Observe the child's sleep: if the breath from the mouth is warm, or if the child sleeps with their face pressed against the pillow, and exhibits upward rolling of the eyes and grinding of teeth, these are all signs of Heart Heat (Xin Re). The primary formula for treatment is Daochi San (Guide Out Red Powder).
When there is heat in the Qi of the Heart, the chest and heart region also become hot. The child may wish to speak but cannot, and shows a desire to seek coldness; therefore, they prefer sleeping with their face down.
In cases of Excess Heat in the Heart Qi, the movement of Qi becomes obstructed as it travels up and down. Sleeping face down further impedes the free flow of Qi. Therefore, the child prefers supine position (lying on the back), which allows the Qi to circulate freely up and down. The primary formula for treatment is Xiexin Tang (Drain the Heart Decoction).
Children are inherently weak and timid. If this stems from incomplete prenatal Qi formation, their Shen Spirit (Shen) will be deficient. Manifestations include excessive white sclera, open fontanelles (unossified cranial sutures), and a pale complexion. Such conditions are difficult to nurture; even if they survive to adulthood, their lifespan rarely exceeds 64 years (eight times eight). If parents indulge excessively in sexual activity, leading to inadequate prenatal essence, the child may not survive past forty years of age. Some may develop Kidney Deficiency due to illness, but this is not the root cause described here. Furthermore, when Kidney Qi is insufficient, the body tends to curl inward and descend ("cuan"), because the bones feel heavy and wish to fall downward, causing the body to shrink. The Kidney belongs to Water and is Yin; Kidney Deficiency leads to photophobia (fear of light). All such cases should be treated by tonifying the Kidney, using Dihuang Wan (Rehmannia Pill) as the primary formula.
The left cheek corresponds to the Liver, the right cheek to the Lung, the forehead to the Heart, the nose to the Spleen, and the chin to the Kidney. Redness indicates Heat; treatment should follow the specific syndrome.
Redness of the face indicates Heart Heat; treat with Daochi San (Guide Out Red Powder).
Pale redness indicates Deficient Heat in the Heart; treat with Shengxi San (Fresh Rhinoceros Horn Powder).
Bluish-green color indicates Liver Heat; treat with Xieqing Wan (Drain the Green Pill). If the color is faint, apply tonification.
Yellowish color indicates Spleen Heat; treat with Xiehuang San (Drain the Yellow Powder).
Lack of luster in the eyes indicates Kidney Deficiency; treat with Dihuang Wan (Rehmannia Pill).
Liver diseases often manifest in autumn. (Alternatively, some texts state "at dusk," i.e., 3–5 PM). When Liver Qi is strong and overacts upon the Lung, and Lung Qi is weak and unable to control the Liver, one should tonify the Spleen and Lung while treating the Liver. Tonifying the Spleen follows the principle that "the mother makes the son full" (tonifying the Spleen, the Mother organ, strengthens the Lung, the Child organ). Use Yihuang San (Benefit the Yellow Powder) to tonify the Spleen; use Xieqing Wan (Drain the Green Pill) to treat the Liver.
Lung diseases often manifest in spring. (Alternatively, some texts state "in the morning"). When Lung Qi is strong and overacts upon the Liver, one should tonify the Liver and Kidney while treating the Lung. The weakness of the Liver indicates it has been affected by disease. Use Dihuang Wan (Rehmannia Pill) to tonify the Liver and Kidney; use Xiebai San (Drain the White Powder) to treat the Lung.
Frequent blinking without convulsions, if progressing to convulsions, is caused by Heart Heat. Treat the Liver with Xieqing Wan (Drain the Green Pill); treat the Heart with Daochi San (Guide Out Red Powder).
Staring straight ahead without convulsions, if progressing to convulsions, is caused by Heart Heat. Treat the Liver with Xieqing Wan (Drain the Green Pill); treat the Heart with Daochi San (Guide Out Red Powder).
