导赤散、羌活散、皆可服;热甚者、小柴胡汤,渴甚者、白虎汤;大便秘甚数日不通者,宣风散一服便可,间服前药;

Lu Bosi states: The signs of chuangzhen (smallpox/rubella-like eruptions) include a dry face, red cheeks, and red eyelids; yawning with a dull expression; alternating sensations of cold and heat; coughing and sneezing; coldness in the fingertips of the hands and feet; restlessness and palpitations during sleep at night; excessive sleepiness accompanied by irritability; fever with facial redness, red lips, body pain, headache, and phlegm-saliva. These symptoms resemble those of Shanghan (Cold Damage syndrome), but they actually belong to Tianxing Shiqi (epidemic infectious diseases). At the onset, the condition may arise from Wind-Cold or Cold Damage pathogens, from epidemic Qi transmission, from food stagnation and vomiting, or from blood stasis caused by falls, fright, or anger.
Furthermore, it may present as toxin invading the eyes, locked jaw, convulsions resembling wind-stroke; or pain in the mouth, tongue, throat, abdomen, and umbilicus; or irritability, fever, facial redness, mania, stuffiness, lethargy, and delirious speech; or spontaneous sweating, diarrhea, fever, or absence of fever. The manifestations are diverse and difficult to distinguish quickly.
Ancient medical texts describe verifying the condition by checking if the ears, the area behind the ears (Yifeng region), and the feet are cold. This is because chuangzhen belongs to Yang syndromes, and the Kidney itself does not show direct pathogenic signs. Since the ears, the area behind the ears, and the feet are governed by the Kidney meridian, only these areas governed by the Kidney feel cold. In cases of doubt, this method is sometimes accurate and sometimes not. It is better to observe whether there are red meridians and red threads behind the ears; if present, it confirms true chuangzhen, allowing for verification. The pulse is typically surging (Hong), large, wiry (Xian), and rapid (Shu). During pulse diagnosis, the patient's body may tremble slightly, which is a sign of the disease.
The method of care and treatment dictates that neither diaphoresis (sweating) nor purgation (downward draining) should be used from beginning to end. Instead, one should use agents of moderate warmth and coolness, combining detoxification, harmonization of the Spleen and Stomach, and stabilization of the Exterior. Fever should not be abruptly suppressed; rather, it should be gently resolved. Without heat release, the eruption cannot emerge. For deficiency, tonify; for excess, reduce; for cold, warm; for heat, cool. This is the principle of balance. Metaphorically, it is like steaming in a bamboo basket: the key is to keep it loose and breathable. If one recklessly uses diaphoresis, Ying and Wei Qi will leak out, worsening ulceration. If one recklessly uses purgation, Zheng Qi (Upright Qi) will collapse internally, causing the pathogen to retreat to the Kidney, resulting in shivering and aversion to cold, heat in the ears and neck, closed eyes, abdominal distension, blackening and necrosis of the lesions, with a mortality rate of nearly ten out of ten. (If the tongue coating turns black, it is even harder to treat.) Why? Because chuangzhen manifests according to the Five Zang organs. Before eruption, signs of all five Zang organs may be present; after eruption, it localizes to one organ, reflecting the severity of toxin accumulation. Thus, Liver involvement presents as watery tears (tears flowing like water, small and bluish); Lung involvement presents as purulent nasal discharge (thick turbid snot, white and abundant); Heart involvement presents as macules (red blood spots, small); Spleen involvement presents as papules (yellowish-red color, small spots). Only when it returns to the Kidney does it turn black (bluish-purple, dry, and sunken). Chuangzhen belongs to Yang and originally has no Kidney syndrome (the Kidney is located below the Fu organs and does not accept turbid toxins, hence no typical symptoms). Yang corresponds to Fire, and the Kidney corresponds to Water. When Fire is active and controlled by Water, is this not dangerous?
