The Song Dynasty physician Liu Fang's *Youyou Xinshu* (New Book for Infants and Children), Chapter: Fifth on Startling Deficiency / Sixth on Convulsions (1/2)(1/2), Category: Comprehensive.
*Er Xiao Xing Zheng Lun* (Discussion on Infant Physical Manifestations) Verse on the 48th Syndrome of Startling Deficiency:
Long-term deficiency with teeth grinding and yellowish-green complexion; choking during suckling, high phlegm-saliva damaging Stomach Qi. Frequent crying, inability to sleep, upward rushing Qi, rapid breathing, and yellowish stool. Excess Wind leads to excessive phlegm and should trigger convulsions; changes occur frequently with irregular complexion. If startling deficiency recedes without other symptoms, as long as phlegm-saliva descends, the illness poses no harm.
This syndrome arises from deficiency leading to startling deficiency. When phlegm-saliva rises, it can be purged using Bianxian Wan (Phlegm-Transforming Pill), followed by tonifying deficiency and harmonizing Qi. If teeth grinding and crying occur, mild purging followed by multiple doses of medicines that unblock the Heart and circulate in the Small Intestine will lead to recovery; Anshen San (Calming the Spirit Powder) is suitable. (Formula see this section.)
*Bianxian Wan* (Phlegm-Transforming Pill) from *The 48 Syndromes*:
Yaxiao (Natrii Sulfas, 1 qian), Pengsha (Borax), Nanxing (Arisaemae Rhizoma, Praeparatum, half qian each), Fenshuang (Calomelas, half qian), Banxia (Pinelliae Rhizoma, 10 pieces, peeled and navel removed), Zhusha (Cinnabaris, 1 fen, wrapped in vinegar dough), Badou (Crotonis Fructus, as needed). Boil Badou together with Banxia and Zhusha, remove the beans, and use the paste.
Grind all into powder, form pills with Gaojiu paste (Gleditsiae Sinensis Extract) the size of mung beans. Administer 7 to 10 pills per dose to expel startling phlegm-saliva. Follow with Qi tonification. This medicine is used for any condition with phlegm-saliva.
*Anshen San* (Calming the Spirit Powder) from *The 48 Syndromes*, treating startling deficiency:
Renshen (Ginseng Radix et Rhizoma), Fuling (Poria), Zhusha (Cinnabaris) (each 1 qian), Zhenzhu (Margarita), Gancao (Glycyrrhizae Radix et Rhizoma, roasted, 3 cun), Chantui (Cicadae Periostracum, 7 pieces), Shexiang (Moschus), Xijiaoxie (Rhinocerotis Cornu, sliced) (each a little).
Grind into powder. Take 1 qian with Bohe Tang (Mentha Decoction); also treats night crying.
*Dinghun Tang* (Soul-Stabilizing Decoction) from *The 48 Syndromes*, treating startling deficiency:
Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Fuling (Poria) (each 1 qian), Suanzaoren (Ziziphi Spinosae Semen, half liang).
Grind into powder. Take 1 qian with Mi Yin (rice drink). If startled, take with Bohe Tang (Mentha Decoction).
*Woshe Wan* (Black Snake Pill) from *Zhao Family Collection*, treating infant startling due to deficiency:
Woshe Rou (Dermochelys Coriacea Meat, roasted), Quanxie (Scorpio), Shenqu (Massa Medicata Fermentata, fried), Baijiangcan (Bombyx Batryticatus) (each half liang), Xeshao (Scorpio tip, 1 fen), Lougu (Myrmecophila Thamnophila, finely ground, used after other medicines are powdered).
Grind into powder, form pills with Jiu Hu (wine paste) the size of millet grains. Take 20 pills per dose with Mi Yin (rice drink).
Qian Yi discusses Liver having Wind. Eyes blinking without convulsions; if Heart Heat arises, convulsions occur. Treat the Liver with Xieqing Wan (Draining Green Pill), treat the Heart with Daochi San (Directing Red Powder) as main formulas. (Xieqing Wan formula see Hot Startling section, Daochi San formula see Excess Heat section.)
