[Background Information]
Book: Youyou Xinshu (New Book for Young Children)
Author: Liu Fang
Dynasty: Song
Chapter: Volume 31 / Chong Dong Di Yi (First on Worm Movement)
Category: Comprehensive
(Three Worms and Hernia) Total of nine sections.
*Chishi Bingyuan* (Source of Diseases by Master Chao) on the syndrome of three worms in children: The so-called "three worms" refer to Changchong (Long Worm), Chichong (Red Worm), and Naochong (Pinworm). They are part of the count of nine worms. Changchong is Ascaris lumbricoides (roundworm), about one chi long; when active, it causes vomiting of clear fluid and heart pain, and if it pierces the heart, death ensues. Chichong resembles raw meat; when active, it causes borborygmus (intestinal rumbling). Naochong is extremely minute, resembling a vegetable worm, residing in the intestines; in large numbers, it causes hemorrhoids, and in severe cases, leprosy-like conditions, often arising from skin sores. It is involved in various conditions such as carbuncles, ulcers, tinea, scabies, dental caries, and lice. These are three of the nine worms, now given separate names because these three tend to cause disease independently, hence called the "Three Worms."
*Waitai Miyao* (Essential Prescriptions Worth a Thousand Gold for Emergencies) citing *Zhouhou Beiji Fang* (Emergency Prescriptions Worth Keeping at the Sleeve): The three worms refer to Changchong, Chichong, and Naochong. In fact, there are nine types, among which Naochong and Cunbai Chong (Tapeworm) are commonly seen in patients. Cunbai Chong results from consuming beef and drinking white wine; if it connects into a strand one chi long, it can be fatal. Medicinal treatment to expel worms requires the worms to rot internally and then be expelled; complete expulsion is ideal. If the worm breaks, it may regenerate, necessitating rapid removal. Naochong is mostly suffered by children, though adults may also suffer from it; symptoms include heart pain, morning vomiting of saliva, and restlessness. Other various worms are present in everyone's stomach; medicinal treatment is suitable for their removal.
Qian Yi discusses worm colic: Pale complexion, heart and abdominal pain, vomiting of foam and clear fluid, pain occurring at specific times. Treat with Anchong San (Stabilize Worm Powder) (Formula see within this section). Children with inherently timid constitutions are prone to this disease. Accumulation colic, food colic, and deficiency colic are largely similar with minor differences. Only worm colic presents with a bland taste in the mouth and spontaneous salivation; treatment follows the specific syndrome.
Qian Yi discusses the similarity between worm colic and epilepsy: Children with inherently timid constitutions have weak and cold stomachs, causing worm movement and heart pain, which slightly resembles epilepsy. However, the eyes do not deviate, and hands do not convulse. Treat with Anchong San (Stabilize Worm Powder) (Formula see within this section).
Qian Yi records a case of fatal worm colic: Ms. Xin’s daughter, aged five, suffered from worm colic. Various physicians treated her with Banli (Radix Euphorbiae Lathyri) and Qi (Lac Chinense) without effect. By the fifth day, she cried frequently, moved her body up and down restlessly, slept poorly, pressed her heart and abdomen herself, and screamed occasionally. Her facial color was irregular, shifting between blue, yellow, white, and black; her eyes lacked luster and were slow; her lips were white with foaming. On the sixth day, her chest protruded, and she could not sleep peacefully. Qian Yi examined her carefully and prescribed Wuyi San (Melia Powder) three doses. Seeing that the bluish hue around the eyes persisted, he exclaimed in alarm: "This illness is critically severe. If diarrhea occurs, it will be a counter-indication." The next day, Qian Yi observed that Ms. Xin experienced diarrhea at the third watch of the night. Looking into the chamber pot, the excrement resembled medicinal juice; stirring it revealed pill fragments. Qian Yi said: "This child has thick flesh and should have robust Qi, yet the syndrome indicates deficiency. It is untreatable." Why? The teacher stated: "Spleen deficiency and stomach cold cause worm movement. Now the eyes are blue, indicating Liver Wood overacting on Spleen Earth. Furthermore, the Spleen suffers diarrhea, indicating extreme Qi depletion. The pills passing with feces indicate the collapse of Stomach and Spleen Qi. Additionally, the physical state does not match the disease severity, thus I know it is a fatal case." Five days later, she became comatose and died on the seventh day.
Zhang Huan notes: When a child’s Stomach Qi is disharmonious for a long time, worms naturally move restlessly. Severe abdominal pain caused by worms involves them moving up and down, with pain having intervals, resembling wind syndromes. In children older than six or seven years, if not caused by vomiting or diarrhea, it is mostly due to consuming sweet and fatty foods. If worms attack the heart, it leads to premature death; this is no minor ailment, requiring utmost caution.
*Qianjin Fang* (Prescriptions Worth a Thousand Gold) formula for treating three worms in children.
Leiwan (Polyporus Umbellatus) and Xiong (Ligusticum Wallichii)
Grind the above two ingredients in equal parts into powder. Take one qianbi per dose, twice daily.

Place the above four ingredients in a copper vessel. Decoct over low heat, stirring until thick. Add flour to allow coagulation for pill formation. Place on earth to cool until warm. Add egg white, stir well, and decoct again until pill-forming consistency is reached. Fast overnight. Take one hundred twenty pills the size of small beans with water; fifty pills for children. Effective.
*Waitai Miyao* citing *Zhouhou Beiji Fang* formula for treating three worms.
Zhuyu Gen (Root of Evodia Rutaecarpa, select east-facing ones finger-thick and one chi long), Gualou (Trichosanthes Kirilowii Root, four liang, sliced)
Finely chop the Zhuyu Gen. Soak in one sheng of wine overnight. Strain in the morning. Fast overnight. In the morning, eat some dried meat first, then take the decoction in one dose. Divide into two or three doses for children. Also treats Cunbai Chong (Tapeworm).

Grind and sieve the above seven ingredients. Mix with honey to form pills the size of small beans. Fast overnight. Take one dose in the morning with Jiangshui (fermented rice liquid); eat lunch only after taking the medicine. Worms will be immediately expelled. A man suffered from abdominal distension, yellow complexion, and craving for meat. After taking this medicine, he expelled a Red Worm like a bamboo shoot one chi long, with over a hundred heads and eyes, and recovered. Another case: Xie Qiu from Jiujiang had an accumulation under the ribs the size of a cup, hardness in the lower abdomen, migrating fixed pain, nausea, excessive saliva, constant epigastric pain, and craving for meat. After taking this medicine, he expelled over two hundred headless worms, red-bodied with mouths and tails, and recovered. Another case: Chen Gong from Jiujiang suffered from abdominal distension and fullness, constantly craving raw vegetables. After taking this medicine, he expelled a White Worm as large as an arm and over a hundred smaller ones, recovering immediately. Pregnant women must not take this.
*Tujing Bencao* (Illustrated Materia Medica) main indications: Pediatric Wugu (Innocent) Phthisis, Scrofula, alternating cold and fever with abdominal distension, killing worms.
Decoct the seedlings and seeds of Kudan (Clerodendrum cyrtophyllum) in water and drink. Alternatively, crush fresh and squeeze out juice to drink. Alternatively, grind and apply topically for pediatric Shanpi (flash phthisis). (Grows in old ruins and wall ditches, two to three chi high; fruit is horn-shaped like a pouch containing pearl-like seeds, turning red when ripe.)
*Tan Shi Shusheng Fang*: Child with chest oppression, coarse breathing, dry lips, and limb weakness. Frequent vomiting of milk and food, spleen and stomach distension.
