Women whose menstruation is irregular due to sexual desire or improper sexual conduct have their pathology located in the Kidney Meridian. This syndrome is most common and should be carefully differentiated and treated.
Whenever desires are unfulfilled, leading to deep contemplation and accumulated depression, causing Qi stagnation in the Heart and Spleen, thereby damaging the source of the Chong and Ren vessels, the Kidney Qi will gradually diminish. Mild cases may present with menstruation occurring either early or late; severe cases may gradually develop into amenorrhea due to blood exhaustion. In such cases, treatment should simultaneously address the Heart, Spleen, and Kidney, primarily using formulas such as Xiaoyao Yin (Dissatisfaction Decoction) and Mi Yuan Jian (Secret Origin Decoction).
Xiaoyao Yin (see *New Formulas Eight Arrays · Array of Causes*) treats women who suffer from excessive worry and depression, injuring the source of the Heart, Spleen, Chong, and Ren vessels, causing daily depletion of Qi and Blood, gradually leading to menstrual irregularities.
Dang Gui (Angelicae Sinensis Radix) (2-3 qian), Bai Shao (Paeoniae Radix Alba) (1.5 qian), Shu Di Huang (Rehmanniae Radix Praeparata) (3-5 qian), Zao Ren (Ziziphi Spinosae Semen) (2 qian, stir-fried), Fu Shen (Poria Cum Radix Pini) (1.5 qian), Yuan Zhi (Polygalae Radix) (processed, 0.3-0.5 qian), Chen Pi (Citri Reticulatae Pericarpium) (0.8 fen), Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle) (1 qian).
Add two zhong of water, decoct until seven-tenths remain, take warm after meals. If Qi deficiency exists, add Ren Shen (Panacis Ginseng Radix) 1-2 qian; if menstruation is overdue accompanied by pain and stagnation, add Jiu Chao Xiang Fu (Cyperi Rhizoma prepared with wine) 1-2 qian.
Mi Yuan Jian (see section on Blood Deficiency Menstrual Irregularity).
If sexual fire is blazing, causing daily erosion of True Yin, Bao Yin Jian (Protect Yin Decoction) or Mi Yuan Jian should be used primarily.
Bao Yin Jian (see section on Heat in Blood Early Menstruation).
Mi Yuan Jian (see section on Blood Deficiency Menstrual Irregularity).
If sexual conduct is unrestrained and careless, it must damage the flow of the Chong and Ren vessels, causing Kidney Qi to fail to secure itself. Treatment must secure the Mingmen (Gate of Life), primarily using Gu Yin Jian (Secure Yin Decoction) or Mi Yuan Jian.
Gu Yin Jian (see *New Formulas Eight Arrays · Array of Security*) treats syndromes of Yin deficiency with slippery discharge, leukorrhea turbidity, stranguria with enuresis, and menstrual blood insecurity due to deficiency. This formula specifically targets the Liver and Kidney.
Ren Shen (Panacis Ginseng Radix) (dose according to suitability), Shu Di Huang (Rehmanniae Radix Praeparata) (3-5 qian), Shan Yao (Dioscoreae Rhizoma) (stir-fried, 2 qian), Shan Zhu Yu (Corni Fructus) (1.5 qian), Yuan Zhi (Polygalae Radix) (0.7 fen, stir-fried), Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle) (1-2 qian), Wu Wei Zi (Schisandrae Chinensis Fructus) (14 grains), Tu Si Zi (Cuscutae Semen) (stir-fried until fragrant, 2-3 qian).
Add two zhong of water, decoct until seven-tenths remain, take warm after meals. If deficiency slipperiness and seminal emission are severe, add Jin Ying Zi (Chrysobalani Fructus) flesh 2-3 qian, or Cu Chao Wen Ge (Meretrix Shell prepared with vinegar) 1 qian, or Wu Mei (Mume Fructus) 2 pieces.
