Ms. Sun Wenyue, aged eighty-three, suffered from epigastric pain (stomach region pain) so severe that she could not bear touch. Although her abdomen felt hungry, she could not swallow food or drink. The pulse at the Cun and Guan positions on both wrists was slippery and large, while the Chi position was deep and weak. This is a syndrome of blood deficiency with qi stagnation. Initially, Jiqi Wan (Accumulated Qi Pill) was administered; after one dose, the pain decreased by half. Subsequently, raw Baishao Yao (Radix Paeoniae Alba), Shanzhizi (Fructus Gardeniae), and Wulingzhi (Faeces Trogopterori) each one qian, wine-fried Baishao Yao two qian, Fencacao (Radix Glycyrrhizae Preparata), Shanzha (Fructus Crataegi), and Xiangfu (Rhizoma Cyperi) each eight fen were prescribed. After one decoction, she recovered completely.
The younger male cousin of Yingzhang suffered from San Yin Malaria (Three-Yin Malaria) occurring on Zi, Wu, Mao, and You days, persisting for four months. Each episode occurred at night, with more heat and less chills. The left pulse was slightly wiry, and the right Guan pulse was slippery and large. Erchen Tang (Two-Aged Decoction) was modified with Chaihu (Radix Bupleuri), Huangbai (Cortex Phellodendri), Chuanxiong (Rhizoma Chuanxiong), Danggui (Radix Angelicae Sinensis), and Huanglian (Rhizoma Coptidis). After two doses, the heat subsided slightly, but appetite remained poor, limbs were lethargic, indicating significant Spleen Qi deficiency. Baizhu (Rhizoma Atractylodis Macrocephalae) and Heshouwu (Radix Polygoni Multiflori Preparata) each three qian, Biejia (Carapax et Plastrum Testudinis) two qian, Qingpi (Exocarpium Citri Reticulatae Viride) seven fen, and Wumei (Fructus Mume) one piece were prescribed. After one dose, the malaria was arrested.
Cheng Haowu, a student of the Imperial Academy, was a free-spirited and erudite scholar. In late spring, he suffered from bilateral Taiyang headache, slight chest and hypochondriac pain, thirst, and watery diarrhea. The left pulse was floating, wiry, and rapid; pressing the middle position revealed strength; the right Guan pulse was slippery and large. I diagnosed this as Chun Wen Bing (Spring-Warmth Disease). Chaihu (Radix Bupleuri), Qianhu (Radix Peucedani), Gegen (Radix Puerariae), Fencacao (Radix Glycyrrhizae Preparata), Qingpi (Exocarpium Citri Reticulatae Viride), Huangqin (Radix Scutellariae), Zhimu (Cortex Anemarrhenae), Jiegeng (Radix Platycodonis), Banxia Qu (Fermented Rhizoma Pinelliae), and Shigao (Gypsum Fibrosum) were prescribed. After midnight, he sweated slightly. Upon rising to defecate, he caught wind and fever recurred, accompanied by persistent thirst and increased irritability. Shigao (Gypsum Fibrosum) three qian, Zhimu (Cortex Anemarrhenae) and Chaihu (Radix Bupleuri) each two qian, Gegen (Radix Puerariae) and Huangqin (Radix Scutellariae) each one qian, Fencacao (Radix Glycyrrhizae Preparata) and Jiegeng (Radix Platycodonis) each five fen, and Zhu Ye (Folium Bambusae) twenty leaves were prescribed. After two successive doses, sweat emerged, heat resolved, and all symptoms subsided. His limbs remained fatigued, mouth slightly dry, and speech lacked strength. Sheng Mai Tang (Generate the Pulse Decoction, likely referring to Shengmai San) was modified with Yiyiren (Semen Coicis), Shihu (Caulis Dendrobii), Gancao (Radix Glycyrrhizae), Baishao Yao (Radix Paeoniae Alba), and Huangqin (Radix Scutellariae) for recuperation until he returned to normal.
The wife of Changqing, my daughter-in-law, experienced mild abdominal pain, irregular menstruation, sore throat, numbness and trembling of the four limbs, and loss of appetite. The right pulse was surging and large like a pea. Chuanxiong (Rhizoma Chuanxiong), Xiangfu (Rhizoma Cyperi), Maiya (Fructus Hordei Germinatus), Shanzha (Fructus Crataegi), Wumei (Fructus Mume), Fencacao (Radix Glycyrrhizae Preparata), Jiegeng (Radix Platycodonis), Jiu Qin (Wine-fried Radix Scutellariae), Fangfeng (Radix Saposhnikoviae), Jingjie (Herba Schizonepetae), Baizhu (Rhizoma Atractylodis Macrocephalae), and Fuling (Poria) were prescribed for four doses, bringing relief. During the next menstrual cycle, bleeding was heavy and lasted ten days. Huangqi (Radix Astragali), Ejiao (Colla Corii Asini), and Puhuang (Pollinis Typhae) each one qian, Baishao Yao (Radix Paeoniae Alba) two qian, and Fencacao (Radix Glycyrrhizae Preparata) three fen were prescribed. After one dose, bleeding stopped. Thereafter, she only felt a sensation of turbid qi sinking downward, with flatulence passing through the vagina (Zihu). Butong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was modified with wine-fried Huanglian (Rhizoma Coptidis) for regulation until she recovered.
