The symptoms of diarrhea are recorded in medical classics as having five types. The so-called "Five Diarrheas" refer to: Wei (Stomach) Diarrhea, Pi (Spleen) Diarrhea, Dachang (Large Intestine) Diarrhea, Dajia Xie (similar to dysentery), and Xiaochang (Small Intestine) Diarrhea. The manifestation of Wei (Stomach) Diarrhea is: food and drink are not digested, and the stool color is yellow; the manifestation of Pi (Spleen) Diarrhea is: abdominal distension and fullness, watery diarrhea, and vomiting after eating; the manifestation of Dachang (Large Intestine) Diarrhea is: urgency and discomfort after eating, loud bowel sounds with severe pain, and white-colored stool; the manifestation of Dajia Xie (Dysentery-like condition) is: tenesmus (urgent desire to defecate but difficult passage), frequent trips to the toilet with little output, and lower abdominal pain; the manifestation of Xiaochang (Small Intestine) Diarrhea is: pus and blood in the stool during urination, lower abdominal pain, and pain in the urethra. Although there are five names, the causes of diarrhea are not limited to one. Some cases arise from Spleen and Stomach deficiency, where food cannot be transported and transformed; some arise from excessive dietary injury, where food stagnation accumulates; some arise from long-term Yin conditions activating dampness; some arise from improper regulation of cold and heat; and some arise from a wet nurse being ill, causing the infant to suckle hot milk. For those caused by Spleen deficiency, warm tonification is used; for those caused by food retention, digestion-promoting and guiding methods are used; for those caused by accumulation, purgative methods are used according to the severity; for those caused by dampness, acrid dispersing and leaching methods are used; for those caused by cold and heat, regulation is adjusted according to seasonal changes; and for those caused by a sick wet nurse, her condition is diagnosed and treated accordingly. At the onset of diarrhea, tonifying agents should not be used immediately, lest the retained food fail to digest and cause other complications. If the syndrome presents with the "Five Deficiencies" (pulse thready, cold extremities, shortness of breath, diarrhea/dysentery, and anorexia), it is considered difficult to treat.
