(Startling and Lockjaw Diseases) There are twelve categories in total.
Mr. Wang from Handong discusses eleven types of pediatric convulsive syndrome (Xiao'er Jingfeng, a condition characterized by convulsions and staring in children) that can be cured:
Abnormal eye symptoms indicate the pathogen has entered the Liver (Gan). How do we know it is in the Liver? The Liver governs the sinews; when the Liver is affected by pathogens, the eyes twitch. If the eyes are red, this is an external manifestation of the Liver; thus, abnormal eye symptoms must be accompanied by green stools. Grinding teeth in dreams indicates the pathogen has entered the Kidney (Shen). How do we know it is in the Kidney? The Kidney governs bones and teeth. Itchy teeth and night-time grinding (night being Yin) indicate the pathogen has entered the Kidney. Crying at night until dawn indicates the pathogen has entered the Small Intestine (Xiaochang). How do we know it is in the Small Intestine? The Small Intestine is the Fu organ of the Heart. The Heart belongs to Fire (Bing-Ding in the South); Yin and Yang mutually overcome each other, so the pathogen enters the Small Intestine and stops at dawn. Day is Yang, night is Yin; their mutual overcoming causes crying. A blueish-white face with white stool/urine indicates the pathogen has entered the Gallbladder (Dan). How do we know it is in the Gallbladder? The Liver belongs to Wood and its color is blue; the Gallbladder is the Fu organ of the Liver; thus, a blue face and white excreta indicate the pathogen is in the Gallbladder, and vomiting of milk is inevitable. (White excreta refer to white urine and stool. Generally, a blue face indicates a cold pattern, affecting both urine and stool.) Panting and craving water indicate the pathogen has entered the Lung (Fei). How do we know it is in the Lung? The Lung governs Qi; startled and damaged Lung Qi causes panting. Craving water indicates deficiency-heat in the Lung; thus, the pathogen is in the Lung, causing thirst. Red cheeks indicate the pathogen has entered the Heart. How do we know it has entered the Heart? The Heart belongs to Fire and governs blood, externally manifesting in the cheeks; thus, red cheeks indicate the pathogen has entered the Heart. Weak and fine breathing indicates the pathogen has entered the Liver. How do we know it has entered the Liver? This panting actually involves the Lung. The Liver belongs to Wood, and the Lung to Metal; thus, we know Yin has injured Yang. This person will likely have green stools and restless twitching of hands and feet during sleep. To treat startling, purgative methods should be used, such as Xi Gan Wan (Liver-Washing Pill). Heat in the five centers (palms, soles, and chest) indicates the pathogen has entered the Spleen (Pi). How do we know it has entered the Spleen? The Stomach and Spleen both correspond to Earth; the Stomach governs the four limbs; thus, the pathogen enters the Spleen. The Stomach is the Fu organ of the Spleen. This person will likely experience intermittent fever after vomiting. A sawing sound in the throat indicates the pathogen has entered the Large Intestine (Dachang). How do we know it has entered the Large Intestine? The Large Intestine is the Fu organ of the Lung. The Lung is the canopy of the Zang organs and governs Qi; when the pathogen enters the Large Intestine, it rushes upward to the throat, making noise. Without phlegm, we know the pathogen is in the Large Intestine. Purging with medicines for startling and accumulation is appropriate. Spontaneous dry heaving indicates the pathogen has entered the Stomach. How do we know it has entered the Stomach? The Stomach is below the throat and governs the digestion of grain. When food is startled and wind-pathogen enters, it causes itching and frequent dry heaving; thus, we know it is in the Stomach. It is appropriate to regulate Stomach Qi first before administering anti-convulsant medicines. Startled crying during sleep and thirst indicate the pathogen is in the Sanjiao (Triple Burner). How do we know it is in the Sanjiao? The Sanjiao has no specific shape but consists of fatty membranes. When asleep, if the Upper Jiao is disturbed by startling pathogens, the child wakes up crying; thus, we know the disease lies in the Sanjiao. It will recover naturally after some days. Only anti-startling medicines are appropriate.
