(Note: "Nai Pi" refers to this condition, attached below)
*Shenghui Fang* discusses: The so-called pediatric *Ru Pi* (Milk Lump), arises because the nursing mother has irregular eating and drinking habits, consumes alcohol or eats to excess and immediately nurses the infant without knowing moderation. The infant's spleen and stomach are weak and tender, unable to digest the milk; or if the mother lies on one side while nursing, the infant cannot turn over, causing the infant to fall asleep with milk stagnating in the hypochondrium. Consequently, it knots into a mass and causes pain; this is called *Ru Pi*. Its manifestations include a bluish-yellow complexion, intermittent high fever, vomiting of milk with excessive sleepiness, sores inside the mouth, gradual emaciation and yellowish skin, and an unresolving abdominal mass, hence the name *Ru Pi*.
Mr. Mao discusses: After birth, the infant suddenly presents with cold sensations throughout the body, generalized yellow edema, red-yellow urine, and slight steaming heat. This condition originates from fetal heat, compounded by the mother's affection after crying, leading to immediate feeding that causes indigestion, thus forming a milk accumulation. Treatment should first use *Niuhuang Gao* (Ointment containing Calculus Bovis, see section on Diaphragm Heat), *Tianzhuhuang San* (Powder of Calamus and Bamboo Resin, see section on Excess Heat), and *Tiaozhong Yin* (Decoction for Regulating the Middle, see section on Disharmony of Stomach Qi). Taking these three medicines will lead to recovery. If yellow color enters the eyes, the eyes stare fixedly, there is asthma-like breathing resembling a fish's mouth, and the hands are clenched, these are ominous signs indicating untreatability.
Qian Yi discusses: There is a lump in the abdomen; the child does not eat solid food but only drinks milk. One should gradually use *Baibing Zi* (White Pill) to purge it.
*Yingtong Baojian* discusses: Pediatric *Nai Pi* (Milk Lump) refers to infants who have not yet learned to eat solid food and only drink milk, resulting in overwhelming milk flow. Or when the mother is irritated, she nurses her son immediately; the son drinks, the milk becomes congested and undigested, mixing with stubborn phlegm to form a痞块 (mass) in the abdomen, sometimes causing abdominal pain. This is *Nai Pi*.
*Yu Jue* Song for Pediatric Milk Lump Accumulation Injury Syndrome:
The milk spleen lump forms due to injury from accumulation;
Abdominal distension, blue veins, yellow complexion.
Fever comes at times, or thirst for drinking;
Malnutrition injures spleen heat, sores appear in the mouth.
This ailment should first be resolved with purgatives, then the accumulation removed, followed by regulating stomach qi, and no error will occur.
Another version states: First use *Xingxing Wan* (Wonderful Incense Pill) to purge, then use *Yinbai San* (Silver-White Powder) to regulate qi. (Both formulas are seen in this section.)
*Shenghui Fang* treats pediatric *Ru Pi* with firmness, or stagnant evil accumulation that does not dissipate, causing the child to become emaciated day by day, with a sallow complexion, frequent in spring and summer, loss of appetite. Formula for Jing Sanleng San:
Jing Sanleng (Spatholobi Caulis, lightly roasted and chopped) Chuan Dahuang (Rhei Radix et Rhizoma, chopped and lightly fried) Binglang (Arecae Semen) Biejia (Testudinis Carapax et Plastrum, coated with vinegar and roasted until yellow, skirt removed) Chi Fuling (Poria Cocos with Red Mycelium, each half liang) Zhike (Aurantii Fructus Immaturus, bran-roasted until slightly yellow, pith removed, one fen)
Crush the above herbs, sift into powder. Take one qian per dose, decoct with one small cup of water until five fen remains, remove dregs, divide into two doses, take three to four times daily. Effectiveness is indicated by the discharge of foul matter.
*Shenghui Fang* treats pediatric *Ru Pi* with high fever and emaciation; suitable to take Zhusha Wan:
Zhusha (Cinnabaris, half liang, finely ground and washed with water) Xionghuang (Realgar, finely ground, one qian and half) Hanshuishi (Gypsum Fibrosum) Longnao (Borneolum Syntheticum, each finely ground) Nifen (Calomelas) Binglang (Arecae Semen, powdered, each one qian)
Grind all ingredients evenly, mix with refined honey to make pills the size of mung beans. For children aged two or three, administer three pills with ginger soup, twice daily. Adjust according to the child's size.
*Shenghui Fang* treats pediatric *Ru Pi* with hot palms and soles, bluish-yellow complexion, refusal of milk and food, gradual emaciation. Formula for Renshen Wan:
