The pulse should be fine and small. It should not be floating, slippery, or large. It should not wiry and rapid. If the body is cold, there is hope for survival; if the body is hot, it indicates death.
Dysentery (Li Ji), known in ancient times as "Zhi Xia" (Stagnant Downward Flow), is mostly caused by contracting wind-cold, summer-heat, or dampness, as well as irregular diet that damages the Spleen and Stomach, leading to the accumulation and stagnation of old food residues.
Its symptoms include a sense of urgency and heaviness in the rectum (Li Ji Hou Zhong - tenesmus with a feeling of incomplete evacuation), frequent trips to the toilet with little stool passed, abdominal pain. The discharge may be white, red, mixed red and white, pure bright blood, like bean soup, like fish brain, mixed with pus and blood, or like leaking roof water. This reflects the severity of the pathogenic invasion and the depth of the accumulation. Damp-heat stagnating in the Blood level manifests as red; in the Qi level as white. Mixed red and white indicates both Qi and Blood are affected by pathogens. Although there are two colors, ultimately there is no distinction between cold and heat; all are treated as damp-heat. One must distinguish between new and chronic cases and adjust medication based on the condition of the Original Qi (Yuan Qi). For initial dysentery where Original Qi is not yet deficient, purgation is necessary. If unresolved after purgation, treat according to symptoms. For dysentery that has persisted for some time, purgation is contraindicated as the Stomach Qi is failing. Dysentery is mostly heat-patterns, but can also be deficiency or cold. Deficiency patterns require tonification; cold patterns require warming. Elderly and weak individuals should not undergo purgation. Incomplete evacuation with a lack of sensation indicates Blood deficiency; frequent urges with inability to pass stool indicate Qi deficiency. Danxi stated: Red dysentery belongs to Blood and originates from the Small Intestine; white dysentery belongs to Qi and originates from the Large Intestine.
Discharge resembling fish brain (semi-raw, semi-cooked) is difficult to treat; discharge resembling leaking roof water indicates certain death.
Discharge resembling dust or rotting matter indicates certain death; pure black discharge indicates death.
Discharge flowing straight out like water from a bamboo tube indicates death; pure red discharge is difficult to treat.
Body heat with a large pulse indicates death. These are general statements. Complete cessation of urination indicates the exhaustion of Stomach Qi.
Zhen Guo Jiang Jun Wan (State-Securing General Pill) is the master formula for treating dysentery.
Take one jin (approx. 500g) of high-quality Jinwen Rhubarb (Da Huang), sliced thinly, divided into four parts.
One part is mixed with juice from one liang (approx. 37g) of Chuan Lian Lian (Coptidis Rhizoma, Huanglian) (deveined), sliced and soaked in water. Stir-fry with Da Huang until dry.
One part is mixed with juice from one liang of Wu Zhu Yu (Evodiae Fructus) (stalks removed), soaked in water. Stir-fry with Da Huang until dry.
One part is mixed with human breast milk. Stir-fry with Da Huang until dry.
One part is mixed with urine from an uncircumcised boy (Tong Bian). Stir-fry with Da Huang until dry.
Combine all four parts, grind into fine powder, and make pills using rice paste prepared with wine, sized like wu zi (Sophorae Semen). Half of the pills are steamed three times and sun-dried three times; the other half are simply sun-dried. Store separately.
For white dysentery, take 30 pills (half raw, half cooked) washed down with Wu Zhu Yu decoction.
For red dysentery, take 30 pills washed down with Huanglian decoction.
For mixed red and white dysentery, take 30 pills washed down with a decoction of Wu Zhu Yu and Huanglian.
Regarding the onset of mixed red and white dysentery with stagnant accumulation, tenesmus, frequent defecation with little output, and abdominal pain, use this formula first to purge.
