The eyelids feel hot and the eyes ache as if rubbed with sand; this is due to blood stasis and spleen heat latent in the Liver system. If eye pain is accompanied by headache and hard eyelids, with tearing, dryness, and blurred vision, the condition may evolve into other manifestations.
Regardless of whether there is an opacity (corneal clouding), this syndrome is diagnosed when both eyelids are hard and the eyes ache. If accompanied by headache, the condition is particularly urgent. This arises from wind-heat invading the Liver, leading to Liver deficiency and blood insufficiency, which fails to nourish and moisten the eyes. Lacking yin fluid for nourishment, fire pathogenic factors enter vacuously instead, combining with phlegm, dryness, dampness, and heat. Symptoms may include a sensation of tight binding around the head, blood stagnation within the Spleen, and eye pain caused by the connection with fire. Mild cases may develop internal pepper-like sores (trachoma or follicular conjunctivitis); severe cases may present with eyelid swelling like a cup, or blood stasis penetrating the eyeball. Treatment should involve applying external medications and turning up the eyelids to open pathways. If the eyelids are too hard to turn up, or if headache and brain distension do not subside, this indicates that head-wind is about to transform into toxicity. It is appropriate to take:
Erzhu San (Two Atractylodes Powder): Treats hard eyelids and eye pain, removing opacities.
Ingredients include: Cicada Slough (Chantui, 蝉蜕) (remove head and feet), Gentian Root (Longdan Cao, 龙胆草) (wine-washed, stir-fried), Coptis Rhizome (Huanglian, 黄连) (wine-washed, stir-fried), Goji Berry (Gouqizi, 枸杞子) (baked dry), Atractylodes Rhizome (Cangzhu, 苍术) (soaked in rice water, stir-fried), Lycium Bark (Digupi, 地骨皮), White Atractylodes Rhizome (Baizhu, 白术) (earth-stir-fried), Moutan Cortex (Mudanpi, 牡丹皮) (equal parts).
Grind the above ingredients into fine powder. Take one qian per dose, mixed with Schizonepeta Decoction after meals.
Zhong Gao (Swelling Paste):
Ingredients include: Calomel (Nifen, 腻粉) (a small amount), Yellow Beeswax (Huangla, 黄蜡), Red Hematite (Daizheshi, 代赭石) (5 qian each, ground), Fine Porcelain Dust (Xicimo, 细瓷末), Phellodendron Bark (Huangbai, 黄柏) (fine powder), Sesame Oil (Mayou, 麻油) (2 liang each).
Grind the above ingredients into extremely fine powder, place in a copper spoon, add oil and wax, and boil together to form a paste. Apply to the hard eyelids.
Those suffering from alternating chills and fever usually have wind pathogens externally lodged in the couli (interstices of skin and hair follicles), and phlegm-fluid internally infiltrating the zang-fu organs, resulting in insufficient qi and blood, and alternating excess of yin and yang. When yang is in excess, fever occurs; when yin is in excess, chills occur. When yin and yang contend, and pathogenic factors interfere with healthy qi, alternating chills and fever occur, appearing and disappearing intermittently. However, this syndrome resembles malaria but differs in that chills do not cause shivering, and fever does not cause mental restlessness.
"Red pain like evil spirits often attracts chills and fever demons; failing to distinguish between wind-cold and malaria leads to inappropriate use of cooling agents, causing lower deficiency and upper excess, internal urgency and external laxity. All stem from external heat disturbing the body; treatment must aim for harmony." This passage specifically discusses eye diseases characterized by redness and pain, headache, and alternating chills and fever, resembling wind-cold malaria. When the disease manifests, eye pain may occur several times a year in mild cases, or frequently in severe cases, unlike sudden-onset conditions such as wind-stroke and guest-heat (acute conjunctivitis). The root cause lies in Liver and Kidney deficiency; deficiency of both leads to internal heat, and yin deficiency causes ministerial fire to move. The "chills" result from deficiency and damage of Ying and Wei qi, making the couli loose and insecure, thus feeling cold. If mistaken for wind-heat malaria or phlegm, and treated with harsh, acrid-hot medicines, blood will become more deficient, and the disease will deepen. It is appropriate to take:
Shizhen Tang (Ten Precious Decoction): Treats deficiency damage leading to blood exhaustion, eye pain caused by qi and blood attacking upward; it nourishes yin, descends fire, nourishes blood, and clears the Liver.
Ingredients include: Rehmannia Root (Shengdihuang, 生地黄) (wine-washed, 2 qian), Angelica Sinensis (Danggui, 当归) (wine-washed, 1.5 qian), White Peony Root (Baishaoyao, 白芍药) (stir-fried), Lycium Bark (Digupi, 地骨皮) (stir-fried), Anemarrhena Rhizome (Zhimu, 知母) (mixed with salt and wine, stir-fried), Moutan Cortex (Danpi, 丹皮) (soaked in child's urine, stir-fried), Asparagus Tuber (Tianmendong, 天门冬) (remove heart), Ophiopogon Tuber (Maimendong, 麦门冬) (remove heart, 1.5 qian each), Ginseng (Renshen, 人参) (remove rhizome), Licorice Root Tip (Gancaoshao, 甘草梢) (0.5 qian each).
Cut the herbs into pieces. Add two zhong of water, decoct until eight fen remains, filter out dregs, and take warm.
In cases of yin deficiency, internal fire invariably arises. Bitter-cold drugs that directly drain fire are only suitable at the very beginning of the disease, when yuan qi is not yet deficient, the pathogenic force is vigorous, and the pulse is full and rapid, temporarily to treat the branch symptoms. After a short time, when deficiency sets in, they should not be taken. Wang Taipu said: "If heat treatment is incomplete and middle-jiao cold arises, damaging the Foot-Taiyin Spleen, the source of Lung Metal's generation is severed." Therefore, Rehmannia is used as the sovereign herb, Anemarrhena as the assistant, to strengthen Tianyi Water (Kidney Water) to control Bing-Ding Fire (Heart Fire), rather than contending directly with the fire. Angelica and White Peony nourish the Jueyin Liver channel, representing the method of treating Kidney and Liver simultaneously. When water is deficient, fire flares; thus, Lycium Bark and Moutan Cortex are used to restrain the fire. When fire is excessive, Metal weakens; thus, Asparagus and Ophiopogon serve as a barrier. Ginseng tonifies the mother of Lung Metal (Spleen Earth); raw Licorice is used to execute orders and assist in the treatment.
Jiu Diao Xigan San (Wine-Mixed Liver-Clearing Powder): Treats excess heat qi attacking the eyes, causing severe pain without fixed timing.
Ingredients include: Scrophularia Root (Heixuanshen, 黑玄参), Rhubarb (Dahuang, 大黄), Scutellaria Root (Huangqin, 黄芩), Gardenia Seed (Shanzhiren, 山栀仁) (stir-fried), Rehmannia Root (Shengdihuang, 生地黄), Anemarrhena Rhizome (Zhimu, 知母), Platycodon Root (Jiegeng, 桔梗), Angelica Sinensis Tail (Dangguwei, 当归尾), Mirabilite (Yuanmingfen, 元明粉) (equal parts).
Grind the above ingredients into fine powder. Take 2-3 qian per dose, mixed with warm wine far after meals, twice daily.
