Huangdi asked: What causes the trembling of the human body? Qibo replied: When the Stomach Qi is not substantial, the meridians become deficient. When the meridians are deficient, the sinews and vessels become lax and sagging. Laxity of the sinews and vessels leads to an excess of Yin during movement; after exertion, recovery fails to occur, thus resulting in trembling. Treatment should be based on the location of the pathology, applying tonification methods between the muscle layers (Fenroujian). The *Medical Outline* (*Yixue Gangmu*) states that tremor and convulsion are similar; convulsion involves traction of the limbs with extension or flexion, whereas tremor is merely shaking without extension or flexion. If there is Stomach deficiency with phlegm, use Ginseng Radix et Rhizoma (Renshen) and Atractylodis Macrocephalae Rhizoma (Baizhu) to tonify Qi, and Poria (Fuling) and Pinelliae Tuber (Banxia) to resolve phlegm; if there is excess heat with accumulation, apply the three methods (sweating, vomiting, purgation) of Zhang Hezi.
Xue Ji stated: Tremor is synonymous with falling and dizziness (Diaoxuan). The *I Ching* says: "Nothing moves the myriad things faster than Wind." "Gudong" implies movement. Generally, falling and dizziness result from the swaying motion of Liver-Wood. All wind diseases causing falling and dizziness belong to the Liver. Therapeutic methods: If there is Liver-Wood excess heat, use Xieqing Wan (from *Youke Gan*, discussing Liver disorders). If there is Liver-Wood deficiency heat, use Liuwei Wan (from *Zabing Xulao*, discussing deficiency consumptive diseases). If Lung-Metal overacts on Liver-Wood, use Xiebai San (from *Zabing Fare*, discussing fever). If Liver-Wood is weak, use Xiaoyao San (for regulating menstruation), adding Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), and Uncariae Ramulus cum Uncis (Gouteng). If Spleen Blood is deficient, use Liujunzi Tang (from *Zabing Xulao*), adding Ligusticum Chuanxiong Rhizoma (Chuanxiong) and Angelica Sinensis Radix (Danggui). If Stomach Qi is weak, use Buzhong Yiqi Tang (from *Nanbing Shang Laojuan*, discussing difficult diseases and fatigue-induced injury), adding Uncariae Ramulus cum Uncis (Gouteng). If tremor occurs postpartum, it is due to depletion of Qi and Blood, with hyperactive deficiency fire generating wind; it must not be treated merely as a wind syndrome, but requires strong tonification to avoid error. One woman had a irritable disposition, fever, irregular menstruation (either delayed or early), limb fatigue, poor appetite, and tremor. I diagnosed insufficient Spleen Qi, scanty Liver Blood, and hyperactive fire. In the morning, she took Tiaozhong Yiqi Tang added with Poria (Fuling) and Fritillariae Thunbergii Bulbus (Beimu), taken with Liuwei Wan; in the afternoon, she took Xiaoyao San taken with Liuwei Wan. She recovered after more than two months. Another woman suffered from body tremor and delirious speech, unresponsive to other medicines. I attributed it to depression and anger. Upon inquiry, it was found that she had harbored resentment towards her husband for a long time. She was given Xiao Chaihu Tang (from *Shanghan Shaoyang*, discussing Shaoyang disease), which provided slight relief, followed by Jiawei Guipi Tang (Guipi Tang added with Bupleuri Radix (Chaihu) and Gardeniae Fructus (Zhizi)), leading to recovery.
