The infant is congenitally affected by various pathogenic heat. Heat stagnates in the Sanjiao (the Triple Burner, referring to the upper, middle, and lower cavities), resulting in Chong She (Double Tongue, swelling under the tongue). It may also manifest as E Kou (Goose Mouth, oral thrush), a worrying condition. Treatment requires not only Tuina but also acupuncture to pierce the lesion.
Chong She occurs beneath the tongue, protruding like a second tongue, hence the name. The collateral vessels of the Spleen connect to the sides of the tongue; those of the Liver connect to the root of the tongue; and those of the Heart connect to the base of the tongue. If these three meridians are invaded by damp-heat or wind-cold pathogens, the tongue may curl and retract, extend and lengthen, or become swollen and full. Mu She (Wooden Tongue) presents with a swollen, stiff tongue that hinders suckling and eating, indicating an excess of Wind-Heat. Since the tongue is the sprout of the Heart, Heart-Heat causes sores and rupture; Liver stagnation causes bleeding like a gush; Spleen obstruction leads to white coating resembling clouds. Heat causes swelling and fullness; Wind causes stiffness like wood. The mouth remains closed, limbs are hot, there is panting and slurred speech; these are its clinical manifestations.
Treatment Method: Push San Guan (Three Passages, to warm Yang and dispel cold), Clear Xin Jing (Heart Meridian), Clear Pi Jing (Spleen Meridian) (100 repetitions each), Tui Liu Fu (Push Six Yin, to clear heat and cool blood), Yun Ba Gua (Rotate Eight Trigrams), Yun Shui Ru Tu (Move Water into Earth, to strengthen Spleen and aid transport), Fen Yin Yang (Divide Yin and Yang, to harmonize Qi and Blood), Qing Tian He Shui (Clear Heavenly River Water, to clear heat and release the exterior).
E Kou typically affects infants from one month to over six months old. Suddenly, white debris fills the mouth and covers the tongue, rising up to obstruct breathing and swallowing, resembling a goose's mouth, remaining open and unable to close, with loss of voice, and difficulty in suckling and feeding. It may also appear on the upper and lower gums, known as Ma Ya (Horse Teeth, epidermal pearls). Although named differently, all are caused by toxic heat attacking upward, and their treatment principles are identical.
Treatment Method: Push San Guan, Tui Liu Fu (100 repetitions each), Fen Yin Yang, Lao Ming Yue (Retrieve Bright Moon, to calm the spirit and settle fright), Da Ma Guo Tian He (Beat Horse Across Heavenly River, to purge heat and drain fire).
Additionally, use the blunt end of a silver hairpin to scrape the gums until bleeding occurs, wipe clean with soft silk, and apply ancient ink.
Night crying involves four syndromes, with Fright being one. Crying without tears when seeing a lamp indicates Heat and restlessness. A blueish complexion on the cheeks and pain below the navel, with sudden crying during sleep, indicates disturbance of the Spirit.
There are four types of night crying: Fright-Heat, Heart-Heat, Cold Hernia, and Accidental Contact with Spirits. Fright-Heat results from excessive clothing and bedding causing overheating, allowing pathogenic heat to attack the Heart. Since the Heart and Small Intestine are interior-exteriorly related, night crying accompanied by enuresis characterizes this type. Heart-Heat is characterized by intensified crying upon seeing light. Cold Hernia is characterized by crying triggered by cold. Accidental Contact with Spirits presents with a purplish-black complexion, Qi stagnation resembling anger, apparent fear, and startle jumps during sleep.
Treatment Method: Push San Guan (50 times), Tui Liu Fu (120 times), Clear Xin Jing (100 times), Lao Ming Yue, Fen Yin Yang, Tao Dan Jing (Excavate Gallbladder Meridian).
For night crying due to pain from Cold Hernia, it is appropriate to Yun Si Heng Wen (Transport Four Horizontal Lines, to regulate Spleen/Stomach and resolve accumulation), Roup Qi (Knead the Navel), and Rou Yi Wo Feng (Knead Yi Wo Feng, to warm the middle and dispel cold for pain relief).