Zhu Shi stated: Whether chuangzhen has erupted or not, one must never use unblocking or purgative drugs; this is the greatest taboo. From start to finish, chuangzhen must not be purged, nor should laxatives be used indiscriminately, as entering toxins into the interior will kill the patient. From this, we see that although chuangzhen symptoms resemble Shanghan, the treatment methods for rash/pox are fundamentally different from Shanghan. Shanghan transmits from the exterior to the interior, whereas chuangzhen erupts from the interior to the exterior. Since the toxin root lies within, purgation causes internal Qi deficiency, preventing toxin expulsion and causing it to sink inward, leading to Earth failing to control Water and resulting in black sinking lesions. If the toxin erupts on the surface, diaphoresis causes Ying and Wei Qi deficiency, further opening the pores and increasing ulceration, allowing Wind pathogens to invade due to deficiency and causing complications. Diaphoresis and purgation are deeply cautioned against by ancient masters. How should regulation be achieved? The answer is: Invigorate blood and regulate Qi, stabilize the Exterior and harmonize the Middle Jiao, use light and clear detoxifying agents that combine warmth and coolness. Warm agents include Huangqi (Astragali Radix), Danggui (Angelicae Sinensis Radix), Muxiang (Aucklandiae Radix); cool agents include Qianhu (Peucedani Radix), Gengegen (Puerariae Lobatae Radix), Shengma (Cimicifugae Rhizoma). Assisted by Chuanxiong (Ligustici Chuanxiong Rhizoma), Shaoyao (Paeoniae Radix), Zhike (Aurantii Fructus), Jiegeng (Platycodonis Radix), Qianghuo (Notopterygii Rhizoma et Radix), Mutong (Akebiae Caulis), Zicao (Arnebiae Radix seu Lithospermi Radix), Gancao (Glycyrrhizae Radix et Rhizoma), etc., regulation can be achieved.
Whenever a child feels feverish with symptoms resembling Shanghan, but no pox/rash is visible and the diagnosis is uncertain, first administer Xingxing San (Wonderful Powder), Baolong Wan (Dragon-Holding Pill), or Shengsu San (Perilla-Ascending Powder). If heat is severe, first give Shengma Tang (Cimicifuga Decoction), or add Chuanxiong, Zisu (Perillae Folium), and Fuling (Poria). For vomiting, use Xiangsu Yin (Perilla Infusion) with Banxia (Pinelliae Rhizoma), Fuling, and Baishaoyao (White Peony Root). For diarrhea, use Xiangsu Yin with Baizhu (Atractylodis Macrocephalae Rhizoma) and Fuling. For vomiting/diarrhea with Stomach deficiency, use Chen’s Muxiang San (Aucklandia Powder). For thirst, use Shengma Tang with Zisu and Fuling. For convulsions and delirium with severe heat, administer Zhibao Dan (Precious Treasure Pill) and Baolong Wan together. These can be taken if the eruption has not fully emerged; if already fully erupted through the skin, they should not be taken. Fangfeng Tianma Wan (Saposhnikovia and Gastrodia Pill) may also be used. If rashes appear on days one or two as red dots like hemp seeds, varying in size, it is mild; if they appear all at once densely, it is severe. Administer Sisheng San (Four Saints Powder) in both cases. For those with diarrhea, use Zicao Muxiang Tang (Arnebia and Aucklandia Decoction) or Sijunzi Tang (Four Gentlemen Decoction) with Muxiang and Dingxiang (Caryophylli Flos). For those with chills but incomplete eruption, add Guigui (Cinnamomi Cortex). For those with fright-heat and complete eruption, treat with Kuaiban San (Rapid Macule Powder), which contains Chantui (Cicadae Periostracum). On days three or four, if eruption is incomplete, use Sisheng San. For cold skin, add Muxiang and Rougui (Cinnamomi Cortex), and additionally use Huoxue San (Invigorate Blood Powder), administered with warm wine and Zicao Tang (Arnebia Decoction) to assist efficacy, promoting faster eruption. For pain relief, take with warm water. For purple/black sinking lesions with internal heat and constipation, use Longdan Gao (Gentian Ointment): mix Zhu Xin Xue (Porcinis Cor), Chensha (Cinnabaris), and Naobi (Borneolum Syntheticum) into fine powder, and take with Muxiang Tang. Alternatively, Zhuwei Gao (Pig Tail Ointment) may be used. Do not use lightly if there is no severe internal heat. Sprinkling Huisu Jiu (Coriander Wine) around the room and on the body is beneficial, but avoid spraying the face to ward off evil Qi. Burning Cangzhu (Atractylodis Rhizoma) and Jiangzhen Xiang (Daemonorops Resina) is also good. On days five or six, if pox are very painful, only administer Shaoyao Gancao Tang (Peony and Licorice Decoction) to relieve