Mr. Wang from Handong also discusses seven types of pediatric convulsive syndrome that cannot be cured:
Convulsive syndrome with black fingernails cannot be cured. Fingernails are governed by the Liver; Liver exhaustion fails to nourish them. Black fingernails indicate blood exhaustion; without nourishment, fingernails turn black, and ten out of ten cases die. Convulsive syndrome with black blood in stool cannot be cured; this indicates Heart exhaustion. The Heart governs blood; Heart exhaustion fails to nourish the blood; this person will not survive a week. How do we know? The Heart is the master of the five Zang organs. If the master exhausts, death occurs within one day. Convulsive syndrome with excessive daytime sweating cannot be cured. Why? Sweat is governed by Qi; Qi is linked to Wei-Qi (Defensive Qi). Wei-Qi exhaustion fails to control Qi, leading to incessant sweating and death within a few days. Convulsive syndrome with sudden strange sounds cannot be cured. This is the sound of Lung exhaustion, where breath exits but does not return. The Lung also governs voice; Lung exhaustion means the voice goes and does not return; death occurs in three days. Convulsive syndrome with biting people cannot be cured. Biting indicates bone exhaustion. Bones are manifested in the teeth. Bone exhaustion causes itchy teeth, leading to biting; death occurs in about seven days. Because damage originates internally and manifests slowly, it takes seven days. Convulsive syndrome with half-open, half-closed eyes cannot be cured; this is called Kidney exhaustion. The Kidney is the root of the five Zang organs, externally manifesting in the eyes. Swollen black eyes with no light or strength to open/close indicate Kidney exhaustion; death occurs in four days. How do we know? The Kidney belongs to the North and the lower body; the disease rises from below; thus, death occurs in four days. Convulsive syndrome with dry, black mouth and nose cannot be cured; this indicates Spleen exhaustion. How do we know Spleen exhaustion? The Spleen governs fluids; Spleen exhaustion means all fluids are gone; thus, Spleen exhaustion is confirmed; death occurs in two days, typically at Yin-Mao hours (5 AM - 9 AM), because Wood overcomes Earth. All convulsive syndrome patients present with high fever and startle calls while lying down. Pi-Feng (Spleen Wind) often results from vomiting and involves wind-phlegm. Ji-Jing Feng (Acute Convulsive Syndrome) is caused by internal and external pathogens. Man-Jing Feng (Chronic Convulsive Syndrome) is caused by deficiency accumulations, fetal startling, and fetal heat.
Qian Yi discusses the error of using witchcraft: The son of Prince Wang, aged five, had fixed staring eyes and refused to eat. Someone claimed it was caused by spirits and invited a wizard to burn paper money; the illness did not improve. Qian Yi was summoned and said, "Why seek gods for diseases of the Zang and Fu?" Qian Yi prescribed Xi Gan Wan (Liver-Washing Pill), and the child recovered. (Formula see in the Startling Heat section.)
*Ying Tong Bao Jian* discusses: Pediatric convulsive syndrome presents with body heat, incessant startled crying, restless sleep, slight twitching of hands and feet, refusal to nurse, or symptoms resembling Taiyang disease (Exterior Syndrome).
Yang Daye, Hanlin Daizhao, discusses why infants under 100 days are difficult to nurture: Their gastrointestinal tract is immature, and their Shen-Qi (Spirit-Qi) is weak and timid, making movements prone to startling. This is likely due to the mother consuming wind-inducing, sticky, slippery, and greasy foods, roasted meats, etc., during pregnancy, combined with frequent anger, loud speaking, and vigorous intercourse shaking the meridians; or uncleanliness from the midwife; or wind entering the limbs; or improper wrapping after bathing, allowing floating heat to draw evil substances into the Spleen and Stomach, gradually dispersing into the six Fu and five Zang organs. Once toxic vapors enter, Ying and Wei (Nutritive and Defensive) Qi become imbalanced. Initially, frequent vomiting and abdominal distension are common; yawning and spitting occur often; false startling during sleep happens; or limbs become flaccid and eyes roll back. Investigating the source, how could this happen suddenly?