Qian Yi discusses Liver having Heat. Eyes staring straight without convulsions; if Heart Heat arises, convulsions occur. Treat the Liver with Xieqing Wan (Draining Green Pill), treat the Heart with Daochi San (Directing Red Powder) as main formulas. (Formulas see same as above.)
Qian Yi discusses severe Liver Wind. Body arches back and becomes rigid without convulsions; the Heart does not accept heat. One should tonify the Kidney and treat the Liver. Tonify the Kidney with Dihuang Wan (Rehmannia Pill), treat the Liver with Xieqing Wan (Draining Green Pill) as main formulas. Any prolonged illness can draw out Liver Wind. Wind moves and stops at the head and eyes. The eyes belong to the Liver. When Wind enters the eyes, moving up, down, left, and right like wind blowing, neither light nor heavy, the child cannot bear it, causing eye blinking. If Heat enters the eyes, pulling the sinews, both eyes become tight and cannot turn to look, causing staring straight. If Heart Heat arises, convulsions occur because the mother and son organs both have excess heat, and Wind and Fire struggle. Treat the Liver with Xieqing Wan (Draining Green Pill), treat the Heart with Daochi San (Directing Red Powder) as main formulas. (Formulas see same as above.)
Qian Yi discusses convulsions in Jing Xian (Startling Epilepsy). In boys, convulsions with eyes looking left produce no sound; looking right produces sound. In girls, convulsions with eyes looking right produce no sound; looking left produces sound, due to mutual overcoming. There are also specific signs during onset.
Qian Yi discusses morning convulsions. Due to tidal fever, body burns strongly during Yin, Mao, Chen hours, eyes look up, hands and feet tremble, hot phlegm-saliva forms in the mouth, neck is stiff. This indicates Liver prosperity; one should tonify the Kidney and treat the Liver. Use Dihuang Wan (Rehmannia Pill) to tonify the Kidney, Xieqing Wan (Draining Green Pill) to treat the Liver as main formulas. (Dihuang Wan formula see Deficient Cold section, Xieqing Wan formula see Hot Startling section.)
Qian Yi discusses noon convulsions. Due to tidal fever, convulsions occur during Ji, Wu, Wei hours. Heart spirit is startled and palpitations arise, eyes look up, whites of eyes are red, jaw is tight, saliva flows in mouth, hands and feet tremble. This indicates Heart prosperity; one should tonify the Liver and treat the Heart. Treat the Heart with Daochi San (Directing Red Powder), Liangjing Wan (Cooling Startling Pill); tonify the Liver with Dihuang Wan (Rehmannia Pill) as main formulas. (Daochi San formula see Excess Heat section, Liangjing Wan formula see All Hot Startling section, Dihuang Wan formula see same as above.)
Qian Yi discusses evening convulsions. Due to tidal fever, during Shen, You, Xu hours, convulsions are slight but panting occurs. Eyes look slightly oblique, body seems hot, sleeps with eyes open, hands and feet are cold, stool is pale yellow water. This indicates Lung prosperity; one should tonify the Spleen and treat the Heart and Liver. Tonify the Spleen with Yihuang San (Regulating Yellow Powder), treat the Liver with Xieqing Wan (Draining Green Pill), treat the Heart with Daochi San (Directing Red Powder) as main formulas. (Yihuang San formula see Stomach Qi Disharmony section, Xieqing Wan and Daochi San formulas see same as above.)
Qian Yi discusses night convulsions. Due to tidal fever, during Hai, Zi, Chou hours, convulsions are slight but sleep is restless. Body is warm and hot, eyes are tightly oblique, phlegm in throat, stool is silver-brown, milk and food are undigested, sleeps much, fails to absorb fluids. One should tonify the Spleen and treat the Heart. Tonify the Spleen with Yihuang San (Regulating Yellow Powder), treat the Heart with Daochi San (Directing Red Powder), Liangjing Wan (Cooling Startling Pill) as main formulas. (Formulas see same as above.)
