This is the disease of nausea and vomiting during pregnancy (Ezu), which must be provoked by anger; upon inquiry, it was indeed so. Since the Liver Qi has rebelled upwards and is carrying the fetal Qi together, the tonifying effects of Ginseng Radix (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) are highly inappropriate. She was instructed to take Yiqing Wan (Suppressing Blue Pill) with twenty-four pills at a time using Poria Cocos Decoction (Fuling Tang), for five doses, after which she felt slightly relieved. Upon examining her pulse, it showed a slight rapidity (Shu). She stated that her mouth was dry and bitter, and after eating a little congee, she felt sourness in her mouth. I surmised that the stagnant Qi in the diaphragm had not yet fully dispersed. I instructed her to decoct Chuanxiong Rhizoma (Chuanxiong), Citri Reticulatae Pericarpium (Chenpi), Gardeniae Fructus (Shanzhi), Poria Cocos (Fuling), and Zingiberis Rhizoma Recens (Shengjiang) as a decoction, taking fifteen pills of Yiqing Wan with it. After more than ten doses, the other symptoms subsided, but she could only eat half of her usual amount, felt sourness in her mouth after eating, and felt easily hungry when not eating. I told her that the Liver Heat had not yet settled, so she should take twenty pills of Yiqing Wan with plain boiled water. It took twenty days for her to settle. I examined her pulse again and found that although both hands' pulses were harmonious, the left hand's pulse was extremely weak. This indicated that the fetus would inevitably miscarry. At this time, since the Liver Qi had settled, Ginseng Radix (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) could now be used. Thus, I used the initial formula containing Ginseng Radix (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) to tonify, preventing deficiency after the miscarriage. After one month of服用 (taking medicine), the fetus naturally miscarried, yet she remained stable and uneventful. Yiqing Wan consists solely of Coptidis Rhizoma (Huanglian) made into pills.
[Daquan] Fetal movement insecurity during pregnancy arises from deficiency of the Chong and Ren meridians, resulting in an unstable conception. There are also cases caused by excessive drinking or sexual intercourse, damaging and disturbing the fetus. There are cases caused by accidental blows or contact leading to fetal movement. There are cases where joy, anger, or emotional stagnation damages the Heart and Liver, stirring the blood vessels. There are cases where patients believe doctors recommend warm tonification, but are instead harmed by the medication. There are cases where the mother's illness causes fetal movement; treating the mother's illness will naturally stabilize the fetus. There are cases where the fetus is insecure, moving and affecting the mother's health; stabilizing the fetus will naturally heal the mother. One should observe the mother's complexion and tongue: if the face is red and the tongue is cyanotic, the fetus is dead while the mother lives. If the face is cyanotic and the tongue is red, with foam coming from the mouth, the mother dies while the fetus lives. If the lips and mouth are cyanotic with foam flowing from both corners, both mother and fetus die.
[Xue Ji] For the aforementioned conditions involving stagnant fetal Qi, use Zi Su Yin (Perilla Decoction). For Spleen Qi deficiency, use Liu Jun Zi Tang (Six Gentlemen Decoction) added with Zi Su Ye (Perilla Leaf) and Zhi Ke (Aurantii Fructus). For stagnation injuring the Spleen, use Gui Pi Tang (Restore the Spleen Decoction) added with Chai Hu (Bupleuri Radix) and Shan Zhi (Gardeniae Fructus). For depression and anger injuring the Liver and Spleen, use Si Qi Tang (Four Words Decoction) added with Chuan Xiong (Chuanxiong Rhizoma) and Dang Gui (Angelicae Sinensis Radix). For anger stirring Liver Fire, use Jia Wei Xiao Chai Hu Tang (Modified Minor Bupleurum Decoction). If the fetus is already dead, urgently use Ping Wei San (Calm the Stomach Powder) added with Po Xiao (Natrii Sulfas) to rot and expel it.