*Bao Sheng Lun* discusses pediatric startling signs (pulse is rapid and urgent): Startling is caused by the Kidney receiving shock. Shock-heat Qi flows into the Heart. The Heart is the Emperor and does not accept disturbance; once disturbed, wind patterns arise. (*Su Wen* states: The Kidney governs anger; when a child is startled, they show bluish face and yawning.) A bluish face in a child indicates startling. For the first instance of startling, look at the left side of the Taiyang area; a blue vein facing the eye indicates this. (For males, left; for females, right.) For the second instance, a blue vein on the Shan Gen (root of the nose) indicates this. For the third instance, a blue vein appearing on the lower eyelid connected to the Jin Kui (golden enclosure, referring to the inner canthus) indicates this. Blue veins in all three areas indicate extreme startling. This governs children who startle and throw things in sleep, fear people, grind teeth incessantly, have saliva in the throat, whole-body spasms, hand-foot convulsions, vomiting, protruding tongue, reaching out to grab people, and incessant crying. All these are signs of startling. If a child has excess wind-heat, startling phlegm flows into the Liver and Heart, causing sudden upward gaze, acute hand-foot convulsions, screaming, suffocation, and sudden death; this is named Ji Feng Ji Hou (Acute Wind Disease Pattern), and Ya Xian Ru Xiang San (Phlegm-Suppressing Dragon's Blood Powder) should be given twice (see this section), followed by Sheng Yin Wan (Raw Silver Pill) three times (same formula as *Ji Shi Jia Chuan*, see Acute/Chronic Convulsive Syndrome section) to purge startling phlegm. If a child has deficient and thin Zang-Fu, startling phlegm floods the Pericardium and Collaterals, causing slow eye movement, slight hand-foot movement, rattling saliva in the throat, and no body heat; this is Man-Jing Feng (Chronic Convulsive Syndrome) pattern, and Sheng Yin Wan should be given, followed by Zhen Xin Wan (Heart-Calming Pill). (Zhen Xin Wan formula is same as *Feng Sui Jing*, see this section.) If there are signs of death in convulsive syndrome, the pulse is wiry and large, the fish-mouth opens, breathing is coarse, the throat sounds like a saw, the neck is soft and weak, the legs are straight, the fontanelle is swollen or sunken, the eyes seem open but closed, stool resembles bloody dysentery, and the body is soft and weak; all these are signs of death, and medication cannot save them.
*Fei Xian Lun* discusses pediatric disease patterns: Abnormal upward staring of eyes in children is Liver startling. Grinding teeth in dreams is Bone startling. Night crying until dawn is Liver startling entering the Lung. Craving to bite the nipple is startling carrying evil Qi. A blue-white face is convulsive syndrome. Heat in the front/back chest and limbs is startling. Abnormal red face is startling entering the Heart. An upper black face with crying is convulsive syndrome face. Fine panting in the mouth is startling. Night sweats are convulsive syndrome. Excessive daytime sleep is convulsive syndrome.
*Mao Xian Sheng Fang* Song on Initial Signs of Pediatric Convulsive Syndrome:
There are many signs of convulsive syndrome; stating the root reveals difficulty. Grinding teeth in dreams indicates shock entering bones; abnormal eye issues belong to the Liver. Night crying until dawn ends with silent voice; black and blue faces share similar signs. Heat in the front/back chest and limbs; red face like blood points to the Heart. Night crying and daytime sweat cause fatigue; coarse breathing and drinking water indicate Lung pathogen disturbance. Strange sounds and frequent calling with dark form; craving to bite milk indicates Kidney pathogen origin. Mouth panting and fixed staring eyes; carefully observe this convulsive syndrome.
Song on Death Signs of Pediatric Convulsive Syndrome:
If a child gets convulsive syndrome, nails turn blue-black like smoke; mouth vomits white worms and black blood; eyes stay open or half-lift shoulders; biting people, daytime sweat, sudden crow-like sounds are unlookable.
Another Song on Death Signs of Pediatric Convulsive Syndrome:
Neck soft and weak, throat like a saw blade; red face with makeup color, eyes dark and lightless; fish-mouth open with coarse breath, legs and neck straight and long; chewing clothes randomly, bruised blood on bed; eyes open then close, whole body stiff as wood. Ten kinds of startling diseases, do not bother thinking further.
*Hui Ji* Song on Liver Exhaustion Signs from Long-standing Pediatric Convulsive Syndrome:
High fever, dizziness, unable to lift head; yellow eyes and dryness are worrying. Sometimes vomiting milk and turning over food; four limbs like ice, pain entering throat. Believe this originated from startling; lingering for days, what more to seek? Only because life ends returning to dust; pulse chaotic, thin and surging yet hook-like.
*Yang Sheng Bi Yong* states: Snake bones and teeth all have great toxicity; special caution is needed when harvesting them, as harming people often damages life. Silkworm (Jiang Can) remove silk mouth, chop, stir-fry until red. Snake skin roast. Cicada slough wash off dirt, remove head, wings, feet, bake. Snake wine soak, remove skin and bones, take meat, bake dry. Dung beetle remove head, wings, feet, roast until charred. Snake gall roast until red, quench in vinegar five times. Do not use dung beetles injured by water. (Often used in startling medicines, hence recorded here.)