pain; multiple doses are acceptable. Subsequently, administer Ganlu Yin (Sweet Dew Infusion). If there are pus sores in the mouth, swollen/painful throat, and inability to nurse/eat, treat accordingly. On days seven or eight, when lesions are transparent and slightly ulcerated, Dihuang Tang (Rehmannia Decoction) may be taken. For constipation, Xiaodu Yin (Detoxification Infusion) is suitable. Mild cases have red, vibrant, glossy roots; severe cases have damaged, sunken, dense, grayish-white lesions with diarrhea and thirst. For cases without fever, Siwu Tang (Four Substance Decoction) with Maidong (Ophiopogonis Radix), Fangfeng (Saposhnikoviae Radix), and Chantui may be used. On days nine or ten, if heat is severe, Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction) or Xiaodu Yin may be used, provided bowels/bladder are open. For urinary obstruction, modify Dalianqiao Tang (Large Garden Balsam Pod Decoction) until urination resumes; Daochi San (Guide Out Red Powder) and Qianghuo San are also applicable. For severe heat, use Xiaochaihu Tang (Minor Bupleurum Decoction); for severe thirst, use Baihu Tang (White Tiger Decoction). For severe constipation lasting several days, Xuanfeng San (Disperse Wind Powder) once is sufficient, interspersed with previous medicines. Mild cases have plump, full, waxy-colored pox; severe cases have chills, confusion, abdominal distension, irritability, thirst, panting, and teeth-grinding. On days eleven or twelve, pox should crust over and scabs fall off, indicating recovery. If between crusting stages, unable to crust, or presenting with abdominal distension, irritability/thirst, or diarrhea/shortness of breath, do NOT give honey water, melons, or fruits (cold nature); ingestion leads to death. Instead, administer Muxiang San or Yigong San (Extraordinary Efficacy Powder) with modifications. Generally, children who develop convulsions first then pox survive if pox heal then convulsions recur with vomiting/diarrhea (fatal). Those who have fever then pox survive; those with persistent diarrhea after pox eruption perish. Smooth progression involves sudden discharge of pus/blood/scabs after healing; adverse progression involves abdominal sores, bloody diarrhea with undigested food (Spleen/Stomach deficiency), or bloody diarrhea with necrotic non-pus-filled lesions (Stomach rot). Note that Yangming governs muscles; Stomach Qi must not be weak for a day. Protect the eyes first when treating pox/rash; apply Chensha or Hongzhi (Vermilion) to eye orbits, adding Honghua (Carthami Flos) for blood invigoration. If pox enter the eyes, use Jueming San (Cassia Seed Powder) or Zi Bei San (Purple Shell Powder). For stubborn scabs, moisten with Su You (Butyrum Susinum) or White Honey; remove if possible. Delayed removal leads to scars; apply scar-removal ointments. Strictly forbid chicken/duck eggs (causes blindness, pupil turning white like egg white); forbid pork. Avoid Qian Yi’s Baixiang Wan (Hundred Auspicious Pills) for purgation or human tooth preparations; instead, use Dusheng San (Single Saint Powder) and Niuli Gao (Cow Plum Ointment). If pox are injured by Wind-Cold causing Ying/Wei disharmony, or by retained food causing internal Qi blockage and hardening, use Jie Diao San (Regulating and Harmonizing Powder). For persistent diarrhea, use Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) with Muxiang, or Sijunzi Tang with Muxiang and Doukou (Cardamomi Fructus). Although black pox are malignant, proper treatment allows survival. Care failures—nurses eating forbidden foods transmitting toxins, irregular feeding causing hunger/weakness, inadequate clothing causing Wind-Cold invasion, or emotional/environmental triggers—lead to hardening, deterioration, and Kidney involvement. This is self-inflicted harm. Proper care involves warming dispelling decoctions, nursing mothers drinking them, protecting from Wind-Cold, ensuring balanced Qi. Diet should prevent over-fullness (excess Qi) or starvation (deficient Qi). Chuang is called "Holy Rash": fever and eruption on day 7, drying on day 14, recovery on day 21. If one treats excess as excess and deficiency as deficiency, damaging what is insufficient and benefiting what is redundant, eruption becomes difficult. If Qi/Blood are regulated, Spleen/Stomach warmed, and Cold/Heat balanced, eruption is easy. This is the ultimate method of regulation. For early rash sensation with fever before eruption, Bian Que’s Oil Method reduces incidence or resolves it naturally, ensuring safety.