Yong Gu Yun Tang (Eternal Pregnancy Preservation Decoction): Shu Di Huang (Prepared Rehmanniae Radix), Chuan Xiong (Chuanxiong Rhizoma), Huang Qin (Scutellariae Radix) (each 5 fen); Dang Gui Shen Wei (Danggui Root Tip and Tail), Ren Shen (Ginseng Radix), Bai Shao Yao (Paeoniae Radix Alba), Chen Pi (Citri Reticulatae Pericarpium) (each 1 qian); Bai Zhu (Atractylodis Macrocephalae Rhizoma) (1.5 qian); Gan Cao (Licorice Radix) (3 qian); Huang Bo (Phellodendri Cortex) (a little); Sang Shang Yang Shi Teng (Mulberry Twig Eaten by Sheep) (seven round leaves); Nuomi (Glutinous Rice) (14 grains). The above ingredients are chopped, decocted in water, and taken orally.
[Jin Gui Yao Lue] Women during pregnancy should commonly take Dang Gui San (Angelica Powder) as the main treatment.
Dang Gui San: Dang Gui (Angelicae Sinensis Radix), Huang Qin (Scutellariae Radix), Shao Yao (Paeoniae Radix), Chuan Xiong (Chuanxiong Rhizoma) (each 4 liang); Bai Zhu (Atractylodis Macrocephalae Rhizoma) (half jin). The above five medicines are pounded into powder, taken with wine using a square-inch spoon, twice daily. Commonly taking this during pregnancy facilitates easy delivery, ensures the fetus suffers no distress or disease, and treats various postpartum ailments.
For nourishing the fetus during pregnancy, Bai Zhu San (Atractylodes Powder) is the main treatment.
Bai Zhu San: Bai Zhu (Atractylodis Macrocephalae Rhizoma), Chuan Xiong (Chuanxiong Rhizoma) (each 1 liang); Shu Jiao (Sichuan Pepper) (7.5 qian, processed to remove sweat); Mu Li (Ostreae Concha) (half liang). The above four medicines are pounded into powder, taken with wine using a coin-sized spoon, three times daily, and once at night. If pain is severe, add Shao Yao (Paeoniae Radix). If there is severe pain in the epigastrium, double the amount of Chuan Xiong (Chuanxiong Rhizoma). If there is heart irritability and pain, vomiting, and inability to eat, add Xi Xin (Asari Radix et Rhizoma) 1 liang and Ban Xia (Pinelliae Rhizoma) twenty large pieces; afterwards, take Vinegar Grain Water. If vomiting occurs, take Vinegar Grain Water; if still unresolved, drink Wheat Juice. If thirsty afterwards, take Barley Porridge. Even if the disease is cured, do not stop taking the medicine.
[Ji Yan Fang] Treats fetal movement insecurity and abdominal pain with bleeding from the second or third month up to the eighth or ninth month.
Ai Ye (Artemisiae Argyi Folium), E Jiao (Colla Corii Asini), Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Chuanxiong Rhizoma) (each 3 liang); Gan Cao (Licorice Radix) (1 liang). The above are finely cut, decocted in eight sheng of water until three sheng remain, strained, and E Jiao (Colla Corii Asini) is added and melted. Divided into three doses, taken three times daily. [Qian Jin Fang], Wen Zhong, and [Bei Ji Fang] have the same formula.
Si Wu Tang (Four Substances Decoction) added with Shu Ai (Matured Mugwort Leaf), E Jiao (Colla Corii Asini), and Fu Ling (Poria Cocos) treats all types of fetal movement insecurity.
An Tai Dang Gui Tang (Fetus-Safeguarding Angelica Decoction) treats fetal movement insecurity due to sudden fright or movement in the fifth month of pregnancy, with bleeding in the lower abdomen, pain radiating to the waist and hypochondrium, painful urination, and vaginal bleeding. (See previous section on monthly fetal nourishment).
E Jiao San (Donkey-Hide Gelatin Powder) (two formulas) treats fetal movement insecurity due to falling or stumbling, with lower back pain, abdominal fullness, vaginal bleeding, or the fetus pressing upwards against the heart.
Shu Di Huang (Prepared Rehmanniae Radix) (2 liang); Bai Shao Yao (Paeoniae Radix Alba), Ai Ye (Artemisiae Argyi Folium), Dang Gui (Angelicae Sinensis Radix), Gan Cao (Licorice Radix), E Jiao (Colla Corii Asini), Huang Qi (Astragali Radix) (each 1 liang); one formula includes Chuan Xiong (Chuanxiong Rhizoma).