If Yin deficiency with slight heat and insecure menstrual blood exist, add Xu Duan (Dipsaci Radix) 2 qian. If lower Jiao Yang deficiency is accompanied by abdominal pain and loose stools, add Bu Gu Zhi (Psoraleae Fructus) and Wu Zhu Yu (Evodiae Fructus) as suitable. If Liver-Kidney Blood deficiency causes lower abdominal pain and blood not returning to its vessel, add Dang Gui (Angelicae Sinensis Radix) 2-3 qian. If Spleen deficiency with dampness or accompanying nausea/vomiting exists, add Bai Zhu (Atractylodis Macrocephalae Rhizoma) 1-2 qian. If Qi sinking and insecurity exist, add Chao Sheng Ma (Ephedrae Caulis stir-fried) 1 qian. If accompanied by Heart deficiency insomnia or excessive sweating, add stir-fried Zao Ren (Ziziphi Spinosae Semen) 2 qian.
Mi Yuan Jian (see section on Blood Deficiency Menstrual Irregularity).
If Left Kidney True Yin is insufficient, causing menstrual irregularities, Zuo Gui Yin (Left Return Decoction), Zuo Gui Wan (Left Return Pill), or Liu Wei Di Huang Wan (Six-Ingredient Rehmannia Pill) should be used primarily.
Zuo Gui Yin (see *New Formulas Eight Arrays · Array of Tonification*) is a formula for strengthening Water (nourishing Yin). For any case where Mingmen Yin declines and Yang prevails, this formula should be modified and used. It is the main formula for Yi Yin Jian (One-Yin Decoction) and Si Yin Jian (Four-Yin Decoction).
Shu Di Huang (Rehmanniae Radix Praeparata) (2-3 qian, or increased to 1-2 liang), Shan Yao (Dioscoreae Rhizoma) (2 qian), Gou Qi (Lycii Fructus) (2 qian), Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle) (1 qian), Fu Ling (Poria) (1.5 qian), Shan Zhu Yu (Corni Fructus) (1-2 qian. Use less if sensitive to sour taste).
Add two zhong of water, decoct until seven-tenths remain, take after meals. If Lung heat with irritability exists, add Mai Dong (Ophiopogonis Radix) 2 qian; if Blood stagnation exists, add Dan Pi (Moutan Cortex) 2 qian; if Heart heat with restlessness exists, add Xuan Shen (Scrophulariae Radix) 2 qian; if Spleen heat with easy hunger exists, add Shao Yao (Paeoniae Radix) 2 qian; if Kidney heat with bone-steaming and sweating exists, add Di Gu Pi (Lycii Cortex) 2 qian. If Heat in Blood acts recklessly, add Sheng Di Huang (Rehmanniae Radix) 2-3 qian; if Yin deficiency with unrest exists, add Nv Zhen Zi (Ligustri Lucidi Fructus) 2 qian; if Upper Excess and Lower Deficiency exist, add Niu Xi (Achyranthis Bidentatae Radix) 2 qian to guide downward; if Blood deficiency with dryness and stagnation exists, add Dang Gui (Angelicae Sinensis Radix) 2 qian.
Zuo Gui Wan (see *New Formulas Eight Arrays · Array of Tonification*) treats insufficiency of True Yin and Kidney Water, failing to nourish Ying and Wei, gradually leading to weakness, or alternating deficiency heat, spontaneous sweating, night sweats, or Spirit not residing in its house, Blood not returning to its source, or deficiency injury damaging Yin, or involuntary seminal emission and stranguria, or Qi deficiency with dizziness, or blurred vision and deafness, or dry mouth and tongue, or soreness and weakness of waist and legs. For all syndromes of internal depletion of essence and fluids, it is urgent to strengthen the master of Water to cultivate the Original Yin of the Left Kidney, so that Essence and Blood will naturally fill up. This formula should be used primarily.
Da Huai Shu Di Huang (Rehmanniae Radix Praeparata) (8 liang), Shan Yao (Dioscoreae Rhizoma) (stir-fried, 4 liang), Gou Qi (Lycii Fructus) (4 liang), Shan Zhu Yu Rou (Corni Fructus Flesh) (4 liang), Chuan Niu Xi (Achyranthis Bidentatae Radix) (wine-washed and steamed, 3 liang, do not use for seminal emission), Tu Si Zi (Cuscutae Semen) (processed, 4 liang), Lu Jiao Jiao (Corvi Cornus Colla) (broken and stir-fried into beads, 4 liang), Gui Jiao Jiao (Testudinis Carapacis et Plastratis Colla) (chopped and stir-fried into beads, 4 liang, not needed if no fire).
