the root cause is eliminated; this is a final astringent treatment. I believe the aforementioned morning cough belongs to excessive Stomach fire, treated with Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) [sequelae of Cold Damage]. Spleen Qi deficiency is treated with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) [miscellaneous diseases from overwork], added with Chao Zhizi (Gardeniae Fructus fried). Afternoon cough belongs to Yin-blood deficiency, treated with Si Wu Tang, Huangbai, and Zhimu [these two fried black with wine]. Kidney Water deficiency is treated with Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) [miscellaneous consumptive disease]. Dusk cough is treated with Si Wu Tang, Wuweizi, and Maimendong (Ophiopogonis Radix) combined with the previous pill. Fifth-watch cough is treated with Liujunzi Tang (Six-Gentlemen Decoction) [miscellaneous consumptive disease]. Insomnia and bilateral hypochondriac pain are treated with Liuwei Dihuang Wan and Buzhong Yiqi Tang. If due to Qi deficiency and loose interstices allowing invasion of the Six Excesses, dispel external pathogens and strengthen Spleen Earth. If due to excessive Heart Fire, drain Liver Wood and nourish Lung Metal. If Lung Metal Qi is deficient, tonify Spleen Earth to generate Lung Qi. If due to Kidney Water depletion with deficient fire flaming upward, tonify Lung and Kidney to nourish the source of transformation. Generally, Wind Pathogen and Stomach Fire represent excess-heat patterns, which are easy to treat. Only Lung and Kidney depletion represents damage to the True Zang organs, which is the most difficult to regulate. One woman, accustomed to hard labor, suffered cough, fever, hemoptysis, night sweats, generalized body pain, and alternating chills and fever in early winter. After taking phlegm-resolving and fire-descending herbs, she developed lockjaw and tendon spasms. This was due to underlying blood deficiency further damaged by medication. I used Bawei Wan (Eight-Ingredient Pill) as the main formula, assisted by Buzhong Yiqi Tang, Maimendong, Wuweizi, and Shanyao (Dioscoreae Rhizoma), resulting in recovery after more than a year. Another woman suffered cough, fever, vomiting of sputum (about 5-6 bowls day and night), wheezing, chest oppression, dry thirst, anorexia, and metrorrhagia like a gush. This was Spleen Earth deficiency and cold, treated with Bawei Wan and Fuzi Lizhong Tang (Aconite Decoction to Regulate the Middle) [miscellaneous internal cold] until cured. One woman, distressed by her mother-in-law, suffered cough, chest diaphragm discomfort, and loss of taste. This indicated injury to both Spleen and Lung with phlegm stagnation in the middle. Initially, Guipi Tang (Restore the Spleen Decoction) [miscellaneous forgetfulness] was added with Zhizi, Fuxiong (Ligusticum Chuanxiong rhizome), Beimu, and Jiegeng, resolving symptoms gradually. Later, Liujunzi Tang added with Xiong, Danggui (Angelicae Sinensis Radix), and Jiegeng was alternated, leading to full recovery. One woman suffered cough, profuse morning sputum, and nighttime wheezing insomnia. Morning sputum was attributed to Spleen deficiency transforming food; nighttime wheezing to Lung deficiency with deficient fire rushing up. Buzhong Yiqi Tang added with Maimendong and Wuweizi cured her. One woman suffered cough, hypochondriac pain, fever worsening at sunset. I diagnosed Liver-Spleen deficient heat injuring the Lung, treated with Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) [menstrual regulation] added with Shudihuang (Rehmanniae Radix Praeparata) until cured. Later, due to anger and overwork, symptoms recurred with Taiyang headache, alternating chills/fever, cough, and enuresis. This indicated Lung Qi deficiency failing to control the bladder. The previous powder and Dihuang Wan cured it. One woman with chronic cough had sallow complexion, occasional pallor, limb fatigue, poor appetite, and diarrhea upon slight overeating. This was Spleen Earth deficiency failing to generate Lung Metal. Morning: Buzhong Yiqi Tang; Evening: Liujunzi Tang; occasionally supplemented with Bazhen Tang (Eight-Treasure Decoction) [miscellaneous consumptive disease]. Gradual recovery occurred after three months. Subsequently, external cold induced wheezing cough, abdominal distension, anorexia, and cold limbs. This was residual Middle Qi deficiency failing to nourish skin/hair and regulate interstitial opening/closing. Liujunzi Tang added with Shengjiang (Zingiberis Rhizoma Recens) and Jiegeng cured her.