Qian Yi discusses convulsions after Wind Injury. Acquired after Wind Injury, hot breath from mouth, yawning and sudden stuffiness, hands and feet tremble. Should disperse externally, using Daqing Gao (Great Blue Ointment) as main formula. Infants born timid often suffer this disease. (Daqing Gao formula see Hot Startling section.)
Qian Yi discusses convulsions after Food Injury. Acquired after Food Injury, body is warm, excessive saliva and sleep, or vomiting and loss of appetite leading to convulsions. Should first stop convulsions. After convulsions subside, purge with Baibing Zi (White Cake Pills). Then take Anshen Wan (Calming Spirit Pill). (Baibing Zi formula see this section, Anshen Wan formula see Deficient Heat section.)
Qian Yi discusses convulsions within 100 days. True convulsions occurring no more than two or three times will result in death; false convulsions occurring frequently are not severe. True convulsions stem from internal Startling Epilepsy; false convulsions stem from external Wind-Cold injury. Because Qi and Blood are not solid, they cannot withstand and thus convulse. To know if it is false, check if hot breath comes from the mouth. Treatment can involve dispersing, using Daqing Gao (Great Blue Ointment) as main formula, and applying methods for smearing fontanelle and bathing body. (Daqing Gao formula see same as above, Smearing Fontanelle and Bathing Body method see this section.)
Qian Yi discusses distinguishing true and false convulsions: The grandson of Official Li suffered convulsions three to five times at 100 days old. Many doctors were invited; some diagnosed Tian Diao (Heaven Hook), others Tai Jing Xian (Fetal Startling Epilepsy), none effective. Later, Qian Yi administered Daqing Gao (Great Blue Ointment) the size of a soybean, one dose to induce sweating, then applied fontanelle smearing and bathing methods. He recovered in three days. Why? Infants are born with tender bones and muscles, injured by Wind, unable to withstand it, hence convulsions. Frequent convulsions are mild. Why? Pathogenic Qi is internal; every time it cannot withstand, convulsions occur. Rare convulsions indicate internal organ disease, incurable. Frequent convulsions should be treated by dispersing Wind-Cold, hence Daqing Gao (Great Blue Ointment). Do not overdose, as infants easily become deficient or excess; too much generates heat. One dose suffices. Smearing and bathing are essential, always effective.
Qian Yi discusses favorable and unfavorable convulsions: The son of Official Li, aged three, convulsed from Mao to Si hours, untreated by many physicians. Qian Yi was summoned. Convulsions with eyes looking right, loud crying. Li asked: Why convulse to the right? Qian Yi said: Unfavorable. Li asked: Why unfavorable? Qian Yi said: Males are Yang, originally starting left; females are Yin, originally starting right. If a boy looks left, no sound during convulsion; looks right, sound occurs. If a girl looks right, no sound; looks left, sound occurs. Left belongs to Liver, Right to Lung; Liver Wood, Lung Metal. Boy looking left means Lung overcomes Liver. Metal attacks Wood, two organs battle, hence sound. Treatment should drain the strong and tonify the weak; if Heart is excess, drain it. Lung deficiency must not be drained. Signs of Lung deficiency include restlessness, choking breath, long sighs. This is a male presenting female pathology, so males are easier to treat than females. Suppose a girl convulses, looking left; Lung overcomes Liver. If in Autumn, Lung is prosperous, Liver cannot withstand, hence crying. Must heavily drain Lung, then treat Heart and continue treating Liver. Both mention eyes staring straight because Liver governs the eyes. All convulsions are Wind-Heat struggling internally. Wind belongs to Liver, hence reflected in eyes. Qian Yi used Xiefei Tang (Draining Lung Decoction) to drain. After two days without restlessness, Lung disease recedes. Then take Dihuang Wan (Rehmannia Pill) three times to tonify Kidney, then Xieqing Wan (Draining Green Pill) and Liangjing Wan (Cooling Startling Pill) twice each. Using Heart and Liver draining medicines heals in five days, without reckless treatment. Why not heavily drain Lung deficiency? Qian Yi said: If a boy looks right, Wood attacks Metal, Liver overcomes Lung. If only draining Liver, and if in Spring/Summer when Metal Qi is extremely deficient, must tonify Lung, never drain.
