Gan syndrome (Pediatric Malnutrition Syndrome) comprises five types, with distinct etiologies, all resulting from irregular diet and excessive consumption of rich, sweet, and greasy foods. In adults, this condition is termed Lao (Consumptive Disease), while in children, it is called Gan; they share similar symptoms but differ in name. Adult Lao disease often originates from sexual exhaustion, primarily affecting the Kidney Meridian; whereas pediatric Gan disease is caused by dietary injury, primarily affecting the Spleen and Stomach. Deficiency and damage to the Spleen and Stomach lead to the depletion of body fluids. Over time, the disease transmits between organs, causing damage to all five Zang organs. Adult Lao presents with emaciated bones and depleted Qi; pediatric Gan presents with a distended abdomen and weakened spirit. Initially, it may stem from eating raw or meaty foods too early in the evening; or indulging excessively in rich, sweet, and greasy foods; or consuming large amounts of cold and raw foods, causing stagnation in the Zhongwan region; or improper regulation of warmth and cold; or breastfeeding after maternal sexual activity; or transmission of Lao qi from mother to child, among various other causes. Manifestations of Gan include: shiny and tight scalp, scorched and sparse hair, shrunken cheeks and dry nose, dry mouth and cracked lips, dim and rotting eyes, rubbing nose and eyebrows, emaciated muscles, sour-smelling diarrhea, urine as white as rice water, abdominal borborygmus and pain, tidal fever, formation of Zheng Pi (abdominal masses), nail-biting, craving for cold water, or pica such as eating charcoal, rice, dirt, tea, cloth, salty or sour fruits, etc. Such symptoms are all signs of Gan. The so-called "Five Gans" differ in external progression but internally relate to the five Zang organs.
First is Liver Gan, also known as Wind Gan. Symptoms include head shaking and eye rubbing, white membrane obscuring the pupil, profuse sweating over the body, preference for sleeping face down, dry and itchy eyes, upright and scorched hair, visible blue veins and hot brain, abdominal accumulation, frequent dysentery, and gradual emaciation.
Second is Heart Gan, also known as Startled Gan. Symptoms include high fever throughout the body, red cheeks and yellow complexion, irritability in the heart and diaphragm, sores in the mouth and tongue, dark and scanty urination, night sweats with frequent fright, dysentery with pus and blood, or pure purple-black decaying blood, and loss of vitality.
Third is Spleen Gan, also known as Food Gan. Symptoms include dull yellow skin, distended abdomen with thin legs, prominent blue veins on the belly, shortness of breath due to Qi deficiency, crying with vomiting, inability to digest food and water, sour-smelling diarrhea, reduced appetite, craving for dirt, unhappiness, aversion to light preferring darkness, emaciated muscles and withered form.
Fourth is Lung Gan, also known as Qi Gan. Symptoms include coughing and rebellious Qi, scorched and dry skin and hair, gooseflesh with aversion to cold, nasal erosion with runny nose, ulcerated chin with hemoptysis, throat discomfort, loose stools, and irregular diarrhea and dysentery.
Fifth is Kidney Gan, also known as Urgent Gan. Symptoms include upper heat and lower cold, alternating chills and fever, gum sores, scorched ears and chest heat, cold extremities, slippery diarrhea with cold limbs, black nails and sallow complexion, numerous sores and scabies on the body, hemorrhoids in the lower part, rectal prolapse that does not retract, gradual exhaustion, and bone weakness leading to collapse.
Although the five Gans have different origins, they transmit to one another, eventually damaging all five Zang organs. The Classics state the five levels of damage: First damage affects skin and hair, where skin gathers and hair falls; second damage affects muscles, where muscles waste away and food cannot nourish the flesh; third damage affects blood vessels, where vessels become deficient and cannot nourish the six Fu organs and five Zang organs; fourth damage affects sinews, where sinews relax and cannot support the body; fifth damage affects bones, where bone weakness prevents rising from bed, which is fatal. The five Gans also involve five parasites: Liver parasite tails scatter like horsehair; Heart parasite is a blood leech; Lung parasite is tapeworm, feeding between the lungs; Spleen parasite is Enterobius vermicularis (pinworm); Kidney parasite is like fine hair, feeding downwards into the anus. If untreated for long, each parasite attaches to its corresponding Zang organ, consuming essence and blood, depleting Yuan Qi; when essence and blood are exhausted, death ensues.
There is also Ding Xi Breastfeeding Gan, which corresponds to modern smallpox. It refers to a situation where a pregnant mother's milk is sucked by the infant; the infant drinking this milk causes muscle wasting, sallow complexion, distended abdomen with blue veins, constant diarrhea, scorched hair, and symptoms resembling Gan, though it is not true Gan. It is named so because the fetus inherits from the placenta, and the child inherits from the prenatal state. Though severe, it is not fatal; weaning and regulation will allow the child to recover naturally after birth.
Another view describes Fen Liu Gan (Powder Tumor Gan). Initially, a nodule like a bean appears behind the neck, movable upon pressure, soft and painless, growing larger day by day. Parasites inside resemble white stars in flour powder. If not promptly ruptured, the parasites disperse with heat flow, invading organs, causing widespread sores, ulcers, scabies, and leprosy, rendering it incurable.
Another view describes Wu Gu Gan (Innocent Gan). This occurs when clothes worn by children are sun-dried but not collected at night. A bird named "Wu Gu" leaves filth and feathers on the clothes. When the child wears these clothes, this syndrome develops. This statement is considered superstitious; such birds may exist in Fujian and Guangdong regions, recorded here for reference.