Chu Yushi said: The Classics state: Mild cold causes cough; severe cold causes intestinal dysentery. Ancient prescriptions for cough were always based on warming herbs, such as Ganjiang (Zingiberis Rhizoma), Guixin (Cinnamomi Ramulus), and Xixin (Asari Radix et Rhizoma). Since cold pathogen enters the interior, only pungent and sweet herbs can disperse it. Thus, almost all coughs are due to cold. It also states: Heat in the Upper Jiao causes Lung萎 (Flaccidity) via cough. Excess leads to Lung Abscess; deficiency leads to Lung Flaccidity. These patients initially may have had blood deficiency, excessive alcohol/sex/flavor intake, or severe fluid depletion from purgatives, leading to internal burning dryness, Lung Metal injury, rapid pulse, fever, cough with pus/blood. At this stage, the condition is critical. Ancients also used warming herbs like Jianzhong Tang (Middle-Constructing Decoction). Modern practitioners seeing cold/heat cough often use Chaihu (Bupleuri Radix), Biejia (Testudinis Carapax et Plastrum), Mendong (Ophiopogonis Radix), Tingli (Semen Lepidii seu Descurainiae), suffering harm without realizing error—a deep warning. Even if warming herbs are contraindicated, decoctions/pills must be used skillfully with careful regulation, avoiding rashness that harms life.
[Xue] Danxi said: Yin-aspect cough mostly belongs to Yin deficiency. Lung distension with insomnia is difficult to treat. Lung Flaccidity focuses on tonifying Qi, nourishing Blood, and clearing Lung Metal. Excess Lung Qi should be drained, primarily with Sangbaipi (Mori Cortex), assisted by Banxia (Pinelliae Rhizoma) and Fuling (Poria). Drain excess, tonify deficiency. Lung dryness requires moistening; heat patterns use Niubangzi (Arctii Fructus), Jiegeng, Zhimu, and Jiziqing (Album Ovi Galli). Hoarseness indicates cold, treated with Xixin, Banxia, Shengjiang. Lung deficiency uses Ren Shen (Ginseng Radix et Rhizoma) and Ejiao (Colla Corii Asini) as main agents. Yin insufficiency uses Liuwei Dihuang Wan as essential, or Zhimu Fuling Tang. Yin-deficient asthma uses Si Wu Tang added with Chenpi (Citri Reticulatae Pericarpium) and Gancao (Glycyrrhizae Radix et Rhizoma) to descend Qi and nourish Yin. I consider the aforementioned cough with nasal congestion and heavy voice indicates Wind Pathogen injuring Lung, treated with Shen Su Yin (Ginseng and Perilla Decoction) [miscellaneous fever]. Red face with wheezing/cough indicates Fire overcoming Lung, treated with Renshen Pingfei San (Ginseng Lung-Pacifying Powder) [miscellaneous wheezing]. Alternating chills/fever indicates Liver Qi disharmony, treated with Sijunzi Tang added with Zhimu, Chaihu, Jiegeng. Cough/wheezing with shortness of breath indicates Lung deficiency, treated with Renshen Bufei Tang (Ginseng Lung-Tonifying Decoction) [wheezing/fullness]. Fatigue/poor appetite indicates Spleen deficiency, treated with Shen Zhu Tianshi Tang (Ginseng and Atractylodes Spleen-Tonifying Decoction) [this section]. Dry mouth/throat indicates deficient fire flaming upward, treated with Liuwei Wan. Generally, fever/wheezing/cough, or coughing pus/blood, anorexia: urgently tonify Spleen/Lung, nourish Kidney Water; many survive. Floating-large pulse with red face is difficult to treat; floating-short-choppy pulse is treatable.