First steam the Shu Di Huang until soft and mash into paste, add refined honey to make pills the size of paulownia seeds. Take over one hundred pills before meals with boiling water or light salt water.
If True Yin fails to secure and deficient Fire flares upward, use a pure Yin and extremely quiet formula. Remove Gou Qi and Lu Jiao Jiao from this base formula, add Nv Zhen Zi (Ligustri Lucidi Fructus) 3 liang and Mai Dong (Ophiopogonis Radix) 3 liang. If Fire scorches Lung Metal causing dryness and cough, add Bai He (Lilii Bulbus) 3 liang. If night heat with bone-steaming, add Di Gu Pi (Lycii Cortex) 3 liang. If urination is difficult and unclear, add Fu Ling (Poria) 3 liang. If stool is dry and constipated, remove Tu Si Zi and add Cong Rong (Cistanches Herba) 3 liang. If Qi deficiency, add Ren Shen (Panacis Ginseng Radix) 3-4 liang. If Blood deficiency with slight stagnation, add Dang Gui (Angelicae Sinensis Radix) 4 liang. If waist and knee soreness/pain, add Du Zhong (Eucommiae Cortex) 3 liang, stir-fried with salt water. If Zang-Fu are peaceful without fire but Kidney Qi is not filled, add Po Gu Zhi (Psoraleae Fructus) 3 liang, Qu Xin Lian Rou (Nelumbinis Semen without heart) and Hu Tao Rou (Juglandis Semen) each 4 liang; Gui Jiao Jiao is not needed. Since the Five Fluids are governed by the Kidney, all Yin-natured herbs can enter the Kidney. Those who say guidance techniques are necessary show poor understanding.
Liu Wei Di Huang Wan (see section on Menstrual Irregularity).
If Right Kidney True Yang is insufficient, causing menstrual irregularities, You Gui Yin (Right Return Decoction), You Gui Wan (Right Return Pill), or Ba Wei Di Huang Wan (Eight-Ingredient Rehmannia Pill) should be used primarily.
You Gui Yin (see *New Formulas Eight Arrays · Array of Tonification*) is a formula for benefiting Fire (warming Yang). For any case where Mingmen Yang declines and Yin prevails, this formula should be modified and used. This formula interacts with Da Bu Yuan Jian (Great Tonify Origin Decoction). For treating Yin excess repelling Yang, True Cold False Heat, etc., add Ze Xie (Alismatis Rhizoma) 2 qian. After decoction, immerse in cold water before taking; this is especially excellent.
Shu Di Huang (Rehmanniae Radix Praeparata) (2-3 qian, or increased to 1-2 liang), Shan Yao (Dioscoreae Rhizoma) (stir-fried, 2 qian), Shan Zhu Yu (Corni Fructus) (1 qian), Gou Qi (Lycii Fructus) (2 qian), Gan Cao (Glycyrrhizae Radix et Rhizoma) (prepared with honey, 1-2 qian), Du Zhong (Eucommiae Cortex) (ginger-processed, 2 qian), Rou Gui (Cinnamomi Cortex) (1-2 qian), Zhi Fu Zi (Aconiti Lateralis Radix Praeparata) (1-3 qian).
Add two zhong of water, decoct until seven-tenths remain, take warm after meals. If Qi deficiency and Blood collapse occur, or there is Jue (cold extremities), coma, sweating, vertigo, false mania, or shortness of breath, large doses of Ren Shen (Panacis Ginseng Radix) and Bai Zhu (Atractylodis Macrocephalae Rhizoma) must be added as suitable. If Fire decline fails to generate Earth, causing vomiting, hiccup, and acid regurgitation, add Pao Jiang (Zingiberis Rhizoma Praeparatum) 2-3 qian; if Yang decline with Middle Cold, diarrhea, and abdominal pain, add Ren Shen (Panacis Ginseng Radix) and Rou Dou Kou (Myristicae Semen) as suitable. If frequent lower abdominal pain, add Wu Zhu Yu (Evodiae Fructus) 0.5-0.7 qian. If stranguria and leukorrhea persist, add Po Gu Zhi (Psoraleae Fructus) 1 qian.