Qian Yi discusses Heat inevitably causing Startling Convulsions: Grand Relative House Seventh Taivwei, age seven, had tidal fever for several days nearing cure. Qian Yi told his father Second Da Wang: Seventh Shi's tidal fever will heal, Eighth Shi must prevent Startling Convulsions. King angry: Just let Seventh Shi heal, do not speak of Eighth Shi's illness. Qian Yi said: Eighth Shi will have no suffering by next day noon. Next day before noon, indeed acute convulsions occurred. Summoned Qian Yi, healed in three days. Because he foresaw staring eyes and red cheeks, indicating Liver and Heart both hot. Sitting on stone stool shows desire for cold, indicating severe heat. Skin naturally fat, pulse rapid, hence inevitable Startling Convulsions. The timing mentioned, from Yin to Wu hours, are times when Heart and Liver are active. Treatment drains Heart and Liver, tonifies Kidney, naturally safe.
*Shibi Jing* (Stone Wall Classic) Verse on 36 Types of Startling Wind Convulsions:
Frequent crying, urgent convulsions without deviation, vomiting/diarrhea Wind transmission, Qi not spreading. Previously loved sleeping with slight sweat. (*Fengsui Jing* adds here: Originally look for slight tears in eyes, to find the source of the child's original illness. This is because Stomach Qi is abundant, milk/food excessive, plus object startling, causing vomiting or diarrhea. If mistreated, heavy daytime sleep leads to night crying, hands/feet convulse.) Rough breathing, urgent eyes naturally so; red face, illness not improving; loving to cry, phlegm-saliva gathers in heart; red veins on face, epilepsy arises. Chaotic lines grow on fingers, illness spreads. (*Fengsui Jing* says: Blue lines inside fingers, illness spreads.) Observe form and color carefully; spiritual medicine dispels Wind, words suffice. (Rough breathing fever should slightly cool Stomach Qi, then remove Startling syndrome, then dissipate phlegm-saliva. If phlegm, check for blue or purple-red threads on cheeks like snail knots; disease not fully effective, regulate Qi, then remove Startling Wind. Chaotic fish-spine lines on second finger of both hands treated similarly, regulate Qi to remove Startling. *Fengsui Jing* verse similar. Note: Give Wuxi Gao (Black Rhinoceros Horn Ointment) to lower phlegm, then Shengyin Wan (Raw Silver Pill), two formulas same as *Yu Jue*, see Acute/Chronic Startling Wind section.)
*Shibi Jing* (Stone Wall Classic) Verse on 36 Types of Acute Startling Deviated Convulsions:
First look at child's face green, then back cold as ice. Male convulsing left is fine; suddenly convulsing right is alarming. Female convulsing right still treatable; convulsing left need not hear. More fear palms sweating like water, frowning high abdomen how to survive. (*The 48 Syndromes* last two lines say: Fear phlegm-saliva entering Lung, knotting in heart, illness deepens.)
This syndrome caused by Cold Evil. Treatment first regulates Stomach Qi, then induces sweating. If back warms, treatable; if remains cold as before, untreatable. If palms sweat, smell foul, or cry incessantly, it is internal closure, untreatable. (*Fengsui Jing* verse same. Note: If favorable convulsion, give Shengyin Wan (Raw Silver Pill), Zhenxin Wan (Spirit-Calming Pill). Shengyin Wan formula see All Startling section.)