The Classics state: Contracting cold mildly causes cough; severely causes diarrhea. Lung governs Qi and connects with skin/hair. Pathogen injuring skin/hair causes cough, a Lung disease, transmitting to other Zang organs according to seasonal susceptibility. Lung is a tender organ, easily injured and hard to treat. Cough types include Lung, Heart, Spleen, Kidney, Liver, Wind, Cold, Branch, Fluid, and Gallbladder—ten types. There is also Lao Sou (Consumptive Cough), called Xie Sou (Pathogenic Cough) by Hua Tuo and Zhu Sou (Infectious Cough) by Sun Simiao. Caused by excessive sex/alcohol, damaging Lung Meridian. Severe cases expectorate pus/blood; mild cases wax and wane. Sometimes hematemesis precedes cough, or cough precedes hemoptysis. This involves pathogen transmission. Sun Simiao used Tongqi Wan. Dreams of sexual intercourse with ghosts are treated with Siman Wan, Gejie (Geckus), Tianlinggai (Cranii Humanus), Taoliuzhi (Peach and Willow Branches), Anshixiang (Styrax). If worms in Lung cause throat itching/cough, use lozenge-formulated medicine to kill worms and stop cough.
[Xue] Zhang Zhongjing said: Cough with bilateral hypochondriac pain, inability to turn sides, and fullness belongs to Liver; use Xiao Chaihu Tang (Minor Bupleurum Decoction). Cough with bitter vomiting belongs to Gallbladder; use Huangqin Banxia Shengjiang Tang (Scutellaria, Pinellia, and Fresh Ginger Decoction). Cough with sensation of obstruction in throat, severe swelling/closure, belongs to Heart; use Jiegeng Tang (Platycodon Decoction). Cough with involuntary passage of gas belongs to Small Intestine; use Shaoyao Gancao Tang (Peony and Licorice Decoction). Cough with right hypochondriac pain radiating to shoulder/back, immobility worsening cough, belongs to Spleen; use Shengma Tang (Cimicifugae Decoction). Cough with vomiting, severe vomiting expelling roundworms, belongs to Stomach; use Wumei Wan (Mume Pill). Cough with wheezing sounds, severe hemoptysis, belongs to Lung; use Shengma Tang. Cough with urinary incontinence belongs to Large Intestine; use Mahuang Fuzi Xixin Tang (Ephedra, Aconite, and Asari Decoction); cough with bedwetting belongs to Bladder; use Fuling Banxia Tang (Poria and Pinellia Decoction). Persistent cough involving Triple Burner presents with abdominal fullness, anorexia, drooling, eye edema, rebellious Qi; use Yigong San (Yi Gong Powder). Correct application yields miraculous results. One woman with these symptoms had evening/internal fever, alternating chills/fever, thirst, night sweats, frequent urination, and irregular menstruation (every 2-3 months). This was Liver-Spleen Qi-Blood deficiency. Shizhen Tang (Eight-Treasure Decoction) and Liuwei Dihuang Wan for 60+ doses resolved symptoms gradually. Menstruation normalized to every 2 months; continued previous meds with Jiawei Xiaoyao San, 30+ doses each. Anger during menstruation caused excessive bleeding, recurrence of symptoms, poor appetite, abdominal distension, shortness of breath. Shiquan Dabu Tang (Complete Great Tonifying Decoction) for several doses improved condition; continued previous tonification. Loss of son caused chest/abdominal discomfort, poor appetite, internal fever, night sweats, hematochezia, insomnia. Jiawei Guipi Tang (Modified Restore the Spleen Decoction) combined with previous meds cured her. Another woman had intermittent fever/chills. Cooling herbs worsened heat, night sweats, dry mouth, burning feet, generalized heat, confusion/drunkenness, recovering slowly after heat subsided, or evening fever lasting till dawn. This was Yin-blood deficiency and Yang-Qi weakness. Morning: Liuwei Dihuang Wan ingredients; Evening: Shiquan Dabu Tang. After one month, symptoms slightly improved; added Buzhong Yiqi Tang. Recovered after two months.
